Abstract: The intervention plan that addresses the issue of prevention of chronic non- communicable diseases is a result of the Specialization Course in Primary Care at UFPR, funded by UNA-SUS. The elaboration of the proposal includes the prioritized problem, which is the large number of patients affected by chronic diseases and the consequence of this in the quality of life of the individual, in addition to the low adherence to the control and treatment of risk factors or the disease itself, which directly influence the resolution of the tables. The general objective of the work is to implement the "Health Promotion Day" to promote the reduction of chronic diseases and their complications. The specific objectives are to raise awareness and guide individuals about the need for self-care and how the choices and attitudes made now determine the future. The action research includes an event where patients will first be screened, where data such as blood pressure, capillary blood glucose and BMI values will be collected, after which a medical consultation will be held where routine laboratory tests will be requested and will also take place during the event the presentation of lectures on topics that address the prevention of chronic non- communicable diseases, such as the importance of a healthy diet and the practice of physical exercises, for this action educational resources such as posters and folders will be used. The results obtained with this action will only be seen in the long term, so that a comparative assessment of the clinical improvement and the evolution of the patients can be made, so the results and the results achieved will not be shown, but it is expected to be positive, identifying a improvement in the community's standard of living. One of the limitations of this action plan is that the results depend for the most part on the collaboration of patients who need to commit to the stipulated plan and need to put into practice the change in life habits. it is expected that this action will be a model for similar future actions and that it will contribute to the residents of the community, to the team and their managers.
