Alterações oftalmológicas em pacientes com anemia de Fanconi

Resumo: A Anemia de Fanconi (AF) é uma doença rara, geralmente herdada de maneira autossômica recessiva, caracterizada por múltiplas malformações congênitas, falência da medula óssea e suscetibilidade à câncer. Esta tese tem por objetivos descrever a prevalência das alterações oftalmológicas adquiridas em pacientes com AF submetidos ou não ao Transplante de Células-Tronco Hematopoiéticas (TCTH), além de descrever e relacionar as alterações oftalmológicas congênitas com os subtipos genéticos encontrados. Foi realizado um estudo observacional, descritivo, transversal e retrospectivo em 106 pacientes acompanhados no Serviço de Transplante de Medula Óssea do Complexo Hospital de Clínicas da Universidade Federal do Paraná, durante o período de novembro 2014 a agosto de 2017. Cada participante foi submetido a avaliação oftalmológica completa e, em 84 deles, realizada a ecografia ocular. A mediana da faixa etária foi de 13 anos, variando entre 6 meses e 43 anos. A acuidade visual melhor a 20/60 foi observada em 91,5% dos pacientes e os erros refracionais, em 68%. A microftalmia foi a anormalidade oftalmológica mais detectada (95,2%). Ainda, foram observados redução das medidas antropométricas como o comprimento da fenda palpebral (78/103 pacientes - 75,7%), microcórnea (48/103 pacientes - 46,6%) e ptose palpebral(31/103 pacientes - 30%). Com uma frequência menor, foram encontrados tecido epirretiniano sobre o disco óptico (14,1%), hipoplasia de disco (8,5%), estrabismo (7,5%), catarata congênita (0,9%), glaucoma congênito (0,9%) e presença de artéria hialóde (0,9%). Ressalta-se que neste estudo foi descrita uma nova alteração oftalmológica em 15 pacientes com AF: tecido epirretiniano sobre o disco óptico. O subtipo genético foi identificado em 78 probandos (79,6%) e o subtipo FA-A foi o mais frequente (62,8%), seguido do FA-G, FA-E, FA-C. A manifestação ocular adquirida mais encontrada nos pacientes que não receberam TCTH (44 pacientes) foi a neovascularização limbar (13,6%), enquanto que em pacientes que receberam TCTH (62 pacientes), a doença do enxerto contra hospedeiro (DECH) ocular (51,6 %) foi a manifestação mais encontrada. O sintoma mais comum da DECH ocular foi a ceratoconjuntivite sicca (29%). Muitas manifestações oculares foram identificadas nos pacientes com AF. Exames oftalmológicos regulares e cuidadosos são essenciais para o diagnóstico dessas condições. Abstract: Fanconi anemia (FA) is a rare disease, usually inherited in an autosomal recessive manner, characterized by multiple congenital malformations, bone marrow failure, and susceptibility to cancer. The purpose of this thesis is to describe the prevalence of acquired ocular manifestations in FA patients who did or did not undergo hematopoietic stem cell transplantation (HSCT) and to describe and correlate the congenital ocular manifestations with the genetic subtypes of the disease. This is an observacional, descriptive, cross-sectional and retrospective study of 106 patients followed at the Bone Marrow Transplantation Service of the Complexo Hospital de Clínicas of the Federal University of Parana, from November 2014 and August 2017 with patients who had diagnostic confirmation of FA. Each participant underwent a complete ophthalmological evaluation and 84 patients were submitted to an ocular ultrasound exam. The patients ranged in age from 6 months to 43 years (median, 13 years). Visual acuity was greater than 20/60 in 91.5% of the patients and refractive errors occurred in 68%. Microphthalmia was the most common ophthalmic abnormality (95,2%). A reduction in anthropometric measurements was observed, including palpebral fissure length (78/103 patients - 75,7%), microcornea (48/103 patients - 46,6%), and palpebral ptosis (31/103 patients - 30%). At lower frequencies, were found strabismus (7,5%), congenital cataracts (0,9%), congenital glaucoma (0,9%), and presence of hyaloid artery (0,9%). We identified a new ophthalmic condition in 15 patients with FA, i.e., epiretinal tissue on the optic disc. The genetic subtype was identified in 78 patients (79,6%), and the FA-A subtype was most prevalent (62,8%) followed by FA-G, FA-E, FA-C. The most common acquired ocular manifestation in patients who did not undergo HSCT (44 patients) was limbal neovascularization (13,6%), while in patients who underwent HSCT (62 patients), the GVHD related manifestation was ocular graft-versus-host disease (GVHD) (51,6%). The most frequent symptom of ocular GVHD was keratoconjunctivitis sicca (29%). Several ocular manifestations were identified in patients with FA. Careful and regular ophthalmologic examinations are essential to diagnose these conditions.
