Seleção, padronização e validação de novos antígenos para diagnóstico sorológico da tuberculose

Resumo: A tuberculose é uma doença infectocontagiosa que afeta populações de diferentes partes do mundo. A taxa de infecção por 100 mil habitantes varia de um país a outro, sendo que os países com maior incidência estão no continente africano e em partes da Ásia, seguidos de alguns países da América. Dentre os métodos de controle estão o diagnóstico e tratamento precoces para evitar a disseminação do Mycobacterium tuberculosis. Os métodos diagnósticos preconizados são o exame clínico, bacterioscópico e os métodos laboratoriais complementares. Dentre eles estão os métodos imunológicos e moleculares. Todavia, todos possuem vantagens e desvantagens e por esta razão os órgãos responsáveis pela saúde estão sempre incentivando a inovação na área diagnóstica. Este trabalho teve como objetivo a pesquisa de novos antígenos candidatos para testes sorológicos, menos invasivos, e para os quais a estrutura laboratorial da rede pública já tem disponibilidade de equipamentos. Para seleção dos antígenos foi usada a técnica de phage display. Para avaliação da imunogenicidade dos mesmos foram usadas as técnicas de imunodetecção em membrana, avaliação in silico e teste ELISA. Quatro peptídeos (P.Mt. PD 4, P.Mt. PD 6, P.Mt. PD 7, P.Mt. PD 8) se mostraram reagentes na membrana e na análise de bioinformática apresentaram scores que variaram de 0,5338 a 0,9341. Na padronização dos parâmetros concentração de antígenos, diluição de soros, e diluição de conjugado o peptídeo P.Mt. PD 8 apresentou melhor resposta ao diferenciar soros de pacientes positivos de soros negativos. Finalmente a validação deste antígeno frente a soros de pacientes com tuberculose ativa e indivíduos negativos apresentou sensibilidade e especificidade de 100%. O teste de avidez usado em ensaio imunoenzimático foi realizado para avaliar a força de ligação do anticorpo com o antígeno e foi obtido um índice de 85,72%, mostrando que o peptídeo P.Mt. PD 8 é um excelente candidato ao diagnóstico da tuberculose por ter alta sensibilidade, alta especificidade e alta avidez. Abstract: Tuberculosis is an infectious and contagious disease that affects people in different parts of the world. The infection rate per 100,000 inhabitants varies from country to country with the highest incidence being in the African continent and in parts of Asia, followed by some countries in America. Control methods include early diagnosis and treatment to prevent the spread of Mycobacterium tuberculosis. The early diagnostic procedures are clinical, bacterioscopic and complementary laboratory methods which include immunological and molecular methods. However, these methods have advantages and disadvantages that make health organizations to encourage research and innovation in the diagnostic field. The aim of this work was the research of new candidate antigens for serologic tests which are less invasive and for which the public health network already has laboratory structure and equipment available. For the selection of antigens, the phage display technique was used and to assess their immunogenicity the techniques of membrane immunodetection, in silico evaluation and ELISA test were used. Four peptides (P.Mt. PD 4, P.Mt. PD 6, P.Mt. PD 7, P.Mt. PD 8) showed to be reagent on the membrane and, in the bioinformatics analyses, presented scores from 0.5338 to 0.9341. The peptide 8 showed better response in differentiating positive from negative sera during the standardization of the parameters antigen concentration, serum dilution and conjugate dilution. Finally, the validation of this antigen against sera obtained from patients with active tuberculosis and negative individuals showed sensitivity and specificity of 100%. The avidity test used in an enzyme immunoassay was carried out to assess the binding strength of the antibody with the antigen and presented an index of 85.72%, showing that peptide 8 is an excellent candidate for the diagnosis of tuberculosis due to its high sensitivity, high specificity and high avidity.
