Oxidação máxima de gorduras e perfil cardiometabólico em adolescentes com excesso de peso : interação entre polimorfismos genéticos e treinamentos físicos

Resumo: O objetivo principal da presente tese foi analisar o efeito do treinamento com exercícios físicos sobre a oxidação máxima de gorduras (FATMAX) e o perfil cardiometabólico em adolescentes com excesso de peso, bem como investigar a influência de polimorfismos genéticos nos genes ADRB2 e ADRB3. Para isto, três estudos distintos foram conduzidos, sendo que no Estudo 1 o objetivo foi comparar a influência de dois protocolos de treinamento com exercícios físicos sobre a oxidação máxima de gorduras e o perfil cardiometabólico em adolescentes com excesso de peso. No Estudo 2 investigaram-se as mesmas variáveis do primeiro estudo, acrescidas de análise de possíveis interações genéticas com o polimorfismo Gln27Glu do gene ADRB2 e no Estudo 3 com o Trp64Arg do gene ADRB3. A amostra total foi composta por 203 adolescentes com excesso de peso, de ambos os sexos, com idade entre 10 e 17 anos. As avaliações foram realizadas na fase inicial e após 12 semanas dos programas de intervenção. Os adolescentes foram submetidos às análises clínicas, que mensuraram estatura, massa corporal, circunferência abdominal (CA), estágio puberal, pressão arterial sistólica (PAS) e diastólica (PAD). Calcularam-se índice de massa corporal (IMC) e IMC-escore Z. Foram dosadas as concentrações de insulina basal (INS), glicemia basal (GLI), colesterol total (CT), lipoproteína de alta densidade (HDL-c), lipoproteína de baixa densidade (LDL-c), lipídeos totais (LT) e triglicerídeos (TG). A composição corporal foi obtida por impedância bioelétrica (BIA). Os testes de aptidão física foram compostos pela avaliação cardiorrespiratória realizada por meio de teste incremental em esteira ergométrica e pelo teste de força máxima (1RM). A partir do valor de consumo máximo de oxigênio direto (VO2max) houve a classificação do perfil cardiorrespiratório e cálculo da FATMAX. No segundo e terceiro estudos foram acrescentadas as avaliações genéticas, na qual a genotipagem dos polimorfismos foi realizada pelo método de PCR com Taqman. A amostra foi estratificada em dois grupos de intervenção com protocolos distintos e um grupo controle sem exercícios (GControle). O grupo treinamento aeróbio contínuo (GTAContínuo) foi composto de sessões de 90 min de exercício aeróbico (45 min de ciclismo indoor e 45 min de caminhada) mais 20 min de alongamento, totalizando 110 minutos. O grupo treinamento combinado (GTCombinado) consistiu em 10 minutos de aquecimento, 6 exercícios de resistência (3 séries; 6-10 rep; 30 minutos), seguidos por 30 min de exercícios aeróbios (caminhada/corrida), totalizando 70 min/sessão. A frequência semanal dos grupos intervenção foi de três vezes, por um período de 12 semanas, enquanto que a intensidade dos exercícios aeróbios dos protocolos foi progressiva de 35 a 85% do VO2max ou FCRESERVA. Utilizou-se teste de T de Student ou o Teste U - Mann Whitney para comparar os adolescentes de acordo com sexo e a análise da variância (ANOVA) para avaliar os efeitos do tempo e a interação grupo x tempo. Para as principais variáveis que apresentaram efeito significativo no grupo treino após 12 semanas foi realizado teste t pareado e calculado o Cohen's d. O nível de significância adotado foi p?0,05. Resultados Estudo 1: ao dividir a amostra de acordo com o sexo, os meninos apresentaram maior estatura (p<0,01), MLG (p<0,01), %MLG (p<0,01), QUICKI (p<0,01), VO2max absoluto (p<0,01) e FATMAX (p<0,01), enquanto que as meninas possuíram maior RCEst (p<0,01), %MG (p<0,01) e TG (p=0,04). Após 12 semanas, entre as meninas, a interação fator grupo x tempo indicou no GTAContínuo maior redução de MC (p<0,01), IMC (p<0,01), IMC-z (p<0,01), e, aumento de HDL-c (p<0,01), VO2max absoluto (p<0,02), VO2max relativo à MLG (p<0,01) e FATMAX (p<0,04). Além disso, no GTCombinado houve maior redução de MG (p<0,01) e %MG (p<0,01), e, aumento mais significativo de MLG (p<0,01) e QUICKI (p<0,01). Entre os meninos, observou-se no GTAContínuo maior redução de MC (p=0,01), IMC (p=0,01), IMC-z (p=0,01), CA (p=0,01) e RCEst (p=0,01), bem como aumento de FATMAX (p=0,01), enquanto que no GTCombinado houve maior redução de MG (p=0,01), %MG (p=0,03) e HOMA-IR (p=0,01), e, aumento mais de QUICKI (p=0,01). Por fim, no GControle houve aumento de PAD (p=0,01). Conclusão Estudo 1: ambos os protocolos de intervenção resultaram em melhoras nos parâmetros de saúde entre os adolescentes, entretanto o TAContínuo foi mais eficiente no acréscimo da oxidação máxima de gorduras e o TCombinado proporcionou diminuição mais efetiva de massa gorda e aumento da sensibilidade insulínica em adolescentes com excesso de peso, de ambos os sexos. Resultados Estudo 2: a frequência do alelo 27Glu foi de 24,68% (p=0,13). Os portadores do alelo 27Gln apresentaram menores médias de RCEst (p=0,03) e CT (p=0,03), bem como maiores taxas de FATMAX (p=0,02) quando comparados aos portadores do alelo 27Glu. Após 12 semanas, os portadores do alelo 27Gln do GTAContínuo obtiveram maior redução de MC (p<0,01), IMC (p<0,01), IMC-z (p<0,01), CA (p<0,01) e RCEst (p<0,01), além de aumento de HDL-c (p<0,01) e VO2max absoluto (p=0,01). Contudo, os portadores do alelo 27Gln do GTCombinado apresentaram maior redução de MG (p<0,01), %MG (p<0,01) e FCMAX (p=0,05), bem como aumento de MLG (p=0,05), QUICKI (p<0,01) e VO2max relativo à MLG (p<0,01). Entre os portadores do alelo 27Glu do GTCombinado observou-se maior redução de HOMA-IR (p<0,01) e zona de FATMAX (p=0,02), além de aumento de FATMAX (p<0,01). Por fim, identificou-se aumento de PAD (p=0,01) entre os portadores do alelo 27Glu do GControle. Conclusão Estudo 2: os portadores do alelo 27Gln no GTAContínuo se beneficiaram com a redução de parâmetros de adiposidade, além de aumento de HDL-c e VO2max absoluto e redução de massa gorda e FCMAX, e, no GTCombinado obtiveram aumento de MLG, QUICKI e VO2max relativo à MLG. Contudo, entre os portadores do alelo 27Glu do GTCombinado observou-se maior redução de HOMA-IR e zona de FATMAX, além de aumento de FATMAX. Por fim, identificou-se aumento de PAD entre os portadores do alelo 27Glu do GControle. Resultados Estudo 3: A frequência alélica do 64Arg foi de 16,46% (p=0,67). Os adolescentes portadores do polimorfismo (alelo 64Arg) apresentaram maiores médias CT (p<0,01), LDL-c (p<0,01) e FCREP (p=0,04) em comparação aos não portadores do polimorfismo (alelo 64Trp). Após 12 semanas, observou-se interação grupo x tempo apenas do alelo 64Trp e o GTAContínuo na redução do IMC-z (p=0,01). O efeito tempo indicou que houve reduções de IMC-z, CA, MG, %MG, nos grupos GTAContínuo e GCombinado, independente da presença ou ausência do polimorfismo. Notou-se também aumento do HDL-c, VO2max e FATMAX (GTAContínuo > GTCombinado), bem como diminuição de HOMA-IR e aumento de QUICKI (GTCombinado > GTAContínuo) em ambos os genótipos. Além disso, no GTAContínuo os portadores alelo 64Arg apresentaram redução significativa de FCREP (p=0,00) e no GTCombinado os portadores alelo 64Trp apresentaram redução significativa de insulina (p=0,01). Conclusão Estudo 3: os protocolos de intervenção proporcionaram resultados sobre as variáveis cardiometabólicas e FATMAX, independente do polimorfismo Trp64Arg, contudo houve interação entre o alelo 64Trp e o GTAContínuo apenas para o IMC-z. Considerações Finais: exercício físico demonstrou ser fator essencial na promoção de importantes adaptações biológicas com relevantes implicações para prevenir o risco de desenvolvimento ou progressão da obesidade e suas doenças relacionadas em ambos os sexos. Observou-se interação entre portadores e não portadores do polimorfismo Gln27Glu em ambos os protocolos de treinamento, com destaque para os portadores do alelo 27Glu do GTCombinado que apresentaram maiores reduções resistência insulínica e aumento de FATMAX. Em relação ao polimorfismo Trp64Arg, ambos os protocolos de treinamento promoveram reduções de composição corporal, melhora de parâmetros cardiometabólicos e oxidação máxima de gorduras, independentemente da presença ou ausência do polimorfismo. Abstract: The main objective of this thesis was to analyze the effect of training with physical exercises on the maximum fat oxidation (FATMAX) and the cardiometabolic profile in overweight adolescents, as well as to investigate the influence of genetic polymorphisms in the ADRB2 and ADRB3 genes. For this, three different studies were conducted, and in Study 1 the objective was to compare the influence of two training protocols with physical exercises on the maximum fat oxidation and the cardiometabolic profile in overweight adolescents. In Study 2 we investigated the same variables as in the first study, plus analysis of possible genetic interactions with the Gln27Glu polymorphism of the ADRB2 gene and in Study 3 with the Trp64Arg of the ADRB3 gene. The total sample consisted of 203 overweight adolescents, of both sexes, aged between 10 and 17 years. The evaluations were carried out in the initial phase and after 12 weeks of the intervention programs. The adolescents underwent clinical analyzes, which measured height, body mass, waist circumference (WC), pubertal stage, systolic (SBP) and diastolic (DBP) blood pressure. Body mass index (BMI) and BMI-Z score were calculated. Basal insulin (INS), basal glycemia (GLI), total cholesterol (CT), high-density lipoprotein (HDL-c), lipoprotein levels were measured. low density (LDL-c), total lipids (LT) and triglycerides (TG). Body composition was obtained by bioelectrical impedance (BIA). The physical fitness tests were composed by the cardiorespiratory evaluation performed by means of an incremental test on a treadmill and by the maximum strength test (1RM). From the maximum direct oxygen consumption value (VO2max), the cardiorespiratory profile was classified and the FATMAX was calculated. In the second and third studies, genetic evaluations were added, in which the genotyping of the polymorphisms was performed by the PCR method with Taqman. The sample was stratified into two intervention groups with different protocols and a control group without exercises (GControle). The group of continuous aerobic training (Continuous GTAC) was composed of 90 min sessions of aerobic exercise (45 min of indoor cycling and 45 min of walking) plus 20 min of stretching, totaling 110 minutes. The combined training group (combined GTC) consisted of 10 minutes of warm-up, 6 resistance exercises (3 series; 6-10 rep; 30 minutes), followed by 30 min of aerobic exercises (walking / running), totaling 70 min / session. The weekly frequency of the intervention groups was three times, over a period of 12 weeks, while the intensity of the aerobic exercises in the protocols was progressive from 35 to 85% of VO2max or FCRESERVA. Student's T test or the U - Mann Whitney test was used to compare adolescents according to sex and analysis of variance (ANOVA) to assess the effects of time and the group x time interaction. For the main variables that had a significant effect on the training group after 12 weeks, a paired t-test was performed and Cohen?s d was calculated. The level of significance adopted was p?0.05. Results Study 1: Results Study 1: when dividing the sample according to sex, the boys were taller (p <0.01), MLG (p <0.01),% MLG (p <0.01), QUICKI (p < 0.01), absolute VO2max(p <0.01) and FATMAX (p <0.01), while girls had higher RCEst (p <0.01),% MG (p <0.01) and TG (p = 0.04). After 12 weeks, among girls, the interaction factor group x time indicated in the GTACContinuous greater decrease in CM (p <0.01), BMI (p <0.01), BMI-z (p <0.01), and , increase in HDL-c (p <0.01), absolute VO2max (p <0.02), VO2max relative to MLG (p <0.01) and FATMAX (p <0.04). In addition, in the combined GTC there was a greater decrease in MG (p <0.01) and% MG (p <0.01), and a more significant increase in MLG (p <0.01) and QUICKI (p <0.01) ). Among boys, a greater decrease in CM (p = 0.01), BMI (p = 0.01), BMI-z (p = 0.01), WC (p = 0.01) and RCEst (p = 0.01), as well as an increase in FATMAX (p = 0.01), while in Combined GTC there was a greater decrease in MG (p = 0.01),% MG (p = 0.03) and HOMA -IR (p = 0.01), and, increase more than QUICKI (p = 0.01). Finally, in GControle there was an increase in DBP (p = 0.01). Conclusion Study 1: both intervention protocols resulted in improvements in health parameters among adolescents, however the continuous TAC was more efficient in increasing the maximum fat oxidation and the combined TC provided a more effective decrease in fat mass and an increase in insulin sensitivity in adolescents overweight, of both sexes. Results Study 2: the frequency of the 27Glu allele was 24.68% (p = 0.13). The carriers of the 27Gln allele had lower averages of RCEst (p = 0.03) and TC (p = 0.03), as well as higher rates of FATMAX (p = 0.02) when compared to those of the 27Glu allele. After 12 weeks, the carriers of the 27Gln allele of the continuous GTAC obtained a greater reduction in MC (p <0.01), BMI (p <0.01), BMI-z (p <0.01), AC (p <0, 01) and RCEst (p <0.01), in addition to an increase in HDL-c (p <0.01) and absolute VO2max (p = 0.01). However, carriers of the 27Gln allele of the combined GTC showed a greater reduction in MG (p <0.01),% MG (p <0.01) and FCMAX (p = 0.05), as well as an increase in MLG (p = 0 , 05), QUICKI (p <0.01) and VO2max relative to MLG (p <0.01). Among the carriers of the combined GTC 27Glu allele, a greater reduction in HOMA-IR (p <0.01) and FATMAX zone (p = 0.02) was observed, in addition to an increase in FATMAX (p <0.01). Finally, an increase in PAD (p = 0.01) was identified among the carriers of the GControle 27Glu allele. Conclusion Study 2: the carriers of the 27Gln allele in the continuous GTAC benefited from the reduction of adiposity parameters, in addition to an increase in HDL-c and absolute VO2max and a reduction in fat mass and FCMAX, and in the combined GTC they obtained an increase in MLG, QUICKI and VO2max relative to MLG. However, among carriers of the combined GTC 27Glu allele, a greater reduction in HOMA-IR and the FATMAX zone was observed, in addition to an increase in FATMAX. Finally, an increase in PAD was identified among the carriers of the 27Glu allele of GControle. Results Study 3: The 64Arg allele frequency was 16.46% (p = 0.67). Adolescents with polymorphism (64Arg allele) had higher mean CT (p <0.01), LDL-c (p <0.01) and FCREP (p = 0.04) compared to those without polymorphism (64Trp allele) ). After 12 weeks, group x time interaction was observed only for the 64Trp allele and the continuous GTAC in the reduction of the BMI-z (p = 0.01). The time effect indicated that there were reductions in BMI-z, CA, MG,% MG, in the GTACContinuous and Combined GC groups, regardless of the presence or absence of the polymorphism. There was also an increase in HDL-c, VO2max and FATMAX (GTACContinuous> Combined GTC), as well as a decrease in HOMA-IR and an increase in QUICKI (Combined GTC> Continuous GTAC) in both genotypes. In addition, in the continuous GTAC, the 64Arg allele carriers showed a significant reduction in FCREP (p = 0.00) and in the combined GTC, the 64Trp allele carriers showed a significant reduction in insulin (p = 0.01). Conclusion Study 3: the intervention protocols provided results on the cardiometabolic and FATMAX variables, regardless of the Trp64Arg polymorphism, however there was an interaction between the 64Trp allele and the continuous GTAC only for the BMI-z. Final Considerations: physical exercise proved to be an essential factor in the promotion of important biological adaptations with relevant implications for preventing the risk of the development or progression of obesity and its related diseases in both sexes. There was an interaction between carriers and noncarriers of the Gln27Glu polymorphism in both training protocols, with emphasis on the carriers of the combined GTC 27Glu allele that showed greater reductions in insulin resistance and increased FATMAX. Regarding the Trp64Arg polymorphism, both training protocols promoted reductions in body composition, improved cardiometabolic parameters and maximum fat oxidation, regardless of the presence or absence of the polymorphism.
