Projeto Medida Certa : prevenção da diabetes e hipertensão na Atenção Básica

Resumo: Este plano de intervenção apresenta ações preventivas no manejo da hipertensão arterial, aumento anormal e por longo período, da pressão que o sangue faz ao circular pelas artérias do corpo, e da diabetes mellitus tipo II, excesso crônico de glicemia no sangue, conhecido como resistência à insulina o que desencadeia uma série de complicações. Trata-se do resultado do Curso de Especialização em Atenção Básica da UFPR, financiado pelo UNA-SUS. Conforme prévio diagnóstico situacional em uma comunidade da área de abrangência de atuação de uma unidade básica de saúde do município de São Mateus do Sul-PR, percebeu-se grande número de usuários hipertensos e diabéticos. Dessa forma, foi desenvolvido um projeto para auxiliar na identificação e prevenção destas morbidades. Ocupou-se desde a preparo dos trabalhadores de saúde, assim como dos próprios usuários. O principal objetivo deste trabalho é diminuir a prevalência e incidência da hipertensão arterial e da diabetes mellitus tipo II do território da unidade de saúde, habilitando a equipe de saúde sobre os temas em foco. Para este plano utilizou-se o método de pesquisa-ação visando melhorar o acolhimento, identificação, prevenção dos casos de hipertensão arterial e diabetes mellitus II, assim como comorbidades associadas na comunidade. Para a equipe de saúde, ocorreram ações educativas através da participação de todos os profissionais de saúde da unidade. Os usuários receberam informações através da consulta médica. Como recursos educacionais foram desenvolvidos um panfleto (APÊNDICE A), e um banner (APÊNDICE B) educativo utilizados como suporte nas palestras e orientações. O objetivo do projeto está sendo atingido progressivamente, considerando-se o cronograma estabelecido para a intervenção. É fundamental o estabelecimento de estratégias efetivas para a redução do impacto da hipertensão arterial sistêmica e da DM-2. Para isso, é necessária maior atenção referente às estratégias de prevenção, sobretudo para as populações de maior risco de desenvolvimento destas doenças. Abstract: This intervention plan presents preventive actions in the management of arterial hypertension, abnormal and long-term increase in the pressure that the blood makes when circulating through the arteries of the body, and type II diabetes mellitus, a chronic excess of blood glucose, known as resistance insulin which triggers a series of complications. This is the result of the Specialization Course in Primary Care at UFPR, funded by UNA-SUS. According to previous situational diagnosis in a community in the area covered by a basic health unit in the city of São Mateus do Sul - PR, there are a large number of hypertensive and diabetic users. Thus, a project was developed to assist in the identification and prediction of these morbidities. It was concerned with the preparation of health workers, as well as the users themselves. The main objective of the work is to reduce the prevalence and incidence of arterial hypertension and type II diabetes mellitus in the territory of the health unit, enabling the health team on the topics in focus. For this plan, the action research method was used to improve reception, identification, prevention of cases of arterial hypertension and diabetes mellitus II, as well as associated comorbidities in the community. For the health team, educational actions took place through the participation of all health professionals in the unit. Users received information through medical consultation. As educational resources, a pamphlet (APPENDIX A), and an educational banner (APPENDIX B) were developed, used as a support in lectures and guidelines. The objective of the project is being progressively achieved, considering the schedule established for an intervention. It is fundamental for the establishment of effective goals to reduce the impact of systemic arterial hypertension and DM-2. For this, greater attention is paid to prevention strategies, especially for populations at higher risk of developing these diseases.
