Sofrimento psíquico e escuta de vozes : um estudo a partir da determinação social do processo saúde-doença e da psicologia histórico-cultural

Abstract

Resumo: Este trabalho buscou analisar os processos envolvidos na determinação social da escuta de vozes, fundamentando-se no diálogo teórico-prático entre dois campos do conhecimento: a Saúde Coletiva e a Psicologia Histórico-Cultural, ambos orientados pelo materialismo histórico-dialético. Historicamente, a escuta de vozes é amplamente associada a sintomas da esquizofrenia e de outros transtornos psicóticos, e a produção de conhecimento sobre essa experiência tem sido guiada por perspectivas hegemônicas baseadas em concepções biológicas e na ideologia dominante nas ciências. Esta pesquisa alinha-se aos esforços do Movimento Internacional de Ouvidores de Vozes, que busca desenvolver compreensões e estratégias de cuidado alternativas às abordagens tradicionais. Partindo da premissa de que o processo saúdedoença é uma expressão particular do processo social mais amplo, procurou-se investigar, a partir de histórias de vida e cursos do desenvolvimento humano, os processos histórico-sociais que se evidenciam na gênese e no desenvolvimento das experiências de escuta de vozes e sofrimento psíquico de pessoas atendidos em serviço de saúde mental do SUS e em grupos de apoio autônomos. Para isso, foi realizado uma revisão integrativa da literatura e uma pesquisa de campo, que incluiu observação participante e entrevistas com dez pessoas. Cinco delas eram participantes de grupos de ouvidores de vozes; e as outras cinco eram usuários de um serviço de saúde mental, não participantes de grupos de ouvidores de vozes. A revisão de literatura permitiu identificar como a escuta de vozes é descrita nos estudos e quais aproximações possíveis com a teoria da determinação social. Para aprofundar o entendimento das alterações na dinâmica da personalidade envolvidas na esquizofrenia, foram sintetizadas algumas formulações da Psicologia Histórico-Cultural sobre o desenvolvimento humano, a formação da personalidade e suas alterações patológicas. Por fim, os resultados foram sistematizados nas seguintes categorias: circunstâncias desagregadoras e as bases vivenciais do sofrimento; o conteúdo das vozes e relação com a história de vida; sistemas de significação atribuído as vozes, relações entre sentido e significado. Em um nível mais geral de análise, defende-se a tese de que a gênese da escuta de vozes está vinculada a processos críticos da vida social. Esses processos expressam as crescentes exigências psíquicas, o não atendimento de necessidades humanas fundamentais e as obstruções ao modo de viver a vida. A escuta de vozes, assim, emerge como uma denúncia de condições sociais que afetam profundamente os indivíduos

Abstract: This study sought to analyze the processes involved in the social determination of hearing voices, based on the theoretical-practical dialogue between two fields of knowledge: Collective Health and Historical-Cultural Psychology, both guided by historical-dialectical materialism. Historically, hearing voices has been widely associated with symptoms of schizophrenia and other psychotic disorders, and the production of knowledge about this experience has been guided by hegemonic perspectives based on biological conceptions and the dominant ideology in the sciences. This research is in line with the efforts of the International Movement of Voice Hearers, which seeks to develop alternative understandings and care strategies as opposed to traditional approaches. Starting from the premise that the health-disease process is a particular expression of the broader social process, we sought to investigate, based on life histories and courses of human development, the historical-social processes that are evident in the genesis and development of the experiences of hearing voices and psychological suffering of people treated in SUS mental health services and autonomous support groups. To this end, an integrative literature review and field research were carried out, which included participant observation and interviews with ten people. Five of them were participants in voice-hearing groups; the other five were users of a mental health service who were not participants in voicehearing groups. The literature review made it possible to identify how voice hearing is described in studies and what approximations are possible with the theory of social determination. In order to deepen our understanding of the changes in personality dynamics involved in schizophrenia, we synthesized some formulations from Historical-Cultural Psychology on human development, the formation of personality and its pathological changes. Finally, the results were systematized into the following categories: disaggregating circumstances and the experiential basis of suffering; the content of the voices and their relationship to life history; systems of meaning attributed to the voices. On a more general level of analysis, the thesis is that the genesis of hearing voices is linked to critical processes in social life. These processes express psychological demands, the failure to meet fundamental human needs and obstructions to the way life is lived. Hearing voices thus emerges as a denunciation of social conditions that profoundly affect individuals
