Concentrações de adiponectina : influência da aptidão física, dos fatores cardiometabólicos, genéticos e diferentes tipos e intensidades de treinamentos físicos em adolescentes

Resumo: A adiponectina é a proteína mais abundante produzida pelo tecido adiposo e tem concentração reduzida em indivíduos obesos, porém apresenta resultados controversos em relação ao nível de atividade física e aptidão física. Além disso, a relação entre polimorfismos genéticos e a concentração da adiponectina ainda não foi analisada na população infantojuvenil, assim como a comparação dos efeitos de intervenções com diferentes tipos e intensidades de exercícios físicos sobre os fatores que modulam a concentração da adiponectina e seu papel nos fatores de risco cardiometabólicos (FRCM). Portanto, o objetivo geral foi verificar a interação e influência da atividade física, aptidão física, FRCM e o polimorfismo de nucleotídeo único (SNP) do gene ADIPOQ, bem como os efeitos de diferentes programas de exercícios físicos sobre as concentrações de adiponectina em adolescentes. Desta forma, a tese é composta por quatros objetivos específicos e integrados, cujos resultados foram apresentados em quatro estudos: Estudo 1 - verificar a relação e o papel mediador da adiposidade corporal, FRCM e aptidão física com a concentração da adiponectina em adolescentes; Estudo 2 - analisar o papel moderador da adiposidade corporal, atividade física e aptidão física na relação entre os FRCM e a concentração da adiponectina em adolescentes; Estudo 3 - examinar o papel moderador da adiposidade corporal e da aptidão física na relação entre os genótipos do SNP 276G>T (rs1501299), os FRCM e as concentrações da adiponectina em adolescentes; Estudo 4 - investigar o efeito e a prevalência de responsividade após intervenções de diferentes tipos e intensidades de programas com exercícios físicos na concentração da adiponectina, na adiposidade e composição corporal, na aptidão física e nos FRCM em adolescentes. Foram utilizados bancos de dados oriundos de projetos realizados pelo Núcleo de Qualidade de Vida da Universidade Federal do Paraná. Participaram desses estudos 255 adolescentes, com idade entre 11 e 17 anos, de ambos os sexos, avaliados quanto as variáveis antropométricas de massa corporal (MC), estatura (EST), circunferência abdominal (CA), circunferência de cintura (CC), massa gorda (MG), massa livre de gordura (MLG), variáveis metabólicas de colesterol total (CT), lipoproteína de alta densidade (LDL-c), lipoproteína de baixa densidade (HDL-c), triglicerídeos (TG), glicose (GLI) e insulina (INS), variáveis pró e anti-inflamatórias (proteína C-Reativa (CRP) e adiponectina), pressão arterial sistólica (PAS), diastólica (PAD) e pressão arterial média (PAM), aptidão cardiorrespiratória (ACR - VO2pico absoluto e VO2pico_MC relativo à MC), força máxima (RM), força de preensão manual direita (PMD), esquerda (PME) e preensão manual média (PMM), resistência abdominal (ABDO), atividade física (NAF - questionário recordativo de gasto energético). Foram calculados índice de massa corporal (IMC), IMC escore-z (IMC-z), índice de massa triponderal (IMT), relação cintura/estatura (RCEst), modelo de avaliação da homeostase da resistência à insulina (HOMA-IR), modelo de avaliação da homeostase - adiponectina (HOMA-AD) e índice quantitativo de verificação de sensibilidade à insulina (QUICKI). Os adolescentes participaram das intervenções em diferentes programas de treinamento físico: um grupo de treinamento combinado (exercício aeróbio + exercício resistido (TC), n=19), três grupos de treinamento intervalado de alta intensidade (HIIT) em três locais distintos (em quadra esportiva (HIIT-QUA, n=18), meio aquático (HIIT-AQ, n=16) e ciclismo indoor (HIITCI, n=13)), um grupo de treinamento aeróbio de intensidade moderada (MICT-CI, n=16) e um grupo controle (GC, n=44). Os programas foram realizados em três sessões semanais, cada sessão com duração aproximada de 50 minutos e com tempo de intervenção de 12 semanas. A análise estatística foi conduzida no SPSS versão 24.0 e o nível de significância adotado foi de p?0,05. Resultados do Estudo 1 - a concentração da adiponectina foi semelhante entre as categorias de IMC-z (p=0,10) e NAF/IMC-z (p=0,13). Na categorização em grupos quanto à concentração de adiponectina, o grupo baixa concentração (GBC) apresentou maior IMC, IMC-z, IMT, CA, CC, RCEst, INS, HOMA-IR e HOMA-AD e menor força de RM supino (p<0,05) do que os grupos de intermediária (GIC) e alta concentração de adiponectina (GAC), enquanto nas variáveis de CT, LDL-c, TG e frequência cardíaca de repouso (FCrep) melhores resultados foram apresentados pelo GIC (p<0,05). Foram observados menores valores de QUICKI, VO2pico, RM leg press e PMD no GAC (p<0,05). Não foram encontradas relações significativas entre aptidão física e as concentrações de adiponectina, bem como a adiposidade corporal não exerceu papel mediador na relação entre as variáveis de resistência e sensibilidade insulínica e a adiponectina. Resultados do Estudo 2 - a ACR foi moderadora na interação entre IMC, IMC-z, IMT, CA, MG, INS, HOMA-IR, QUICKI e as concentrações de adiponectina (p<0,05); a aptidão muscular (APM) exerce papel moderador na relação do IMC, IMC-z, CA, CC, RCEst, MG, PAD, TG e a adiponectina (p<0,05), o IMC-z, foi moderador na interação da ACR, INS, HOMA-IR e a concentração da adiponectina (p<0,05); e o NAF-coef exerce papel moderador na associação entre IMC, IMT, MG, LDL-c, INS, HOMA-IR, QUICKI e as concentrações de adiponectina (p<0,05). Resultados do Estudo 3 - na relação entre os genótipos do SNP 276G>T e a concentração da adiponectina não foi encontrada interação significativa com variáveis moderadoras; a ACR foi moderadora na relação entre SNP 276G>T e MG e GLI; bem como, a PMM exerce papel moderador na relação entre SNP 276G>T e IMC, IMC-z, IMT, CA, CC, RCEst, MG, HDL-c e QUICKI; enquanto o IMC-z foi moderador na interação do SNP 276G>T e a APM (PMM e ABDO). Resultados do Estudo 4 - para a concentração da adiponectina não foram encontradas mudanças significativas médias e o efeito clínico sugere mudanças triviais dos grupos de exercício e controle após intervenção. Os grupos TC e HIIT-QUA apresentaram maiores mudanças significativas médias, com efeitos possivelmente benéficos e benéficos e maiores frequências de respondentes nos parâmetros antropométricas e composição corporal. Em relação aos FRCM, foram verificadas maiores mudanças significativas médias, sugerindo efeito clínico possivelmente benéfico e benéfico e maiores frequências de respondentes no TC e HIIT-QUA. Na ACR, os grupos de exercícios físicos (exceto HIIT-AQ) demonstraram mudanças médias significativas, sugerindo efeito benéfico e muito benéfico e maior frequência de respondentes, bem como, na APM apresentaram mudanças médias significativas, efeito clínico benéfico e muito benéfico e maior frequência de respondentes. Além disso, no GC foram observadas maiores chances de nãorespondentes em todas as variáveis analisadas. Considerações finais - conclui-se que as concentrações de adiponectina podem variar de forma não linear, conforme o grau de obesidade, do processo inflamatório e dos níveis de aptidão física. Observamos que há maiores concentrações de adiponectina quando os níveis de adiposidade corporal, atividade física e aptidão física são adequados, indicando que a prática regular de exercícios físicos pode exercer papel determinante na relação com a adiposidade e o processo inflamatório. Abstract: Adiponectin is the most abundant protein produced by adipose tissue and has reduced concentration in obese individuals but presents controversial results in relation to the level of physical activity and physical fitness. Moreover, the relationship between genetic polymorphisms and adiponectin concentration has not yet been analyzed in children and adolescents, as well as the comparison of the effects of interventions with different types and intensities of physical exercise on the factors that modulate adiponectin concentration and its role in cardiometabolic risk factors (CMRF). Therefore, the purpose of this study was to verify the interaction and influence of physical activity, physical fitness, CMRF and single nucleotide polymorphism (SNP) of the ADIPOQ gene, as well as the effects of different exercise programs on adiponectin concentration in adolescents. Thus, the thesis is composed of four specific and integrated objectives, whose results were presented in four studies: Study 1 - verify the relationship and the mediating role of body adiposity, CMRF and physical fitness with adiponectin concentration in adolescents; Study 2 - examine the moderating role of body adiposity, physical activity and physical fitness in the relationship between CMRF and adiponectin concentration in adolescents; Study 3 - examine the moderating role of body adiposity and physical fitness in the relationship between SNP 276G>T genotypes (rs1501299), CMRF and adiponectin in adolescents; Study 4 - investigate the effect and prevalence of responsiveness after interventions of different types and intensities of physical exercise on adiponectin concentration, physical fitness and CMRF in adolescents. We used databases from projects conducted by the Center for Quality of Life at the Federal University of Paraná. Participated in these studies 255 adolescents, aged between 11 and 17 years, of both sexes, evaluated regarding anthropometric variables of body mass (BM), height, abdominal circumference (AC), waist circumference (WC), fat mass (FM), fat-free mass (FFM), metabolic variables of total cholesterol (TC), high density lipoprotein (LDL-c), low density lipoprotein (HDL-c), triglycerides (TG), glucose (GLU), insulin (INS); pro- and anti-inflammatory variables (adiponectin and C-Reactive protein), systolic (SBP), diastolic (DBP) and mean (MBP) blood pressure, cardiorespiratory fitness (CRF - VO2peak absolute and VO2peak_BM relative to BM); maximal strength (MS), right handgrip strength (HGS-right), left handgrip strength (HGS-left) and mean (HGS-mean), abdominal endurance (ABDO); level of physical activity (PA - energy expenditure recall questionnaire). Body mass index (BMI), BMI score-z (BMI-z), triponderal mass index (TMI); waist-to-height ratio (WHtR); homeostasis model to assessment insulin resistance (HOMA-IR); homeostasis model to assessment - adiponectina (HOMA-AD) and quantitative insulin sensitivity check index (QUICKI) were calculated. The adolescents participated in interventions in different physical training programs: a combined training group (aerobic + resistance (AT+RT), n=19); three groups of high intensity interval training (HIIT) in three different locations (multi-sport game courts (HIIT-CO, n=18), aquatic environment (HIIT-AQ, n=16) and indoor cycling (HIIT-IC, n=13); a moderate intensity aerobic training group (MICT-IC, n=16) and a control group (CG, n=44). Programs were conducted in three weekly sessions, lasting approximately 50 minutes and with an intervention time of 12 weeks. Statistical analysis was conducted in SPSS version 24.0 and the significance level adopted was p?0.05. Results of Study 1 - adiponectin concentration was similar between BMI-z (p=0.10) and PA/BMI-z (p=0.13) categories. When categorizing into groups regarding adiponectin concentration, the low adiponectin group showed higher BMI, BMI-z, TMI, AC, WC, WHtR, insulin, HOMA-IR, HOMA-AD, and lower MS supine strength (p<0.05), than the intermediate and high adiponectin grupos, while in the variables of TC, LDL-c, TG, and resting heart rate (HRR) better results were presented by the intermediate concentration group (p<0.05). Lower QUICKI, VO2peak, MS leg press and HGS-right values were found in the high adiponectin group (p<0.05). No significant relationships were found between physical fitness and adiponectin, as well as adiposity did not play a mediating role in the relationship between insulin resistance and sensitivity variables and adiponectin concentrations. Results of Study 2 - CRF was a moderator in the interaction between BMI, BMI-z, TMI, AC, FM, INS, HOMA-IR, QUICKI and adiponectin (p<0.05); muscle fitness (MF) plays a moderating role in the relation of BMI, BMI-z, AC, WC, WHtR, FM, DBP, TG and adiponectin (p<0.05); BMI-z was a moderator in the interaction of CRF, INS, HOMA-IR and adiponectin (p<0.05); and PA-coef played a moderating role in the association between BMI, TMI, FM, LDL-c, INS, HOMA-IR, QUICKI and adiponectin (p<0.05). Results of Study 3 - in the relationship between SNP 276G>T genotypes and adiponectin, no significant interaction with moderating variables was found; CRF was a moderator in the relationship between SNP 276G>T and FM and GLU; HGSmean plays a moderating role in the interaction between SNP 276G>T and BMI, BMIz, TMI, AC, WC, WHtR, FM, HDL-c and QUICKI; and BMI-z was a moderator in the interaction of SNP 276G>T and HGS-mean. Results of Study 4 - for adiponectin concentration no mean significant changes were found, and the clinical effect suggests trivial changes in exercise and control groups after intervention. However. The AT+RT and HIIT-CO groups showed higher mean significant changes, with possibly beneficial and beneficial effects and frequency of responders in anthropometric variables and body composition. Regarding CMRF, greater mean significant changes, suggesting possibly beneficial and beneficial clinical effect and higher frequency of respondents in the AT+RT and HIIT-CO groups. In CRF, the exercise groups (except HIIT-AQ) showed significant mean changes, suggesting beneficial and very beneficial effect and with higher frequency of respondents, and in MF, they showed significant mean changes, beneficial and very beneficial clinical effect, and higher frequency of respondents. Moreover, in the CG were observed higher chances of non-responders in all variables analyzed. Final considerations - We conclude that adiponectin concentrations may vary in a non-linear fashion according to the degree of obesity, the inflammatory process, and physical fitness levels. We observed that there are higher concentrations of adiponectin when the levels of body adiposity, physical activity and physical fitness are adequate, indicating that the regular practice of physical exercises may play a determining role in the relationship with adiposity and the inflammatory process.
