Permeabilidade da microcirculação e translocação bacteriana em vigencia de ascite induzida pelo sarcoma 180 : Estudo experimental

Resumo: Este estudo tem por objetivo pesquisar a translocação bacteriana em ratos, em vigência de ascite decorrente do processo tumoral induzido pelo Sarcoma 180. Para este propósito, recorreu-se à pesquisa de bactérias indicadoras de contaminação com flora enteral em sangue, líquido ascítico e órgãos. Determinou-se as concentrações de proteínas totais, albumina, sódio e potássio e avaliou-se a permeabilidade vascular intestinal, peritoneal e de tecido mesentérico. Foram utilizados 70 ratos Wistar, machos, com pesos compreendidos entre 212-257g. Os animais foram divididos em grupos A, B, C, D, E (evolução tumoral) com 10 ratos cada, os quais foram submetidos à inoculação de células tumorais intraperitonealmente e grupo F (controle) com 20 ratos. Nos periodos pré-determinados: 5º, 6º, 7º, 8º e 9º dias, os animais sob evolução tumoral foram avaliados: pela pesquisa bacteriológica através de amostras do sangue, líquido ascítico, nódulos linfáticos mesentéricos, pulmões, rins, fígado e baço, dosagens séncas e ascíticas de proteínas totais, albumina, sódio e potássio, contagem de leucócitos e determinação das concentrações do corante Azul de Evans extraído pela formamida 8 horas após inoculação do mesmo por via endovenosa Os resultados da pesquisa bacteriológica demonstraram translocação de Escheríchia coli e Enterococcus faecalis para 100% de todos os órgãos e líquido ascítico. a partir do 7º dia de evolução tumoral, comparada a 0% do grupo controle, ao passo que não houve isolamento de bactérias em amostras de sangue de nenhum rato dos grupos estudados, controle ou sob evolução tumoral Os resultados das dosagens de proteínas totais no soro, demonstraram diminuição estatisticamente significativa nos ratos sob evolução tumoral, em relação ao grupo controle. Ocorreu uma inversão do gradiente de albumina soro-ascite somente no grupo C (7º dia de evolução tumoral). As concentrações de sódio foram maiores no plasma que em ascite, enquanto que as concentrações de potássio, apresentaram esta relação inversa. A contagem de leucócitos demonstrou leucopema progressiva. Nos animais sob evolução tumoral foi observada a permeabilidade vascular aumentada em paredes intestinal e peritoneal e tecido mesentérico a partir do 5o dia de evolução tumoral Conclui-se que ocorreu translocação bacteriana em ratos com ascite induzida por tumor, e que provavelmente a permeabilidade vascular aumentada, associada ao desequilíbrio iónico e protéico, e a ieucopenia progressiva, sejam neste experimento, os fatores relacionadas à translocação bacteriana. Abstract: The aim of the present study, was to research the bacterial translocation in rats in the presence of ascitic fluid induced by the Sarcoma 180. This proposal was evaluated for the presence of enteric bacteria indicating fecal contamination in blood, ascitic fluid and tissues. The concentrations of total protein, albumin, sodium and potassium, and the evaluation of peritoneal, intestinal and mesenteric vascular permeability were determined. Seventy Wistar rats, all males, weighting from 212 to 257g were used. The animals were divided in groups A. B, C, D and E, (tumoral evolution), each of them with 10 rats, they were submitted to inoculation of tumoral cells intraperitoneally, and group F, (control) with 20 rats. On pre-determined days 5th. 6th. 7th, 8th and 9th the animals in tumoral evolution, were bacteriologically evaluated through samples of blood, ascitic fluid, mesenteric nodes, lung, kidney, liver and spleen , by determining senc and ascitic fluid of total protein, albumin, sodium, potassium and by determining the concentration of leukocytes and Evans blue, extracted by formamid, 8 hours after injected intravenous. The results of bacteriologic research showed that bacterial translocation was present in all organs and ascitic fluid in 100% of the animals, after the 7th day of tumoral evolution and in none of the animals in the control group, although, no bacteria was present in the blood of any groups of animals studied, control or tumoral evolution. The results of seric total protein, showed lower statistic meaning on rats with tumoral evolution, when related to the control group. The serum-ascitic albumin gradient was present only in group C (7th day of tumoral evolution). The seric sodium concentration showed to be bigger in the plasma than in the ascitic fluid, while the potassium concentration, showed an inverse relation. The leukocytes concentration showed leucopema progressive. The vascular permeability was increased in the peritonea! and intestinal wall and mesenteric tissue since de 5th day of tumoral evolution. The results obtained conclude that, there was bacterial translocation in ascitic fluid induced by tumor and probably the increased permeability associated to ionic and protein desequilibrium and leucopenia progressive, were in this experiment related to bacterial translocation.
