Qual a resposta dos fibroblastos gengivais frente ao desafio microbiano do biofilme dental? : uma revisão sistemática

Resumo: A periodontite é uma doença infecto-inflamatória, crônica, multifatorial associada a um biofilme disbiótico. Clinicamente se caracteriza pela perda progressiva dos tecidos de proteção e sustentação do dente, que ocorre quando não tratada. Esta destruição periodontal é causada, principalmente, pela resposta do sistema imunológico do hospedeiro às bactérias e seus produtos. Os fibroblastos gengivais (FG) são o principal tipo celular do periodonto, principalmente nos tecidos conjuntivos. Entre suas funções se destacam a secreção de mediadores inflamatórios e sua ligação à manutenção, reparo e regeneração do tecido conjuntivo periodontal. No entanto, ainda faltam informações na literatura sobre a resposta celular dos FG frente ao biofilme dental disbiótico e como isso interfere na patogênese da periodontite. Dessa forma, uma revisão sistemática foi realizada com objetivo de responder à seguinte questão: qual a resposta dos fibroblastos gengivais frente ao desafio microbiano do biofilme dental? As bases de dados PubMed, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), EMBASE e Bibliografia Brasileira de Odontologia (BBO) via Biblioteca Virtual em Saúde (BVS) foram consultadas. Não foram feitas restrições de ano ou idioma. Os descritores padronizados (Medical Subject Headings, MeSH) utilizados foram: "fibroblasts", "biofilm", "periodontitis" e "inflammatory response", com o uso do operador booleano "AND" para uma combinação de fibroblastos e periodontite com biofilme ou resposta inflamatória. A avaliação de risco de viés foi realizada por meio da ferramenta adaptada ROBINS-I. Como resultados um total de 1.074 títulos foram encontrados, dos quais 12 artigos foram incluídos nesta revisão sistemática. Destes, onze estudos foram considerados de baixo risco e 1, com risco moderado de viés. Em seis estudos o desafio microbiano foi realizado a partir de um biofilme in vitro, e nos demais, o desafio foi em pacientes com periodontite. Em todos os artigos a regulação de citocinas por fibroblastos gengivais (FG) foi considerada como influente na resposta inflamatória na doença periodontal, com aumento na expressão de Interleucina 6, COX-2, RANK/RANKL, CSF1 e IL-34, e redução nas expressões de IL-1ß quando desafiado por biofilmes patogênicos. Assim, apesar da heterogeneidade dos estudos avaliados, que não permitiu uma avaliação mais aprofundada do tema, observou-se que os fibroblastos gengivais reagem com um perfil predominantemente pró-inflamatório diante da exposição ao biofilme dental, podendo então atuar tanto para o início quanto para a progressão da periodontite. Abstract: Periodontitis is a chronic, multifactorial, infectious inflammatory disease associated with a dysbiotic biofilm. Clinically, it is characterized by the progressive loss of protection's and support's tissues of the tooth, which occurs when untreated. This periodontal destruction is mainly caused by the response of the host's immune system to bacteria and their products. Gingival fibroblasts (GF) are the main cell type of the periodontium, mainly in connective tissues. Among its functions are the secretion of inflammatory mediators and their connection to the maintenance, repair and regeneration of periodontal connective tissue. However, there is still a lack of information in the literature on the cellular response of GF against dysbiotic dental biofilm and how this interferes with the pathogenesis of periodontitis. In this way, a systematic review was carried out in order to answer the following focused question: what is the response of gingival fibroblasts to the microbial challenge of dental biofilm? PubMed, Latin American and Caribbean Health Sciences Literature (LILACS), EMBASE and Brazilian Bibliography of Dentistry (BBO) databases were searched by Virtual Health Library (BVS). No year or language restrictions were made. The standardized descriptors (Medical Subject Headings, MeSH) used were: "fibroblasts", "biofilm", "periodontitis", and "inflammatory response", with the use of the Boolean operators "AND" for a combination of fibroblasts and periodontitis with biofilm or inflammatory response. From a total of 1.074 titles found 12 articles were included. Bias risk assessment was performed for ROBINS-I adapted tool. 11 studies were considered to have low risk and 1, moderate risk of bias. Thus, six studies performed the biofilm challenge from the in vitro model. In all the articles included cytokine regulation by fibroblasts was found to influence the inflammatory response in periodontal disease, with an increase of IL-6, COX- 2, RANK/RANKL, CSF1 and IL-34, and a reduction in IL-1ß expressions when challenged by pathogenic biofilms. Conclusion: Besides the heterogeneity of the studies evaluated, it was observed that gingival fibroblasts react with a predominantly pro-inflammatory profile when exposed to dental biofilm, and can therefore act both for the onset and for the progression of periodontitis.
