    Abstract
    Resumo: A hidronefrose por obstrução da junção ureteropélvica (OJUP) é tratada cirurgicamente quando observa-se a perda de função renal associada ao padrão obstrutivo na cintilografia renal. Com base na ultrassonografia, alguns pacientes apresentam evidências de necessidade de cirurgia precoce e o escore de predição de pieloplastia (Pyeloplasty Prediction Score, PPS) é uma ferramenta desenvolvida que pode ajudar a identificar estes pacientes. Neste trabalho foram extraídos dados a partir do levantamento retrospectivo de prontuários de pacientes submetidos à pieloplastia para tratamento de OJUP e de crianças saudáveis submetidas à ecografia. O grupo de estudo foi selecionado a partir das 575 pieloplastias realizadas no período de 2012 a 2022. O grupo controle foi composto por 30 crianças. Com base nos dados de prontuário, foram levantados os dados ultrassonográficos e calculado o PPS. Também foram feitas análises das medidas do rim doente com o rim contralateral de cada paciente do grupo de casos. No grupo de estudo, apenas 120 pacientes atenderam aos critérios de seleção, destes, 83 (69,17%) tinham PPS pré-operatório = 8. Observou-se que o rim doente é maior que o contralateral longitudinalmente. Não houve diferença quanto ao gênero, à lateralidade e à etiologia. Apesar de o PPS ser um critério objetivo para indicação da pieloplastia, amplamente disponível e de baixo custo, não foi compatível com a indicação padrão (clínica e cintilografia renal) em 30% dos casos. Não foi possível estabelecer alguma justificativa para a diferença de indicação. Com base nesta casuística, não é possível afirmar que o PPS pode ser usado isoladamente para a indicação da pieloplastia, sem necessidade da cintilografia renal estática
     
    Abstract: Hydronephrosis due to ureteropelvic junction obstruction (UPJO) is treated surgically when loss of renal function associated with an obstructive pattern is observed on renal scintigraphy. Based on ultrasound, some patients show evidence of the need for early surgery, and the Pyeloplasty Prediction Score (PPS) is a developed tool that can help identify these patients. In this study, data were extracted from a retrospective review of medical records of patients who underwent pyeloplasty for the treatment of UPJO and of healthy children who underwent ultrasound. The study group was selected from the 575 surgeries performed between 2012 and 2022. The control group consisted of 30 healthy children. Based on the medical records, ultrasound data were collected and the PPS was calculated. The measurements of the diseased kidney compared with the contralateral kidney of each patient in the case group were also analyzed. In the study group, only 120 patients met the selection criteria, of which 83 (69.17%) had a preoperative PPS = 8. The diseased kidney was larger than the contralateral kidney longitudinally. There was no difference regarding gender, laterality, or etiology. Although the PPS is an objective criterion for indicating pyeloplasty, widely available and low cost, it was not compatible with the standard indication (clinical and renal scintigraphy) in 30% of the cases. It was not possible to establish any justification for the difference. Based on this case series, it is not possible to state that the PPS can be used alone for indicating pyeloplasty, without the need for static renal scintigraphy
