Tratamento endodôntico de molar com curvatura utilizando o sistema Protaper Ultimate® : relato de caso

Abstract

Resumo : O objetivo do presente trabalho é relatar o caso clínico de uma paciente que compareceu ao serviço da Clínica Integrada da Universidade Federal do Paraná com tratamento endodôntico indicado em um dente molar. No exame clínico foi observado elemento dentário com restauração provisória após atendimento de urgência realizado previamente em Unidade Básica de Saúde e pelo exame radiográfico notou-se curvatura na raiz distal. Para diagnóstico foram realizados questionários sobre dor, testes térmico e de palpação, percussão vertical e horizontal, chegando ao diagnóstico final de necrose pulpar. O tratamento proposto foi de tratamento endodôntico utilizando um novo sistema de limas: Protaper Ultimate®. Este sistema é uma evolução do sistema Protaper®. Sua geometria especial e tratamento térmico proporcionam maior flexibilidade e controle durante o procedimento, com as limas em formato de paralelogramo com ângulos variáveis, melhorando a eficiência de corte e reduzindo o estresse. A preparação dos canais mesial e distal foi feita até a lima F2, enquanto no canal palatino foi realizada até a lima F3. Para a obturação, foram usadas as pontas de guta-percha ProTaper UltimateTM Conform Fit®, junto com o cimento endodôntico AH Plus® Jet. Ao final do tratamento pôde ser observado um excelente desempenho desse sistema, especialmente nas áreas que apresentavam maior curvatura.

Abstract : The objective of the present study is to report the clinical case of a patient who attended the Integrated Clinic service at the Federal University of Paraná with endodontic treatment indicated in a molar tooth. During the clinical examination, a tooth with a temporary restoration was observed after previous urgent appointment performed at a Basic Health Unit, and the radiographic examination revealed curvature in the distal root. For diagnosis, questionnaires about pain, thermal and palpation tests, vertical and horizontal percussion were conducted, leading to the final diagnosis of pulp necrosis. The proposed treatment was endodontic treatment using a new file system: Protaper Ultimate®. This system is an evolution of the Protaper® system. Its special geometry and thermal treatment provide greater flexibility and control during the procedure, with the files having a parallelogram shape with variable angles, improving cutting efficiency and reducing stress. Canal preparation of the mesial and distal canals was carried out up to file F2, while in the palatal canal, it was carried out up to file F3. For obturation, ProTaper UltimateTM Conform Fit® gutta-percha points were used, along with AH Plus® Jet endodontic cement. At the end of the treatment, excellent performance of this system could be observed, especially in areas with greater curvature.
