Plano de ações em saúde para adesão do usuário hipertenso ao tratamento

Resumo: O plano de intervenção que aborda as ações em saúde para adesão do usuário hipertenso a terapêutica e está vinculado ao curso de especialização em atenção básica da UFPR, financiado pelo UNA-SUS. O problema identificado na análise situacional foi a alta prevalência para a hipertensão arterial sistêmica e a ocorrência de agravos frente ao seu descontrole gerando o aumento nos indicadores de saúde sobre a morbimortalidade para doenças cardiovasculares. A hipertensão arterial sistêmica consiste em uma doença de alta prevalência na população a mesma é considerada como um dos principais problemas de saúde. Além disso, é um fator de risco coronário e doenças cérebro vascular. Definiu-se os objetivos: elaborar um plano de intervenção para a abordagem ao usuário hipertenso na adesão ao tratamento e prevenir os agravos; reduzir as complicações decorrentes da hipertensão arterial por meio do controle da HAS; ppromover atividades educativas, orientando os hipertensos sobre a mudança de estilo de vida, hábito alimentar e cuidado à saúde; orientar familiares e pacientes sobre o manejo das medicações de uso contínuo. Trata-se de uma pesquisa-ação e o cenário do estudo foi a unidade básica de saúde vila celeste, na área rural do município de Santa Helena, estado do Paraná. Os participantes compreendem 53 usuários do território que possuem hipertensão arterial sistêmica de difícil controle, com faixa etária entre 43 e 80 anos. Foram planejadas intervenções e observou-se a adesão ao tratamento pela maioria dos participantes, com perda de peso em alguns dos hipertensos obesos e efetivo controle da hipertensão. Considera-se que, o plano de ações "Ações em Saúde para adesão do usuário hipertenso ao tratamento teve impactos positivos, visto que os usuários estão mais informados sobre a hipertensão além de terem mais autonomia para o cuidado á saúde, cabe destacar que ocorreu a melhora na abordagem ao usuário o que fez com que os objetivos propostos fossem contemplados, nos atendimentos verifica-se que os hipertensos encontram-se com a PA controlada e já é visível a perda de peso em alguns dos participantes hipertensos. Desta forma considera-se que as ações implantadas forma exitosas e esperamos em longo prazo reduzir os agravos decorrentes da HAS descompensada. Como recomendação manteremos as ações na rotina de trabalho possibilitando reforçar a abordagem para o autocuidado á saúde. Abstract: The intervention plan that addresses health actions for the adherence of hypertensive users to therapy and is linked to the specialization course in primary care at UFPR, financed by UNA-SUS. The problem identified in the situational analysis was the high prevalence of systemic arterial hypertension and the occurrence of problems due to its lack of control, generating an increase in health indicators on morbidity and mortality for cardiovascular diseases. Systemic arterial hypertension is a disease of high prevalence in the population, which is considered one of the main health problems. In addition, it is a coronary risk factor and vascular brain disease. The objectives were defined: to elaborate an intervention plan to approach the hypertensive user when adhering to treatment and to prevent diseases; reduce complications resulting from arterial hypertension through the control of SAH; ppromote educational activities, guiding hypertensive patients about changing their lifestyle, eating habits and health care; advise family members and patients on the management of medications for continuous use. It is an action research and the study scenario was the basic health unit vila celeste, in the rural area of the municipality of Santa Helena, state of Paraná. Participants comprise 53 users in the territory who have systemic arterial hypertension that is difficult to control, aged between 43 and 80 years. Interventions were planned and adherence to treatment was observed by most participants, with weight loss in some of the obese hypertensive patients and effective control of hypertension. It is considered that the action plan "Actions in Health for the adherence of hypertensive users to treatment had positive impacts, since users are more informed about hypertension in addition to having more autonomy for health care, it is worth mentioning that there was a improvement in the approach to the user, which meant that the proposed objectives were contemplated, in the consultations, it appears that the hypertensive patients have their BP under control and weight loss is already visible in some of the hypertensive participants. Thus, it is considered that the actions implemented were successful and we hope in the long term to reduce the problems resulting from decompensated SAH. As a recommendation, we will keep the actions in the work routine making it possible to reinforce the approach to self-care to health.
