Avaliação do perfil lipidico e lipoproteico em pacientes no climaterio utilizando a terapeutica de reposição estrogenica

Resumo: O objetivo do presente estudo foi avaliar as alterações do perfil lipídico e lipoprotéico em pacientes na pós-menopausa, as quais foram submetidas à terapêutica de reposição estrogênica por duas vias distintas de administração (oral e transdérmica) e comparadas com um grupo de controle. Utilizou-se o esquema cíclico e a seleção das pacientes foi aleatória, no Ambulatório do Climatério da Disciplina de Ginecologia do Departamento de Tocoginecologia do Hospital de Clínicas da UFPR. Foram selecionadas trinta pacientes, dividas em três grupos de dez, as quais foram acompanhadas clínica e laboratorialmente, durante o período de seis meses de pesquisa. As pacientes apresentavam tempo de menopausa entre um e cinco anos. O estudo foi prospectivo, randomizado, unicêntrico, realizado no período de julho de 1992 a julho de 1993. As pacientes compareceram às consultas mensalmente para reposição dos medicamentos, bem como, para avaliação dos níveis pressórios, peso e efeitos colaterais. Para a análise dos resultados, foram utilizados testes paramétricos e não-paramétricos, levando-se em consideração a natureza das variáveis. As conclusões dos testes estatísticos tiveram por base o nível de significância de 5% (p=< 0,05). As amostras de sangue foram coletadas e analisadas a partir do primeiro dia da pesquisa; a seguir com noventa dias e cento e oitenta dias, no Laboratório Central de Bioquímica do Hospital de Clínicas da UFPR. A análise estatística demonstrou que no terceiro mês da pesquisa a LDL-C apresentou uma diminuição em seus níveis nas pacientes que receberam TRE-oral. A HDL-C apresentou uma elevação, a partir do terceiro mês, tanto nas pacientes que receberam TRE-oral, quanto transdérmica. Os triglicerídios apresentaram resposta significativa, com elevação de seus níveis aos três meses, nas pacientes do grupo A. Os níveis de colesterol total não apresentaram alterações significativas durante o período da pesquisa. Esses resultados foram mantidos até o final da pesquisa, evidenciando que a Terapêutica de Reposição Estrogênica, em pacientes menopausadas tem influência significativa sobre o perfil lipídico e lipoprotéico. Abstract: The purpose of the study in question was to evaluate alterations in the lipid and lipoprotein levels by submitting patients to the therapeutics of estrogen administration, during the patients' postmenopausal period, by administering estrogen orally and by transdermic ways and comparing these patients with a group of control. A cyclic scheme was performed and a fortuitous selection of the patients was done in the Climacterium Ambulatory of the Gynecology Department at the Hospital de Clínicas of the UFPR. Thirty patients were selected by chance, which were divided into three groups of ten patients each. The patients received clinical and laboratorial attendance during the six month period of the said research. The period of menopause, among the patients, ranged from one to five years. It was a prospective and random study, carried out during an year period, from July, 1992 to July 1993. During this year, the patients consulted their physician monthly, for replacing medicaments, as well as, to evaluate blood pressure, weight and side effects. Parametric tests and non-parametric tests were applied to analyze the results, taking into consideration the nature of the variables. The conclusion of the statistic tests were based on the level of significance of 5% (p=( 0.05). The blood samples were collected and analysed since the very first day of the research; as well as, ninety days and one hundred and eighty days after the first collection. This was carried out in the Biochemistry Central Laboratory of the Hospital de Clínicas of the UFPR. The statistical analysis showed that in the third month of the said research, the levels of LDL-C decreased in patients to whom the TRE-oral was administered. From the third month on, the HDL-C levels increased, in patients to whom the TRE-oral as well in patients to whom the transdermic TRE was administered. The triglyceride levels showed a significant response, with its levels increasing after three months, in the group A patients. The total cholesterol levels didn't show any significant changes during the period of this research. These results were kept on these levels until the end of the research period, showing clearly that the Therapeutics of Estrogen Administration, applied in patients under the menopausal period, have expressive influence over the lipid and lipoprotein levels.
