Associação entre catastrofização da dor e disfunções temporomandibulares

Resumo : A catastrofização da dor pode ser definida como uma série de pensamentos negativos acerca de uma experiência dolorosa real ou potencial. Disfunção temporomandibular (DTM) é um termo coletivo que designa uma síndrome dolorosa musculoesquelética que acomete os músculos mastigatórios, a articulação temporomandibular (ATM) e estruturas associadas. O objetivo deste estudo foi avaliar o nível de catastrofização da dor em indivíduos com DTM. Foram avaliados 40 indivíduos no total, sendo eles divididos em dois grupos: um com 20 indivíduos no grupo controle e outro com 20 indivíduos com DTM (experimental). Todos os participantes do grupo DTM foram avaliados através de anamnese e exame clínico, considerando-se sua queixa principal, de acordo como RDC/TMD. A versão em Português do Brasil da Escala de Catastrofização da Dor (ECD) foi aplicada em todos os participantes, de ambos os grupos. As mulheres representaram o maior grupo com DTM, mas não tiveram maiores níveis de catastrofização em relação aos homens. A idade não se mostrou estatisticamente relevante, o que contraria alguns estudos já publicados. O grupo DTM apresentou maior índice de catastrofização da dor (p=0,001), mas nenhum subtipo de DTM teve relação com esse índice. Ruminação, magnificação e desesperança foram relevantes nesse estudo, mostrando alta correlação com a presença de DTM. Assim, concluiu-se que pacientes com DTM apresentam maiores índices de catastrofização que indivíduos saudáveis. Abstract: Pain catastrophizing can be defined as a series of negative thoughts about an actual or potential painful experience. Temporomandibular dysfunction (TMD) is a collective term for a painful musculoskeletal syndrome that affects the masticatory muscles, temporomandibular joint (TMJ) and associated structures. The aim of this study was to evaluate the level of pain catastrophizing in individuals with TMD. A total of 40 subjects were evaluated and divided into two groups: one with 20 subjects in the control group and another with 20 subjects with TMD (experimental). All TMD group participants were evaluated by means of a medical history and clinical examination, considering their chief complaint, according to the RDC/TMD. The Brazilian Portuguese version of the Pain Catastrophizing Scale (PCS) was applied to all participants in both groups. Women represented the largest group with TMD, but did not have higher levels of catastrophizing compared to men. Age was not statistically relevant, which contradicts some previously published studies. The TMD group had a higher pain catastrophizing index (p=0,001), but no TMD subtype was related to this index. Rumination, magnification, and hopelessness were relevant in this study, showing high correlation with the presence of TMD. Thus, it was concluded that TMD patients present higher scores of catastrophizing than healthy controls.
