Padrões semiológicos da cefaleia após o tratamento endovascular do aneurisma intracraniano roto

Abstract

Resumo: As características evolutivas das cefaleias primárias, em pacientes portadores de aneurisma cerebral roto embolizado, não têm sido investigado até o momento. Avaliamos o padrão da cefaleia em pacientes que sofreram ruptura de aneurisma cerebral e tratamento endovascular, no período de um ano antes da ruptura, e avaliamos prospectivamente o comportamento da cefaleia por até 12 meses após tratamento endovascular. O objetivo do estudo foi avaliar a influência do tratamento endovascular do aneurisma intracraniano roto, no padrão da cefaleia primária prévia ou o no surgimento de algum novo tipo de cefaleia. Sessenta pacientes foram avaliados no período de 1º de junho de 2013 a 1º de junho de 2014, seguindo um questionário de cefaléia elaborado pelos autores e aplicado no momento do internamento. Esses pacientes foram contatados em 3, 6 e 12 meses após o tratamento para finalizar o estudo preenchendo este questionário. Dos pacientes estudados 37 apresentavam cefaleia antes da ruptura, sendo 16 com cefaleia do tipo tensional, 11 com migrânea sem aura, 9 migrânea com aura e 1 com características inespecíficas. Houve redução significativa da frequência da dor por até 12 meses em portadores de cefaleia do tipo tensional e migrânea sem aura, também houve redução da intensidade da dor, no mesmo período, nos 3 grupos de pacientes. Concluímos que houve redução das cefaleias primárias após tratamento endovascular do aneurisma cerebral roto. Palavras-chave: aneurisma cerebral; cefaleia; migrânea

Abstract: The characteristics of primary headaches in patients with ruptured brain aneurysm embolization have not been well understood. We evaluated the pattern of headache in patients who suffered a ruptured cerebral aneurysm and endovascular treatment within one year before the rupture, and prospectively evaluated the characteristics of headache for up to 12 months after endovascular treatment. This study was conducted to evaluate the influence of endovascular treatment of ruptured intracranial aneurysm, the pattern of previous primary headache or the emergence of a new type of headache. Methods: Sixty patients were evaluated during the period from June 1st, 2013 to June 1st, 2014, and a questionnaire with questions about headache was applied at the time of admission. These patients were contacted 3, 6 and 12 months after treatment to complete the study by filling out follow-up questionnaire on the headache. Results: 60 patients were evaluated, but only 37 had headache before the rupture, 16 with tension-type headache, 11 with migraine without aura, 9 with migraine with aura with and 1 with nonspecific characteristics. There was a significant reduction in the frequency of pain for up to 12 months for patients with tension-type and migraine without aura, but there was a reduction in pain intensity over the same period in 3 groups of patients. Conclusion: there was a reduction of the primary headache after endovascular treatment of ruptured brain aneurysm. Key words: cerebral aneurysm; headache; migraine
