O efeito da aplicação do ozônio no processo de cicatrização de exodontias em indivíduos tabagistas : estudo em boca dividida

Resumo: A terapia com ozônio demonstra ser um coadjuvante importante na cicatrização e na redução da dor pós-operatória, entretanto, seu uso não foi estabelecido nas exodontias em indivíduos tabagistas. Objetivo: avaliar a efetividade da aplicação terapêutica de ozônio sob diferentes formas, no controle da dor e cicatrização após exodontia em indivíduos tabagistas. A pesquisa realizada foi um estudo clínico de boca dividida. Material e Métodos: a amostra foi composta por 42 indivíduos adultos (n=14 por grupo), do sexo masculino, tabagistas, hospitalizados, e que apresentaram dentes com indicação de exodontia contralateral, superior ou inferior. A amostra foi dividida em grupo teste e controle, e ao final das exodontias no lado teste foi aplicado um dos seguintes tratamentos: óleo ozonizado (OZ); gás ozônio (GO) e água ozonizada (AZ). O lado controle recebeu soro fisiológico 0,9%. Todos receberam as orientações pós-operatórias e a prescrição de dipirona sódica 1g, 3 vezes ao dia por 3 dias. A intensidade da dor pós-operatória nos tempos de 1, 2 e 3 dias foi avaliada por meio da Escala Visual Analógica (EVA). A cicatrização das feridas cirúrgicas foi analisada por meio de imagens fotográficas em 0, 2 e 7 dias do pósoperatório, com base nos parâmetros clínicos inflamatórios, fechamento das feridas e classificadas como excelente, boa, moderada e inadequada. A presença ou ausência de biofilme também foi avaliada. Essas imagens foram inseridas no programa Power Point, e analisadas posteriormente de forma cega, por examinadora calibrada e com experiência em cirurgia buco maxilofacial. O intervalo entre as cirurgias do grupo teste e controle foi de duas semanas. Resultados: No total, 84 exodontias foram realizadas em 42 indivíduos. Os testes de Friedman, Kruskall-Wallis e Wilcoxon foram usados para análise quantitativa da dor e da cicatrização entre os diferentes dias e grupos (p<0,05). Os grupos testes e seus controles, apresentaram redução significativa da dor no 3º dia comparados ao 1º dia do PO. No 2º dia os grupos que receberam OZ mostraram redução da dor comparados aos seus controles. O GO apresentou redução da dor no 2º dia. No primeiro dia do pós-operatório, verificou-se que o grupo que recebeu óleo OZ apresentou redução da dor quando comparado com gás ou água (p<0.05), porém sem diferença estatística quando comparado ao seu controle (p>0.05). Ao comparar o grupo teste com controle, no segundo dia pós-operatório, verificou-se que os grupos que receberam óleo ozonizado ou gás ozônio mostraram redução da dor comparados aos seus controles (p<0.05). No segundo dia do pósoperatório, o grupo que recebeu OZ apresentou uma melhora da cicatrização no comparado ao grupo controle. A média do consumo diário e tempo de uso de cigarro foi de 22,48 e 17,48 anos respectivamente. A presença do biofilme foi encontrada em menos de 10% dos casos. Não houve diferença estatística com relação a técnica e tempo cirúrgico entre os grupos. Conclusão: As formas gasosa e do óleo ozonizado demonstraram no período inicial pós-operatório controlar a dor, e somente o óleo ozonizado auxiliou na cicatrização do alvéolo no 2º dia do pós-operatório em indivíduos tabagistas. Abstract: Ozone therapy proves to be an important adjuvant in healing and reducing postoperative pain, however, its use has not been established in tooth extractions in smokers. Objective: to evaluate the effectiveness of the therapeutic application of ozone in different forms, in the control of pain and healing after tooth extraction in smokers. The research performed was a split-mouth clinical study. Material and Methods: the sample consisted of 42 adult individuals (n=14 per group), male, smokers, hospitalized, and who had teeth with an indication for contralateral extraction, upper or lower. The sample was divided into test and control groups, and at the end of the extractions on the test side, one of the following treatments was applied: ozonized oil (OZ); ozone gas (GO) and ozonated water (AZ). The control side received 0.9% saline solution. All received postoperative guidelines and a prescription for sodium dipyrone 1g, 3 times a day for 3 days. The intensity of postoperative pain at 1, 2 and 3 days was evaluated using the Visual Analog Scale (VAS). Healing of surgical wounds was analyzed using photographic images at 0, 2 and 7 days postoperatively, based on inflammatory clinical parameters, wound closure and classified as excellent, good, moderate and inadequate. The presence or absence of biofilm was also evaluated. These images were inserted into the Power Point program, and subsequently analyzed blindly, by a calibrated examiner with experience in oral and maxillofacial surgery. The interval between surgeries in the test and control groups was two weeks. Results: In total, 84 extractions were performed in 42 individuals. Friedman, Kruskall-Wallis and Wilcoxon tests were used for quantitative analysis of pain and healing between different days and groups (p<0.05). The test groups and their controls showed a significant reduction in pain on the 3rd day compared to the 1st PO day. On the 2nd day, the groups that received OZ showed a reduction in pain compared to their controls. The GO showed pain reduction on the 2nd day. On the first postoperative day, it was verified that the group that received OZ oil presented a reduction in pain when compared to gas or water (p<0.05), but without statistical difference when compared to its control (p>0.05). When comparing the test group with the control, on the second postoperative day, it was verified that the groups that received ozonated oil or ozone gas showed a reduction in pain compared to their controls (p<0.05). On the second postoperative day, the group receiving OZ showed improved healing compared to the control group. The average daily consumption and time of cigarette use was 22.48 and 17.48 years respectively. The presence of biofilm was found in less than 10% of cases. There was no statistical difference regarding technique and surgical time between groups. Conclusion: The gaseous and ozonized oil forms were shown to control pain in the initial post-operative period, and only ozonized oil helped the healing of the alveolus on the 2nd post-operative day in smokers.
