Polimorfismos nos genes das ficolinas 1 e 3 em pacientes com artrite reumatóide

Resumo: Introdução: A artrite reumatoide (AR) é uma doença autoimune sistêmica, que compromete a membrana sinovial das articulações periféricas resultando em um processo inflamatório crônico. As ficolinas atuam na ativação do complemento como Moléculas de Reconhecimento de Padrões (PRM) associados a patógenos e autoantígenos alterados. A Ficolina-1 é codificada pelo gene FCN1 e sintetizada por monócitos e granulócitos. A ficolina-3 é codificada pelo gene FCN3 e expressa principalmente no fígado e pulmão. Níveis elevados de ficolina-1 foram observados nas articulações de pacientes com AR, no entanto os resultados de associação com polimorfismos de FCN1 e AR são controversos. Também foram encontrados níveis elevados de ficolina-3 em plasma de pacientes com AR recente quando comparados a controles saudáveis. Objetivo: Investigar polimorfismos nos genes das ficolinas 1 e 3 em pacientes com AR e em controles, e verificar a possível associação destes com a susceptibilidade à AR e suas formas clínicas. Métodos: Foram genotipados um total de 148 pacientes com AR e 160 controles através da amplificação alélica por PCR sequência-específica (PCRSSP) para os polimorfismos rs2989727 (-1981G>A), rs10120023 (-542G>A), rs17039495 (-399G>A) e rs10117466 (-144C>A), presentes na região promotora do gene FCN1 e rs10858293 (+33T>G) presente no exon 1 do gene FCN1. Já para o gene FCN3 foram genotipados os seguintes polimorfismos: rs532781899 (+1637delC) do exon 5, rs28362807 (+3524_+3532delTATTTGGCC) do intron 5 e rs4494157 (+4479C>A) do intron 7. Resultados: Verificamos que para o gene FCN1, a frequência dos genótipos GG e alelo G do SNP -542 e do genótipo CC do SNP -144 foi maior em pacientes do que em controles (p= 0,025; p= 0,041 e p= 0,055, respectivamente). O diplótipo -542G;-144C também foi observado mais frequentemente em pacientes que em controles (p=0,050). Além disso, o haplótipo FCN1 AAGAG foi associado à ausência de fator reumatoide (FR) (p= 0,0065). Por outro lado, não encontramos associação entre os genótipos e alelos dos polimorfismos avaliados do gene FCN3 com a susceptibilidade à AR ou características clínicas da doença. Conclusão: Nosso estudo traz achados inéditos com relação à associação dos polimorfismos do gene FCN1 -542G>A e -144C>A à susceptibilidade a AR. Além disso, a relação entre o haplótipo FCN1 AAGAG com a ausência de FR também foi demonstrada pela primeira vez. A avaliação de polimorfismos de FCN3 na AR também é inédita. A ausência de associação dos polimorfismos da ficolina-3 com AR sugere que as ficolinas 1 e 3 desempenham diferentes papéis na doença. Palavras-chave: Artrite reumatoide. Ficolina-1. Ficolina-3. Abstract: Introduction: Rheumatoid arthritis (RA) is a systemic autoimmune disease, which compromises the synovial membrane of the peripheral joints resulting in a chronic inflammatory process. Ficolins act on complement activation as Pattern Recognition Molecules (PRMs) associated with pathogens and altered autoantigens. Ficolin-1 is encoded by the FCN1 gene and synthesized by monocytes and granulocytes. Ficolin-3 is encoded by the FCN3 gene and expressed primarily in the liver and lung. High levels of ficolin-1 have been observed in the joints of patients with RA, however the results of association with polymorphisms of FCN1 and RA are controversial. Elevated plasma levels of ficolin-3 were also found in patients with recent RA when compared to healthy controls. Objective: To investigate the polymorphisms in the genes of the ficolins 1 and 3 in patients with RA and controls and to verify possible association with susceptibility to RA and its clinical forms. Methods: A total of 148 patients with RA and 160 controls were genotyped by sequence-specific PCR (PCR-SSP) for the polymorphisms rs2989727 (-1981G> A), rs10120023 (- 542G>A), rs17039495 (-399G>A), rs10117466 (-144C>A) present in the promoter region of the FCN1 gene and rs10858293 (+33T>G) present in exon 1 of the FCN1 gene. For the FCN3 gene, the following polymorphisms were genotyped: rs532781899 (+1637delC) of exon 5, rs28362807 (+3524_+3532delTATTTGGCC) of intron 5 and rs4494157 (+4479C>A) of intron 7. Results: We found that, for the FCN1 gene, the frequency of the GG and G allele genotypes of the SNP -542 and the CC genotype of the SNP -144 were higher in patients than in controls (p = 0.025, p = 0.041 and p = 0.055, respectively). The diplotype -542G;-144C was also observed more frequently in patients than in controls (p = 0.050). In addition, the haplotype FCN1 AAGAG was associated with absence of rheumatoid factor (p = 0.0065). On the other hand, we did not find an association between the genotypes and alleles of the evaluated polymorphisms of the FCN3 gene with the susceptibility to RA or clinical characteristics of the disease. Conclusion: Our study has unpublished findings regarding the association of the polymorphisms of the gene FCN1 - 542G>A and -144C>A and the susceptibility to RA. In addition, the relationship between the haplotype FCN1 AAGAG with absence of rheumatoid factor was also demonstrated for the first time. The evaluation of FCN3 polymorphisms in RA is also unprecedented. The absence of association of ficolin-3 polymorphisms with RA suggests that ficolins 1 and 3 play different roles in the disease.
