Avaliação in vivo do efeito de dentifrícios contendo fluoreto estanhoso sobre a composição do microbioma dental

Resumo: Antigamente, os benefícios do uso de dentifrícios durante a escovação dental eram restritos à limpeza e prevenção de cárie. Entretanto, as formulações atuais podem conter princípios ativos que além de contribuir para o controle mecânico também podem conter componentes que promovem o controle químico do biofilme ou do processo de des/remineralização . O efeito anti-cárie do fluoreto estanhoso é conhecido há mais de 50 anos e é proporcionado pela sua ação antibacteriana. Contudo, devido ao gosto adstringente e a formação de manchas extrínsecas na superfície dental, em 2000, o hexametafosfato de sódio foi adicionado pela primeira vez à sua formulação. Essa alteração foi realizada mantendo os benefícios do fluoreto estanhoso, tais como: efeitos anti-cárie, anti-placa, anti-gengivite e somando a estes benefícios, o agente estabilizador, hexametafosfato de sódio. Este componente promove a redução da hipersensibilidade dentinária e cálculo dental sem promover manchamento dental. O objetivo desse estudo foi verificar o efeito do fluoreto estanhoso sobre a microflora dental in vivo. Para isso, um estudo in vivo duplo-cego foi realizado por 63 dias. O estudo contou com a participação de 3 voluntários que usaram o dentifrício em duas fases distintas: controle e teste. O biofilme dental supragengival da superfície vestibular dos primeiros molares superiores foi coletado antes e após cada fase experimental e foi utilizado para identificação das bactérias através do sequenciamento do DNAr 16 S. Os resultados mostraram diferenças na composição dos gêneros bacterianos entre os três voluntários. O voluntário 2, apresentou ao longo das seis semanas maior abundância de Rothia, que representou 28,93% de todas as sequencias, seguido por Streptococcus (27,29%) e Ralstonia (15,53%). O voluntário 3, apresentou ao longo das seis semanas maior abundância de Streptococcus, (35,24%), seguido por Lautropia (10,35%) e Neisseria (8,78%). O voluntário 4, apresentou ao longo das seis semanas maior abundância de Streptococcus(16,21%), seguido por Escherichia-Shigella (7,50%) e Aggregatibacter (6,80%). Com base nos dados obtidos, concluiu-se que o fluoreto estanhoso foi capaz de alterar a microbiota residente, aumentando a prevalência de microrganismos pioneiros do biofilme dental. Assim, estudos devem ser realizados para verificar se o uso a longo prazo deste dentifrício não poderia favorecer o aparecimento de outras doenças. Abstract: In the past, the benefits of using toothpastes during dental brushing were restricted to cleaning and preventing tooth cavity. However, current formulations may contain active principles that, in addition to contributing to mechanical control, may also contain components that promote the chemical control of the biofilm or the des/remineralization process. The anti-caries effect of stannous fluoride has been known for more than 50 years because of its antibacterial action. However, due to the astringent taste and the formation of extreme spots on the dental surface, in 2000, hexametaphosphate was removed from its first time in its formulation. This change was made to maintain the benefits of stannous fluoride, such as: anti-plaque, anti-plaque, anti-gingivitis, adding to the benefits of hexametaphosphate of a stabilizing agent. This component promotes the reduction of dentin hypersensitivity and dental calculus without promoting dental staining. Thus, the study was done with the use of stannous fluoride and changes the dental microflora in vivo. The double-blind study was performed in vivo for 63 days. The study had the participation of 3 volunteers who used the dentifrice in two distinct phases: control and test. The supragingival dental biofilm of the buccal surface of the maxillary first molars was collected before and after each experimental phase and was used to identify the matrices by sequencing the 16S rDNA. An analysis of the results showed that differences were observed in the composition of the bacterial genera between the three volunteers. Volunteer 2, initiated during the month, was higher in Rothia, representing 28.93% of all sequences, followed by Streptococcus (27.29%) and Ralstonia (15.53%). The volunteer 3, compared to the disadvantaged Streptococcus, (35.24%), followed by Lautropia (10.35%) and Neisseria (8.78%). Voluntary 4 is higher than Streptococcus (16.21%), followed by Escherichia-Shigella (7.50%) and Aggregatibacter (6.80%). Therefore, studies must be carried out for the diagnosis of prolonged use of the dentifrice could not favor the emergence of other diseases.
