Um olhar psicanalítico sobre o adoecimento físico no processo de luto

Abstract

Resumo: Embora as perdas objetais sejam inevitáveis à existência humana, acompanhando as pessoas do início até o fim de suas vidas, muitos parecem adoecer no processo de luto, tornando-se esse um problema de saúde pública. Sob esse panorama, buscou-se compreender, por uma perspectiva psicanalítica, o papel do adoecimento físico na economia psíquica de pessoas enlutadas. Para tanto, utilizou-se a Construção de caso como estratégia de pesquisa, uma metodologia de caráter clínico na qual a produção dos dados para o estudo é proveniente da própria prática clínica com o método psicanalítico, exigindo-se, dessa forma, o pesquisador enquanto analista. Assim, partindo-se de um referencial teórico freudiano, foi selecionada para a pesquisa uma voluntária que se inscreveu na lista de espera do Centro de Psicologia Aplicada da Universidade Federal do Paraná, a fim de receber tratamento psicoterapêutico. Trata-se de Irene, nome fictício para uma mulher idosa que foi acometida pela psoríase e pela rosácea após o falecimento do marido com quem conviveu por mais de quarenta anos. Como resultado da pesquisa, vemos, na particularidade do caso Irene, a maneira com que o trabalho de luto faz emergir a violência da pulsão desligada, que sumariamente ofende o Ideal de Eu da paciente. A pele lesionada entra então no lugar da ambivalência amorosa, tornando menos ruidoso o agir da pulsão de morte e um pouco mais tolerável a dor do luto para o aparelho psíquico. Adoecer, nesse sentido, entra como parte do processo de luto da paciente, demonstrando que o adoecimento físico pode cumprir um papel na reorganização da economia psíquica abalada pela perda objetal. Abstract: Although object losses are inevitable to human existence, accompanying people from the beginning to the end of their lives, many people seem to fall ill in the mourning process, making this a public health problem. In this context, we sought to understand, from a psychoanalytic perspective, the role of physical illness in the psychic economy of mourned people. To this end, Case Construction was used as a research strategy, a methodology of a clinical nature in which the production of data for the study comes from the clinical practice itself with the psychoanalytic method, thus requiring the researcher as analyst. Thus, based on a Freudian theoretical framework, a volunteer, registered on the waiting list of the Center for Applied Psychology at the Federal University of Paraná was selected, in order to receive psychotherapeutic treatment. This is Irene, a fictitious name for an elderly woman who was affected by psoriasis and rosacea after the death of her husband with whom she lived for more than forty years. As a result of the research, we see, in the particularity of the Irene Case, the way in which the work of mourning makes the violence of the disconnected drive emerge, which summarily offends the patient's Ideal of Ego. The injured skin then takes the place of loving ambivalence, making the action of the death drive less noisy and a little more tolerable to the pain of mourning for the psychic apparatus. Getting sick, in this sense, enters as part of the patient's grieving process, demonstrating that physical illness can play a role in the reorganization of the psychic economy shaken by object loss.
