Doenças degenerativas e disfunção temporomandibular, um relato de caso

Abstract

Resumo : A Artrite reumatoide (AR) é uma doença inflamatória autoimune que acomete a membrana sinovial das articulações. Embora não seja usual o acometimento da articulação temporomandibular (ATM) de forma primária, ela pode ser afetada com a evolução da doença, gerando um quadro de osteoartrite articular. Essa condição pode provocar dor quando as alterações ósseas estão ativas, além de disfunção mecânica e estrutural, má oclusão dentária e redução na qualidade de vida relacionada à saúde podem estar presentes. Assim, o objetivo deste trabalho é relatar o caso de uma paciente do sexo feminino, de 57 anos, atendida na disciplina de Disfunção Temporomandibular e Dor Orofacial da Universidade Federal do Paraná, que relatava sentir dores de ouvido do lado direito e possuir bruxismo há 15 anos, além de ser diagnosticada com AR há 22 anos. Após avaliação clínica detalhada, a paciente foi diagnosticada com artrite sistêmica envolvendo manifestação nas ATM, dor miofascial, e bruxismo do sono. O tratamento indicado foi a utilização de dispositivo interoclusal de estabilização e exercícios mandibulares de alongamento. Com estabilidade do quadro durante os 3 anos de acompanhamento e uma piora no retorno pós pandemia, tanto para sensação dolorosa, quanto para os parâmetros de movimento mandibular. No entanto, a paciente ainda se encontra em acompanhamento. Conclui-se que quando há manifestação de artrite sistêmica nas ATM, o tratamento conservador e multidisciplinar, envolvendo o cirurgião-dentista além do reumatologista, contribuem para o sucesso clínico.

Abstract : Rheumatoid arthritis (RA) is an autoimmune inflammatory disease that affects the synovial membrane of joints. Although it is not usual to affect the temporomandibular joint (TMJ) in a primary way, it can be affected with the evolution of the disease, generating a condition of articular osteoarthritis. This condition can cause pain when bone changes are active, in addition to mechanical and structural dysfunction, dental malocclusion and a reduction in health-related quality of life may be present. The aim of the presente case report was to evaluate a 57 years old female, attended in the discipline of Temporomandibular Disorders and Orofacial Pain at the Federal University of Paraná, who reported feeling ear pain on the right side and having bruxism for 15 years, in addition to being diagnosed with RA for 22 years. After clinical evaluation in detail, the patient was diagnosed with systemic arthritis involving TMJ manifestation, myofascial pain, and sleep bruxism. The indicated treatment was the use of an interocclusal stabilization device and mandibular stretching exercises. With stability of the condition during the 3 years of follow-up and a worsening in the post-pandemic return, both for painful sensation and for mandibular movement parameters. However, the patient is still being followed up. It is concluded that when there is a manifestation of systemic arthritis in the TMJ, conservative and multidisciplinary treatment, involving the dentist in addition to the rheumatologist, contribute to clinical success.
