Arnica montana L. (Asteraceae) e cicatrização de feridas : uma revisão

Resumo: A espécie Arnica montana L. da família Asteraceae é conhecida popularmente como arnica, kraut, perdição do leopardo, espirrar e tabaco da montanha. Possui atividade antibacteriana, antifúngica, anti-inflamatória, antisséptica, antioxidante, imunomoduladora e anti-esclerótica. É utilizada em contusões, hematomas, alívio da dor e no pós-operatório, entretanto, há poucas evidências científicas sobre a sua eficácia. Sendo assim, esse trabalho teve como objetivo realizar uma revisão integrativa a respeito do uso de Arnica montana da família Asteraceae na cicatrização de feridas. A primeira busca foi realizada nas bases de dados Pubmed, Science Direct, Scielo, Capes e Lillacs utilizando a estratégia (arnica AND "cicatrización de herida") OR (arnica AND "wound healing"). Foram obtidos 555 artigos que após empregar os critérios de inclusão restaram 13 artigos. A arnica foi utilizada principalmente em formulação homeopática, em solução ou para uso tópico. Os ensaios avaliaram a cicatrização por meio de parâmetros de medição do edema, do tempo de cicatrização e análise microscópica tecidual/celular. O mecanismo de ação foi por meio de modulação da expressão gênica e estímulo da proliferação celular. Na segunda busca foi utilizada a base de dados Science Direct utilizando "Asteraceae AND wound healing" e "Asteraceae AND cicatrización de heridas" resultando 654 artigos, sendo que apenas 9 se enquadraram em todos os critérios de inclusão. Os estudos utilizaram extratos com aplicação tópica com um maior número de análise microscópica celular/tecidual. Dentre os 9 artigos, apenas 7 trouxeram o mecanismo de ação, a citar, a redução do estresse oxidativo, a capacidade de inibir a colagenase e a elastase, o aumento da taxa e migração dos queratinócitos, a ligação de compostos ao DNA do fator nuclear kappa B, a redução do infiltrado inflamatório e estímulo da angiogênese. As pesquisas a respeito da arnica e da família Asteraceae, apresentaram perfis diferentes, porém, demonstraram ação efetiva sobre a cicatrização de feridas. O presente trabalho evidenciou que são poucos os estudos experimentais a respeito do uso da Arnica montana na cicatrização de feridas. Abstract: The species Arnica montana L. from the Asteraceae family is popularly known as arnica, kraut, leopard doom, sneeze and mountain tobacco. It has antibacterial, antifungal, anti-inflammatory, antiseptic, antioxidant, immunomodulatory and anti-sclerotic activity. It is used in bruises, contusions, pain relief and in the postoperative period, however, there is little scientific evidence about its effectiveness. Therefore, this work aimed to carry out an integrative review regarding the use of Arnica montana from the Asteraceae family in wound healing. The first search was performed in Pubmed, Science Direct, Scielo, Capes and Lillacs databases using the strategy (arnica AND "cicatrización de herida") OR (arnica AND "wound healing"). A total of 555 articles were obtained, which left 13 articles after employing the inclusion criteria. Arnica was mainly used in homeopathic formulation in solution or for topical use. The assays evaluated healing through parameters measuring edema, healing time and tissue/cellular microscopic analysis. The mechanism of action was through modulation of gene expression and stimulation of cell proliferation. In the second search was used the Science Direct database using "Asteraceae AND wound healing" and "Asteraceae AND scaración de heridas" resulting in 654 articles, in which only 9 met all the inclusion criteria. It was used in the studies extracts with topical application with a greater number of cellular/tissue microscopic analysis. Among the 9 articles, only 7 brought the mechanism of action, to mention, the reduction of oxidative stress, the ability to inhibit collagenase and elastase, the increase in the rate and migration of keratinocytes, the binding of compounds to the DNA of the nuclear factor kappa B, the reduction of the inflammatory infiltrate and stimulation of angiogenesis. Research on arnica and the Asteraceae family showed different profiles, however, they showed effective action on wound healing. The present work showed that there are few experimental studies regarding the use of Arnica montana in wound healing.
