Displasias ectodérmicas do grupo A : elaboração de um banco de dados informatizado para auxílio ao diagnóstico genético-clínico

Resumo: As displasias ectodérmicas são afecções em que ocorre comprometimento das estruturas derivadas da ectoderme, como a epiderme e seus anexos, constituindo um grupo nosológico grande e complexo, composto de entidades raras e com etiologia genética (em sua maioria). FREIRE-MAIA em 1971 e 1977, propôs uma definição clínica e uma classificação clínico-mnemônica das displasias ectodérmicas, hoje utilizada por pesquisadores e clínicos do mundo todo. Esta classificação divide-as em 2 grupos principais (A e B), de acordo com a presença de sinais clássicos em estruturas relacionadas a pelos, dentes, unhas e glândulas sudoríparas. Este projeto teve como objetivos principais: 1) A análise da literatura sobre as displasias ectodérmicas já descritas, com relação aos aspectos genéticos e clínicos. 2) A atualização da distribuição das displasias ectodérmicas na classificação de FREIRE-MAIA (1971, 1977), revista por PINHEIRO e FREIRE-MAIA (1994), com a realização das alterações necessárias no grupo A (união de afecções descritas anteriormente como distintas, mudanças de subgrupo, inclusão de novas afecções encontradas na literatura). 3) A criação de um sistema de controle informatizado (disponível em Home Page na Internet), responsável pelo gerenciamento de um banco de dados contendo as informações obtidas, bem como um banco de imagens selecionadas, referentes às displasias ectodérmicas conhecidas e também um sistema de pesquisa, onde as informações fornecidas pelo usuário (de acordo com os sinais clínicos do paciente) são confrontadas com o banco de dados, resultando na filtragem das possíveis displasias ectodérmicas. O banco de dados encontra-se disponível na Internet no endereço http://displasias.bio.ufpr.br. Cada afecção foi analisada individualmente, tendo sido elaborado para cada uma delas uma "ficha", com os seguintes dados: informações gerais, aspectos genéticos, sinais clínicos clássicos, outros sinais frequentes, comentários e referências bibliográficas utilizadas. Um banco de imagens e sistemas de pesquisa (por palavra chave e por critérios clínicos) também foram disponibilizados. Este trabalho ampliou o número de displasias ectodérmicas conhecidas para 192 (grupo A). Os subgrupos com maior número de afecções são os chamados 1-2-3 e 1-2-3-4, com 48 e 40 displasias respectivamente. Seguem-se os subgrupos 1-2 com 34 afecções, o subgrupo 1-3 com 25 e o subgrupo 2-3 com 15 displasias. Os demais subgrupos contém um número inferior a 10 displasias. A maioria das displasias ectodérmicas tem herança autossômica recessiva (33,9%), seguidas das que possuem herança autossômica dominante (27,6%). As displasias com herança ligada ao cromossomo X constituem 5,7% e os 32,8% restantes incluem síndromes cuja etiologia é desconhecida. Abstract: The ectodermal dysplasias are conditions with involvement of the structures derived of the ectoderm, as the epidermis and its enclosures, constituting a large and complex nosologic group, composed of rare entities with genetic etiology (in its majority). FREIRE-MAIA, in 1971 and 1977, proposed a clinical definition and a clinical-mnemonic classification of the ectodermal dysplasias, used today by researchers and clinicians of the whole world. This classification divides them in 2 main groups (A and B), according to the presence of classic signs in structures related to hair, teeth, fingernails and sweat glands. This project had as main objectives: 1) The analysis of the literature about the ectodermal dysplasias already described, concerning genetic and clinical aspects. 2) The update of the ectodermal dysplasias in FREIRE-MAIA's classification (1971, 1977), reviewed by PINHEIRO and FREIRE-MAIA (1994), with the accomplishment of the necessary alterations in group A (union of conditions described previously as different, subgroup changes, inclusion of new conditions found in the literature). 3) A creation of a computerized control system (available in Home Page in the Internet), responsible for the management of a database that contains the obtained information, as well as a bank of selected images, referring to the known ectodermal dysplasias and also a research system, where the information supplied by the user (in agreement with the patient's clinical signs) are confronted with the database, resulting in the filtering of the possible ectodermal dysplasias. The database is available in the Internet in the address http://displasias.bio.ufpr.br. Each condition was analyzed individually, having been elaborated for each one of them one "file card", with the following data: general information, genetic aspects, "classic" clinical signs, other frequent signs, commentaries and bibliographical references. A photo bank and research systems (key words and clinical criteria) also are available. This work enlarged the number of known ectodermal dysplasias to 192 (group A). The subgroups with a larger number of conditions are the called 1-2-3 and 1-2-3-4, with 48 and 40 dysplasias respectively. They are followed by subgroup 1-2 with 34 conditions, subgroup 1-3 with 25 and subgroup 2-3 with 15 dysplasias. The other subgroups contain an inferior number of 10 dysplasias. Most of the ectodermal dysplasias have autossomal recessive inheritance (38%), followed by the ones that possess autossomal dominant inheritance (28%). The Xlinked dysplasias constitute 5% and the 29% remaining ones include syndromes whose etiology is not known.
