Fatores prognósticos associados à sobrevida global e específica em pacientes com câncer de próstata no estado de São Paulo (Brasil) 2000-2022

Resumo: O objetivo deste trabalho foi analisar a sobrevida global e específica de pacientes com câncer de próstata, do banco de dados da Fundação OncoCentro (São Paulo, Brasil) em um seguimento de 22 anos (jan. de 2000 a dez. 2022). As curvas de sobrevida foram analisadas pelo método de Kaplan Meier utilizando testes de Log Rank para investigar o efeito da escolaridade, exames diagnósticos, tipos de tratamentos e outras variáveis sobre a sobrevida global (morte por qualquer causa) e específica (morte por câncer de próstata). Além dos modelos simples e múltiplos de regressão de Cox tempo dependente, para investigar o efeito das variáveis na sobrevida global. Um total de 115.956 pacientes foram incluídos, a idade média foi de 70 anos, cerca de 15,49% foram a óbito devido ao câncer de próstata e 18,50% por outras causas. Neste estudo foi possível verificar que faixa etária, PSA, escore de Gleason, estágio clínico, tratamento, recidiva regional e metástase foram fatores que influenciaram na sobrevida global e na sobrevida específica. Abstract: The objective of this study was to analyze the overall and specific survival of patients with prostate cancer using data from the OncoCentro Foundation database (São Paulo, Brazil) over a 22-year follow-up period (January 2000 to December 2022). Survival curves were analyzed using the Kaplan-Meier method, and Log Rank tests were employed to investigate the effect of education, diagnostic tests, types of treatments, and other variables on overall (death from any cause) and specific (death from prostate cancer) survival. Additionally, both simple and multiple time-dependent Cox regression models were utilized to investigate the effect of variables on overall survival. A total of 115,956 patients were included, with an average age of 70 years. Approximately 15.49% of the patients died due to prostate cancer, while 18.50% succumbed to other causes. This study revealed that age group, PSA levels, Gleason score, clinical stage, treatment methods, regional recurrence, and metastasis all played a role in influencing both overall and specific survival.
