Avaliação de parâmetros hemodinâmicos em pacientes portadores de fibrilação atrial isolada submetidos ao estresse ortostático

Resumo: A fibrilação atrial (FA) é uma condição associada a diversos fatores de risco cardiovasculares e doenças concomitantes. Apesar de inúmeros tratamentos disponíveis para o controle da condição, como medicações antiarrítmicas, betabloqueadoras e até invasivas, como o estudo eletrofisiológico e o isolamento das veias pulmonares, a resposta ao tratamento continua sendo decepcionante em muitos pacientes. A perda da contratilidade atrial, leva a adaptações hemodinâmicas e neuro-humoral. Entretanto, há poucos dados sobre a resposta hemodinâmica durante o teste de inclinação (tilt test) em pacientes com FA sem doenças concomitantes ou fatores de risco cardiovasculares tradicionais conhecidos. O entendimento dos parâmetros hemodinâmicos nos pacientes com FA, tanto em posição supina como em ortostase, comparativamente a uma população sem a condição e saúdável, pode fornecer dados que permitam uma melhor compreenção dos mecanismos envolvidos nessa condição e quiça propor o desenvolvimento de novas modalidades terapêuticas. Objetivos: Investigar alterações hemodinâmicas em pacientes com FA idiopática (sem fatores confusionais conhecidos para essas alterações) utilizando-se a cardiografia por bioimpedância durante o teste de inclinação. Pacientes e métodos: Foram avaliados 40 pacientes com FA idiopática, subdivididos em dois grupos: 1) em ritmo fibrilatório durante o exame (FAfib, N=12); e 2) em ritmo sinusal durante o exame (FAsin, N=28). Um grupo de 38 indivíduos assintomáticos sem comorbidades foi utilizado para comparação (Controle). O tilt test foi realizado com inclinação de 70 graus durante 20 minutos sem drogas sensibilizadoras. Os parâmetros hemodinâmicos foram obtidos em posição supina e em 5, 10, 15 e 20 minutos após inclinação, através de monitor não invasivo de bioimpedância (Task force monitor). Os dados estão descritos como mediana e intervalo interquartil ou proporções. Resultados de p<0,05 foram considerados significativos. Resultados:Em posição supina, pacientes FAfib apresentaram maior índice de resistência vascular periférica [IRVP, FAfib = 3332(3104-3933) vs. FAsin = 3109(2522-3910) vs. Controle = 2126(1833-2760) dyne.seg.m3/cm-5, p<0,0001], menor índice de volume sistólico [IVS, FAfib = 26,7(21,3-32,4) vs. FAsin = 38,5(32,2- 48,6) vs. Controle = 46,1(42,3-55,6) mL/m2, p<0,0001]. Durante a inclinação, houve queda do IRVP no grupo FAfib, mas aumento nos grupos FAsin e Controle [?IVRP em 20 min, FAfib = -477(-1148;82) vs. FAsin = 131(-525;887) vs. Controle = 357(- 29;681) dyne.seg.m3/cm-5, p=0,005].. O gupo FAfib apresentou menor queda no IVS comparativamente aos outros grupos em ritmo sinusal [?IVS em 20 min, FAfib = -1,3(-3,4;1,7) vs. FAsin -6,4(-17,3; -0,1) vs. Controle = -11,8(-18,7;-8,0) mL/m2, p<0,001]. As diferenças observadas no IRVP e IVS entre pacientes com FA e Controle foram significativas mesmo quando os pacientes foram pareados por sexo, idade, IMC e uso de medicações cronotrópicas negativas. Conclusões: Pacientes com FA idiopática apresentam adaptações hemodinâmicas que os diferenciam de pacientes sem essa arritmia, salientando-se um maior IRVP e menor IVS em posição supina; e uma resposta diferenciada ao desafio ortostático, evidenciada pela maior variabilidade do IRVP e menor queda do IVS. Os achados hemodinâmicos sugerem uma adaptação diferencial do SNA associada à presença do ritmo de FA. Abstract: Atrial fibrillation (AF) is a condition associated with several cardiovascular risk factors and concomitant diseases. Despite numerous treatments available to control the condition, such as antiarrhythmic, beta-blocker and even invasive medications, such as electrophysiological studies and isolation of the pulmonary veins, the response to treatment continues to be disappointing in many patients. The loss of atrial contractility leads to hemodynamic and neurohumoral adaptations. However, there is little data on the hemodynamic response during the tilt test in patients with AF without concomitant diseases or known traditional cardiovascular risk factors. Understanding the hemodynamic parameters in patients with AF, both in supine and orthostatic positions, compared to a healthy population without the condition, can provide data that allow a better understanding of the mechanisms involved in this condition and perhaps propose the development of new therapeutic modalities. Objectives: To investigate hemodynamic changes in patients with idiopathic AF (without known confounding factors for these changes) using bioimpedance cardiography during the tilt test. Patients and methods: Forty patients with idiopathic AF were evaluated, subdivided into two groups: 1) in fibrillatory rhythm during the exam (AFfib, N=12); and 2) in sinus rhythm during the exam (AFsin, N=28). A group of 38 asymptomatic individuals without comorbidities was used for comparison (Control). The tilt test was performed at a 70-degree inclination for 20 minutes without sensitizing drugs. Hemodynamic parameters were obtained in the supine position and at 5, 10, 15 and 20 minutes after tilting, using a non-invasive bioimpedance monitor (Task force monitor). Data are described as median and interquartile range or proportions. Results of p<0.05 were considered significant. Results:In the supine position, AFfib patients had a greater peripheral vascular resistance index [PVRI, AFfib = 3332(3104-3933) vs. AFsin = 3109(2522-3910) vs. Control = 2126(1833-2760) dyne.seg.m3/cm-5, p<0.0001], lower stroke volume index [SVI, AFfib = 26.7(21.3- 32.4) vs. AFsin = 38.5(32.2-48.6) vs. Control = 46.1(42.3-55.6) mL/m2, p<0.0001]. During the tilt, there was a decrease in PVRI in the AFfib group, but an increase in the AFsin and Control groups [?PVRI at 20 min, AFfib = -477(-1148;82) vs. AFsin = 131(-525;887) vs. control = 357(-29;681) dyne.seg.m3/cm-5, p=0.005]. The AFfib group showed a smaller decrease in SVI compared to the other groups in sinus rhythm [?SVI in 20 min, AFfib = -1 .3(-3.4;1.7) vs. AFsin -6.4(-17.3; -0.1) vs. Control = -11.8(-18.7;-8.0) mL/m2, p<0.001]. The differences observed in PVRI and SVI between patients with AF and Control were significant even when patients were matched for sex, age, BMI and use of negative chronotropic medications. Conclusions:Patients with idiopathic AF present hemodynamic adaptations that differentiate them from patients without this arrhythmia, highlighting a higher PVRI and lower SVI in the supine position; and a differentiated response to orthostatic challenge, evidenced by greater variability in PVRI and smaller drop in SVI. The hemodynamic findings suggest a differential adaptation of the ANS associated with the presence of the AF rhythm.
