Memória de trabalho verbal no TDAH : revisão sistemática e meta-análise

Resumo: Pesquisas científicas têm identificado reiteradamente que crianças e adolescentes com TDAH apresentam déficits em memória de trabalho. O objetivo desta revisão sistemática com síntese quantitativa de dados é investigar primariamente o effect size do déficit em memória de trabalho verbal de crianças e adolescentes com TDAH, cuja avaliação (neuro)psicológica tenha aplicado o subteste Dígitos e/ou sua ordem indireta, com base em alguma versão das escalas WISC e/ou WAIS. Secundariamente, a memória de curto prazo foi investigada considerando-se os escores da forma direta do subteste. A busca sistematizada de artigos científicos foi feita nas bases de dados PubMed/MEDLINE, PsyINFO, PsyArticles e Web of Science, nos meses de junho e julho de 2015. O período de interesse abrange os anos de 2005 a 2014. A revisão sistemática gerou uma amostra de 5064 sujeitos com TDAH e outra amostra de 3867 sujeitos sem TDAH. Os resultados indicam que, em memória de trabalho verbal, crianças e adolescentes com TDAH apresentam déficits de magnitude moderada (Hedges's g = 0,55), enquanto em memória de curto prazo o effect size estimado foi de magnitude média a baixa (g = 0,46). Esses resultados replicam em grande parte os resultados de estudos anteriores de meta-análise que se valeram de variadas medidas de memória de trabalho verbal. As evidências das sínteses quantitativas sugerem que o subteste Dígitos, mais especificamente a ordem indireta, é de fato uma medida de memória de trabalho verbal, algo não compartilhado por todos os pesquisadores ao longo de 100 anos de pesquisas. A principal explicação para o déficit em sujeitos com TDAH provém, sobretudo, de evidências empíricas de que circuitos fronto-parieto-temporais (córtex pré-frontal dorsolateral, sobretudo), importantes para o funcionamento da memória de trabalho, estão deficitários em crianças e adolescentes diagnosticados com TDAH. Conclui-se que o subteste Dígitos possui eficácia semelhante ao de outras medidas neuropsicológicas na investigação da memória de trabalho verbal. Entretanto, como há divergências na literatura científica sobre o assunto, sempre que possível é recomendável o emprego de outros instrumentos, conjuntamente, para avaliação da memória de trabalho verbal. Palavras-chave: Meta-análise. TDAH. Memória de Trabalho. Subteste Dígitos. Abstract: Scientific research has shown repeatedly that children and adolescents with ADHD present working memory deficits. The goal of this review of quantitative synthesis of data is twofold. First, the review investigates the effect size of verbal working memory deficits in children and adolescents with ADHD whose neuro(psychological) assessment used Digit Span and/or Digit Span Backward, based on some version of WISC or WAIS scales. Second, the review investigates short-term memory using scores from Digit Span Forward. The systematic search of scientific articles was carried out in June and July of 2015 using the PubMed/MEDLINE, PsyINFO, PsyArticles and Web of Science databases. The period of interest spans the years 2005 to 2015. The systematic review generated samples of 5064 individuals with ADHD and 3867 individuals without ADHD. The results show that children and adolescents with ADHD have medium effect size deficits in verbal working memory (Hegdes's g = 0.55), whereas in short-term memory the estimated effect size was medium to low (Hegdes's g = 0.46). These results largely replicate the results from previous meta-analysis studies that used several other verbal working memory measures. Evidences from quantitative syntheses suggest that Digit Span, more specifically the Digit Span Backward, is in fact a verbal working memory measure, although this theory has not been shared by all reaserchers over the past 100 years of research. The main explanation for the deficit in subjects with ADHD comes from empirical evidence that fronto-parieto-temporal circuitries (mainly dorsolateral prefrontal cortex) crucial for the proper functionting of working memory are impaired in children and adolescents with ADHD. In summary, this study indicates that Digit Span has an efficacy in the investigation of verbal working memory which is comparable to other neuropsychological measures. However, since there is no consensus in the scientific literature regarding the subject, whenever possible the use of other measures is recomménded in conjunction with Digit Span for assessment of verbal working memory.
