Avaliação dos efeitos de polimorfismos de nucleotídeo único (SNPs) no gene ABCG2 em pacientes com câncer de mama

Resumo : O estudo do câncer compreende diversas abordagens para seu manejo, combate e compreensão, tendo a quimioterapia como principal base para seu enfrentamento. A garantia da efetividade das terapias convencionais depende da capacidade de se contornar a resistência intrínseca ou adquirida que muitos tumores desenvolvem, sendo a resistência à múltiplas drogas uma grande representante desse processo e a presença massiva de transportadores da família ABC em células neoplásicas reconhecidamente associada ao seu desenvolvimento. O estudo de polimorfismos de nucleotídeo único (SNPs) em ABCG2, membro desse grupo de transportadores, têm demonstrado seu papel na alteração de parâmetros farmacocinéticos de drogas que são seus substratos, o que pode resultar em uma importância clínica da investigação de suas variantes na busca por maior efetividade terapêutica. Tendo esse contexto como base, o presente trabalho buscou investigar a significância de SNPs em ABCG2 em pacientes com câncer de mama através da avaliação de artigos selecionados pelo seguinte critério: trabalhos que se caracterizavam como estudos de efeitos clínicos de algum dos polimorfismos de ABCG2 em pacientes com câncer de mama. Dessa pesquisa, foi possível estabelecer como principal achado que o SNP rs2231142 é o mais investigado entre os artigos em pacientes com câncer de mama, apresentando efeitos compatíveis com os previstos na literatura, qual seja a sua influência na manifestação de efeitos adversos decorrentes da farmacocinética de drogas antineoplásicas. Diante disso, visualiza-se que a presença de SNPs em ABCG2 podem resultar em alterações relevantes nos efeitos das drogas e, consequentemente, nos resultados das terapias quimioterápicas aplicadas atualmente, podendo até mesmo, num futuro próximo, ter sua genotipagem necessária para definir alternativas terapêuticas mais efetivas e seguras. Abstract : The study of cancer comprises several approaches for its management, combat and understanding, with chemotherapy as the main basis for its confrontation. Ensuring the effectiveness of conventional therapies depends on the ability to overcome the intrinsic or acquired resistance that many tumors develop, with resistance to multiple drugs being a major representative of this process and the massive presence of ABC family transporters in neoplastic cells known to be associated with their development. The study of single nucleotide polymorphisms (SNPs) in ABCG2, a member of this group of transporters, have demonstrated their role in altering the pharmacokinetic parameters of drugs that are their substrates, which may result in the clinical importance of investigating their variants in the search for greater therapeutic effectiveness. Based on this context, the present study aimed to investigate the significance of SNPs in ABCG2 in patients with breast cancer by evaluating articles selected according to the following criteria: studies that were characterized as studies of the clinical effects of some of the ABCG2 polymorphisms in patients with breast cancer. From this research, it was possible to establish as the main finding that the SNP rs2231142 is the most investigated among the articles on patients with breast cancer, presenting effects compatible with those predicted in the literature, what is its influence on the manifestation of adverse effects resulting from the pharmacokinetics of antineoplastic drugs. In view of this, it is seen that the presence of SNPs in ABCG2 can result in relevant changes in the effects of drugs and, consequently, in the results of currently applied chemotherapy therapies, and may even, in the near future, have their genotyping necessary to define therapeutic alternatives more effective and safe.
