Sobrecarga dos cuidadores e percepção do suporte familiar de adultos jovens após acidente vascular cerebral

Abstract

Resumo: Este estudo teve como objetivo compreender a relação entre a ocorrência de sobrecarga do cuidador e a percepção do nível de suporte familiar em adultos jovens que vivem com o acidente vascular cerebral. O trabalho foi realizado em três partes: a) revisão sistemática da literatura sobre a sobrecarga do cuidador familiar de jovem adulto após acidente vascular cerebral, abrangendo o período de 2010 a 2014; b) estudo descritivo transversal; metodologia analítica correlacional de natureza quantitativa para identificar a sobrecarga do cuidador e a percepção de suporte familiar do adulto após AVC; e c) estudo de análise temática, exploratória e descritiva, para compreender as formas de enfrentamento após AVC, sob a perspectiva do cuidador familiar. A amostra do estudo b foi composta por 20 díades - adulto com AVC e seu cuidador - e foi selecionada em Hospitais Universitários do Sul do Brasil. Os instrumentos consistiram em entrevista semiestruturada, questionário sociodemográfico, Inventário de Percepção de Suporte Familiar (IPSF) e escala de sobrecarga Burden Interview (BI). Os resultados demonstraram a escassez de estudos sobre a percepção de adultos com AVC e a sobrecarga que a doença impõe à família. Na análise quantitativa, observou-se uma relação inversa entre a percepção do suporte familiar do adulto com AVC e a sobrecarga do cuidador. Na análise das entrevistas do estudo c várias temáticas foram observadas, tais como as percepções e sentimentos diante da notícia do AVC no jovem, a escolha do cuidador, as dificuldades percebidas após alta hospitalar, adaptação, reabilitação e as expectativas quanto ao futuro da pessoa com AVC. Desde o diagnóstico do AVC de um familiar até o processo de reabilitação, estratégias de enfrentamento são necessárias para a autorregulação do sistema familiar, tais como, redefinição de papéis entre os familiares, escolha do cuidador e organização da rotina para o processo de reabilitação. Isso requer compreender e observar o indivíduo com AVC em seu contexto familiar e de reabilitação, num processo de auto-organização do indivíduo em uma nova realidade complexa de relações interpessoais. Abstract: This study aimed to understand the relationship between the occurrence of caregiver burden and perception of family support level in young adults living with stroke. It was conducted in three parts: a systematic review of the literature on the burden of family caregivers of young adults after stroke, between 2010 and 2014; a cross-sectional study; correlational analytical method with quantitative data, to identify the caregiver burden and perception of family support adult after stroke; and a thematic, exploratory and descriptive analysis to understand the way of coping after stroke, observed by family caregivers. The sample consisted of 20 dyads - adults with stroke / caregiver - recruited at University Hospitals in southern Brazil. The instruments consisted of semi-structured interview, sociodemographic questionnaire, Inventory Family Support Perception (IPSF) and the Burden Interview (BI); and the scores were submitted to statistical analysis. Results showed the lack of studies on perception of adults with stroke and the burden that the disease imposes on the family. In the quantitative analysis of the scales, there was an inverse relationship between the perception of family support by the adult with stroke and his/her caregiver burden. Several themes were observed, such as perceptions and feelings about stroke, the choice of caregiver, the perceived difficulties after discharge, adaptation, rehabilitation and expectations for the future of people with stroke. After the stroke diagnosis of a family member until the end of the rehabilitation process, coping strategies will be required to self-regulation of the family system, such as redefinition of roles among family, choice of caregiver and organization of routine for the rehabilitation process. This requires understanding and observing the person with stroke in their family context and rehabilitation process, as the self-organization of the individual is a new complex reality of interpersonal relationships.
