Estudo microtomográfico do preparo de canais radiculares com curvatura acentuada utilizando instrumentos com diferentes ligas de níquel-titânio

Resumo: A qualidade do preparo químico-mecânico dos canais é influenciada pela condição anatômica do sistema de canais radiculares, design e liga metálica utilizada na fabricação dos instrumentos, além da técnica de instrumentação. Canais com curvaturas acentuadas representam um grande desafio, pois se eleva o risco de iatrogenias, dentre as quais o desvio do eixo central, transporte do canal e/ou fratura de instrumento pode dificultar a descontaminação de determinadas regiões do sistema de canais radiculares, além de comprometer a estrutura radicular. Atualmente, tratamentos térmicos são estratégias utilizadas para otimizar a microestrutura dos instrumentos de níquel-titânio (NiTi) e assim melhorar as propriedades mecânicas de flexibilidade e resistência à fratura. Até o momento não há relatos na literatura sobre alterações estruturais causadas por dilatações apicais maiores que instrumentos com diâmetro de ponta com 0,40mm em canais curvos de molares superiores, tampouco há informações sobre a performance do sistema VDW.ROTATE nesta condição anatômica. Este estudo resultou em duas propostas de artigos, a primeira teve como objetivo avaliar os efeitos decorrentes do preparo radicular em canais curvos empregando a sequência de instrumentos dos sistemas Hyflex EDM e VDW.Rotate, até os instrumentos 60.02 e 60.04, respectivamente. A segunda proposta teve como objetivo analisar as alterações pós-preparo com instrumentos 0,25mm e 0,40mm nas raízes mesiais com curvatura acentuada de molares superiores utilizando três sistemas de instrumentos mecânicos rotatórios com diferentes secções transversais e ligas de níquel-titânio. Foram utilizadas quarenta raízes de molares superiores escaneadas com micro-CT e pareadas anatomicamente de acordo com o volume, classificação da configuração dos canais e com curvatura entre 20º e 40º. Os espécimes foram novamente escaneados após o preparo com os diferentes instrumentos de três sistemas rotatórios, sendo divididos em grupos, no artigo 1, grupo Hyflex EDM (n=15) e VDW.ROTATE (n=15) e artigo 2 grupo ProTaper Next (n=10), Hyflex EDM (n=10) e VDW.ROTATE (n=10). Como solução irrigadora foi utilizado hipoclorito de sódio a 1% e complementação com irrigação ultrassônica passiva (PUI). Foram mensurados volume de canal e dentina, diâmetro do canal, espessura de dentina, transporte, centralização, acúmulo de debris e áreas não tocadas. Os dados obtidos foram avaliados estatisticamente pelos testes de teste t, Mann Whitney, ANOVA e Tukey ou Kruskal Wallis e Dunn's e considerado o nível de significância de 5% (P < 0.05). A comparação entre Hyflex EDM e VDW.ROTATE não revelou diferenças significativas para volume de canal, diâmetro do canal, áreas não tocadas e acúmulo de debris (P> 0,05). Houve diferença significativa (P < 0.05) na redução do volume cervical e total de dentina após a utilização do instrumento VDW.ROTATE 60.04. Entretanto, na comparação intragrupos, com o aumento na dilatação com instrumentos de maiores diâmetros, houve diferenças significativas (P < 0.05) no volume do canal e dentina, além do diâmetro no sentido M-D em ambos os grupos. Para os três grupos não houve diferença significativa (P>0,05) para os dados analisados. Concluiu-se que os sistemas Hyflex EDM, VDW.ROTATE e PropTaper Next proporcionaram um preparo efetivo e seguro em canais radiculares mesiais com curvatura acentuada de molares superiores. A maior dilatação do canal foi associada ao aumento de volume do canal radicular, diminuição do acúmulo de debris e áreas não tocadas com manutenção da estrutura dentinária. Apesar de serem avaliadas diferentes ligas metálicas observamos que não houve diferenças significativas nas alterações causadas nos preparo realizados nos canais curvos. Abstract: The quality of the chemical-mechanical preparation of the root canals is influenced by the anatomical condition of the root canals system, design and metal alloy used in the manufacture of the instruments, in addition to the instrumentation technique. Channels with sharp curvatures represent a great challenge, since the risk of iatrogenesis is increased, among which the deviation of the central axis, canal transport and / or instrument fracture can make it difficult to decontaminate certain regions of the root canal system, in addition to compromise the root structure. Currently, heat treatments are strategies used to optimize the microstructure of nickel-titanium (NiTi) instruments and thus improve the mechanical properties of flexibility and fracture resistance. To date, there are no reports in the literature on structural changes caused by apical dilations greater than instruments with a 0.40mm tip diameter in curved upper molar channels, nor is there information on the performance of the VDW.ROTATE system in this anatomical condition. This study resulted in two proposals for articles, the first of which aimed to evaluate the effects of root preparation in curved channels using the sequence of instruments in the Hyflex EDM and VDW.Rotate systems, up to instruments 60.02 and 60.04, respectively. The second proposal aimed to analyze the post-preparation changes with 0.25mm and 0.40mm instruments in the mesial roots with marked curvature of upper molars using three systems of rotating mechanical instruments with different cross sections and nickel-titanium alloys. Forty roots of maxillary molars scanned with micro-CT and paired anatomically according to volume, classification of channel configuration and with curvature between 20º and 40º were used. The specimens were scanned again after preparation with the different instruments of three rotating systems, being divided into groups, in article 1, Hyflex EDM group (n = 15) and VDW.ROTATE (n = 15) and article 2 ProTaper Next group ( n = 10), Hyflex EDM (n = 10) and VDW.ROTATE (n = 10). As an irrigating solution, 1% sodium hypochlorite was used and supplemented with passive ultrasonic irrigation (PUI). Canal and dentin volume, canal diameter, dentin thickness, transport, centralization, accumulation of debris and untouched areas were measured. The data obtained were statistically evaluated using the t-test, Mann Whitney, ANOVA and Tukey or Kruskal Wallis and Dunn's tests and the significance level was set at 5% (P <0.05). The comparison between Hyflex EDM and VDW.ROTATE did not reveal significant differences for channel volume, channel diameter, untouched areas and accumulation of debris (P> 0.05). There was a significant difference (P <0.05) in the reduction in cervical volume and total dentin after using the VDW.ROTATE 60.04 instrument. However, in the intragroup comparison, with the increase in dilation with instruments with larger diameters, there were significant differences (P <0.05) in the volume of the canal and dentin, in addition to the diameter in the M-D direction in both groups. There was no significant difference for the three groups (P> 0.05) for the analyzed data. It was concluded that the Hyflex EDM, VDW.ROTATE and PropTaper Next systems provided an effective and safe preparation in mesial root canals with marked curvature of the upper molars. The enlargement of the canal was associated with an increase in the volume of the root canal, a decrease in the accumulation of debris and untouched areas with maintenance of the dentinal structure. Although different metallic alloys were evaluated, we observed that there were no significant differences in the changes caused in the preparation made in the curved roots canals.
