Projeto de intervenção para melhorar a qualidade de vida dos pacientes com hipertensão arterial sistêmica

Resumo: A Hipertensão Arterial Sistêmica (HAS) é uma doença crônica, multifatorial, responsável por alterações funcionais e/ou estruturais de órgãos como coração, encéfalo, vasos sanguíneos e rins. Caracteriza-se pelos níveis elevados da pressão arterial (PA) com indicadores igual ou maior que 140 x 90 mmHg. O presente projeto de intervenção efetivou-se na Estratégia Saúde da Família (ESF) Linha Vitória, na cidade de Cruz Machado/PR, onde a equipe de profissionais da saúde busca realizar ações por meio da orientação e de informações oportunizando aos pacientes hipertensos conhecimentos e autonomia para a administração correta dos medicamentos, bem como, cuidados relacionados à saúde, à prática de atividade física e alimentação adequada, promovendo qualidade de vida. Este trabalho teve o objetivo geral de planejar e desenvolver ações de orientação e informação sobre os cuidados necessários para o controle da HAS por meio da utilização de medicamentos e da mudança do estilo de vida. O método caracteriza-se pela pesquisa-ação, com etapas definidas, tais como: exploratória, interesse do tema, definição do problema, fundamentação teórica, elaboração da proposta, implantação e avaliação do impacto. Para executar os objetivos propostos estabeleceram-se ações, como reunião de equipe para discussão e planejamento, elaboração de flyer orientativo sobre o tema e atividades de educação em saúde que serão realizadas em momento oportuno, após a pandemia. Os resultados ficarão disponíveis no Portal UNA-SUS, Acervo REA (recurso educacional aberto). As ações que compõem o plano de intervenção para o controle da HAS nos pacientes atendidos pela ESF- Linha Vitória foram planejadas em reunião realizada no dia 04 de novembro de 2020 nas dependências da ESF, com os profissionais da saúde que nela atuam. Envolverão orientações e informações sobre a HAS aos pacientes, acompanhamentos individuais e domiciliares, bem como manutenção das práticas já exercidas na ESF sobre o controle da HAS. Enfatizou-se a necessidade de orientar os pacientes sobre o uso contínuo dos medicamentos, nos horários determinados, bem como o comparecimento às consultas agendadas seguindo os protocolos de prevenção da COVID-19. Os pacientes, num total de 270 diagnosticados com HAS, estão sendo atendidos em consultas médicas e de enfermagem, com orientação, prescrição de medicamentos e/ou renovação de receitas. Recebem atendimento domiciliar, quando necessário, em conformidade com a agenda da ESF, realizados pelos profissionais da equipe. O número de pacientes atendidos e acompanhados no período de 01/11/2020 a 30/11/2020 considerando o número de consultas foi de 415. Os profissionais sugeriram manter atenção em relação aos pacientes com HAS, visto que, devido à pandemia as reuniões periódicas estão suspensas e por este motivo oportunizar as informações e orientações de maneira individual, por meio do atendimento na ESF, nas visitas domiciliares e material impresso (flyer) com linguagem acessível para maior compreensão e adesão ao tratamento da HAS. Este plano de intervenção está gerando impacto para os usuários, suas famílias, comunidade em geral e equipe de saúde, à medida que, promove o controle da PA. Abstract: Systemic Arterial Hypertension (SAH) is a chronic, multifactorial disease, responsible for functional and / or structural changes in organs such as heart, brain, blood ves- sels and kidneys. It is characterized by high blood pressure (BP) levels with indica- tors equal to or greater than 140 x 90 mmHg. The present intervention project was carried out in the Family Health Strategy (FHS) Linha Vitória, in the city of Cruz Ma- chado / PR, where the team of health professionals seeks to carry out actions throu- gh guidance and information, providing hypertensive patients with knowledge and autonomy for the correct administration of medicines, as well as care related to heal- th, physical activity and adequate nutrition, promoting quality of life. This work had the general objective of planning and developing actions of guidance and information on the necessary care for the control of SAH through the use of medications and li- festyle changes. The method is characterized by action research, with defined steps, such as: exploratory, interest of the theme, definition of the problem, theoretical foundation, elaboration of the proposal, implementation and impact assessment. In order to carry out the proposed objectives, actions were established, such as team meeting for discussion and planning, preparation of an orientation flyer on the theme and health education activities that will be carried out in due time, after the pandemic. The results will be available on the UNA-SUS Portal, REA Collection (open educatio- nal resource). The actions that make up the intervention plan for the control of SAH in patients treated by the FHS-Linha Vitória were planned in a meeting held on No- vember 4, 2020 at the ESF facilities, with the health professionals who work in it. They will involve guidance and information about SAH to patients, individual and ho- me follow-up, as well as maintenance of the practices already exercised in the FHS on the control of SAH. The need to guide patients on the continuous use of medicati- ons at specific times was emphasized, as well as attending scheduled appointments following the prevention protocols of COVID-19. A total of 270 patients diagnosed with SAH are being seen in medical and nursing consultations, with guidance, pres- cription of medications and / or renewal of prescriptions. They receive home care, when necessary, in accordance with the FHS agenda, carried out by the team's pro- fessionals. The number of patients seen and followed in the period from 11/01/2020 to 11/30/2020 considering the number of consultations was 415. The professionals suggested maintaining attention in relation to patients with SAH, since, due to the pandemic, meetings periodicals are suspended and, for this reason, provide informa- tion and guidance individually, through attendance at the FHS, home visits and prin- ted material (flyer) with accessible language for greater understanding and adheren- ce to the treatment of SAH. This intervention plan is having an impact on users, their families, the community in general and the health team, as it promotes BP control.
