Ações e financiamentos da terapia renal substitutiva durante a pandemia COVID-19 no Paraná : um estudo documental

Abstract

Resumo : A pandemia COVID-19 aumentou a demanda de tratamento de diversas patologias, como a doença renal. Diante disso, evidenciou-se a necessidade de novos investimentos nos sistemas de saúde para sustentar as demandas assistenciais voltadas à manutenção do tratamento dos pacientes com alterações renais. Este estudo objetivou identificar as ações e financiamentos direcionados à terapia renal substitutiva de pacientes com doença renal crônica durante a pandemia COVID-19 no Paraná. Trata-se de uma pesquisa documental realizada a partir de informações disponibilizadas em seis plataformas oficiais de saúde brasileiras. O levantamento foi realizado entre os meses de junho e julho de 2023. Foram selecionados dezessete documentos, sete eram referentes aos valores investidos na terapia renal substitutiva durante a pandemia COVID-19 no Paraná (41,18%) e dez abordavam somente as ações realizadas, durante o mesmo período (58,82%). O conteúdo foi organizado e analisado de acordo com a tríade de avaliação da qualidade em saúde de Avedis Donabedian "estrutura"; "processo"; "resultados". A categoria mais observada foi a "estrutura", dispondo de documentos atrelados à prestação de cuidados, estrutura física, equipamentos, recursos humanos, organizações e recursos financeiros. A partir da análise dos documentos, constata-se que os financiamentos e ações durante a pandemia foram centralizadas na implementação estrutural e organizacional, com enfoque na prevenção voltados a ambientes prestadores do serviço de hemodiálise, evidenciando a necessidade de recursos destinados a diálise peritoneal e transplante renal.

Abstract : The COVID-19 pandemic has heightened the demand for treating various pathologies, including kidney disease. Consequently, there is a pressing need for increased investments in health systems to meet the health care demands required to sustain the treatment of patients with kidney disorders. This study aims to identify actions and financing strategies targeting renal replacement therapy for patients with chronic kidney disease during the COVID-19 pandemic in Paraná State, Brazil. The research, conducted between June and July 2023, relied on documentary sources available on six official Brazilian health platforms. Out of the selected documents (N = 17 reports), seven addressed the amounts of funds invested in renal replacement therapy during the COVID-19 pandemic in Paraná (41.18%), while 10 focused solely on the actions implemented during the same period (58.82%). The information described in those documents were organized and detailed according to Avedis Donabedian’s framework to evaluate the quality of health care: 'structure,' 'process,' and 'results.' The most frequently observed category was 'structure,' encompassing documents related to the provision of care, physical infrastructure, equipment, human resources, organizations, and financial resources. Upon analyzing the documents, it is evident that financing and actions during the pandemic were centralized in structural and organizational implementation. There was a notable emphasis on preventive measures directed towards environments associated with the hemodialysis service. This work underscores the need for resources allocated to peritoneal dialysis and kidney transplant services.
