Fosfatase alcalina placentária como marcador tumoral para neoplasias seminomatosas do testículo

Resumo: Alfa-feto-proteína (AFP) e a gonadotrofina coriônica humana (~-hCG) são marcadores tumorais clinicamente importantes para o diagnóstico e controle de tratamento dos tumores de células germinativas do testículo (TGT). Com exceção do ~-hCG, que serve como marcador em 10 a 20 % dos seminomas, não existe para o tipo histológico mais freqüente de TGT nenhum marcador. O valor clínico das isoenzimas da fosfatase alcalina placentária (PLAP) tem sido discutido de maneira controversa. No presente estudo foram avaliadas a sensibilidade, a especificidade e a importância deste marcador para como parâmetro de controle para os TGT. Entre agosto de 1989 e dezembro de 1994 foram determinados os valores séricos dos marcadores tumorais AFP, ~-hCG e PLAP em 136 pacientes (média de idade de 35,5 anos), os quais foram tratados na Clínica Urológica do Departamento de Urologia da Universidade Heinrich-Heine de Düsseldorf. A determinação da PLAP foi realizada enzimaticamente em 480 amostras do soro. Nesta reação naftol-AS-MX-fosfato { (3- hidroxi-2-ácido naftóico 2,4-dimetilanilida)-fosfato} foi defosforilado a um produto fluorescente sob pH básico (10.2). Valores menores que 3,75!Jmolfl foram considerados como normais. A média do tempo de seguimento para os pacientes com TGT e PLAP elevado foi de 30 meses. Sessenta e oito pacientes apresentaram um TGT e 60 uma doença benigna dos órgãos da bolsa escrotal ou do trato urinário. Entre os 68 pacientes se encontravam 33 com seminomas puros assim como 22 com tumores mistos que continham elementos seminomatosos. A maior sensibilidade foi verificada entre pacientes com seminomas puros (40,7 %), seguido de uma sensibilidade de 37,5 % para os tumores mistos com elementos seminomatosos. Não se verificou nenhuma correlação entre a magnitude dos valores séricos da PLAP e o estádio tumoral. 16 dos 17 pacientes com níveis séricos da PLAP elevados préoperatoriamente demonstraram uma diminuição destes correspondente à evolução da doença. O seguimento não foi possível para um paciente. 16,6% dos pacientes com doenças benignas dos órgãos da bolsa escrotal ou do trato urinário apresentaram resultados falsos positivos. Caso PLAP venha a ser utilizada como controle no seguimento após uma terapia primária, o hábito de fumar do paciente necessita ser considerado para evitar erros de interpretação. Este hábito pode determinar valores falso positivos. Relevante é a cinética de diminuição deste marcador. O método escolhido para a determinação da PLAP pode influenciar a sensibilidade e especificidade. Devido aos resultados apresentados neste estudo, PLAP pode ser considerada, com uma sensibilidade de 41 %e uma especificidade de 83,3 %, como marcador potencial no diagnóstico e acompanhamento de pacientes com seminomas puros ou tumores mistos com componentes seminomatosos. Apesar da limitação da especificidade, muitos estudos apóiam a utilização da PLAP para os seminomas, tumores estes sem marcadores tumorais mais sensíveis. Abstract: The study of alpha-fetoprotein (AFP) and human chorionic gonadotropin (hCG) is clinically useful in the diagnosis and follow-up of patients with germ cell testis cancer. For seminoma as the most frequent testis cancer, however, no sensitive or specific tumor marker but hCG, which is preoperatively elevated in about only 10 to 20 % of the patients, is available. The role of the isozymes of the tumor marker placental alkaline phosphatase (PLAP) remains controversial. This study aims the determination of the specifity and sensitivity of PLAP among these patients. From August 1989 until December 1994, 136 patients (mean 35,5 years old), treated at the Department of Urology, University of Düsseldorf, underwent tumor marker evaluations (AFP, hCG and PLAP). A sensitive enzymatic assay involving the fluorogenic substrate naphtol AS-MX phosphate { (3-hydroxy- 2-naphtoic acid 2,4-dimethylanilid)-phosphate} under pH o f 10,2 was utilized to determine the levels of PLAP in a total of 480 serum probes. This reaction of dephosphorilation under pH of 10,2 allows the formation of a fluorogenic product. Values under 3,75 JJmol/1 were considered normal. Patients with testis cancer and preoperative elevated PLAP were followed (mean follow-up of 30 months). Sixty-eight patients had a germ cell tumor of the testis and 60 patients a benign disease of the scrotum or urinary tract. 33 and 22 out of the 68 patients with testis cancer had pure seminomas and mixed tumors with seminomatous elements, respectively. The highest incidence (40,7%) was among pure seminomas, followed by composed tumors with seminomatous elements (37,5%). No correlation between the magnitude of elevation and stage could be seen. 16 of the 17 patients with preoperative elevated PLAP-levels demonstrated a decrease along with therapy according to clinicai findings. One patient has lost to follow-up. Among patients with benign diseases of the scrotum and of the urinary tract, 16,6% showed false positive results. If PLAP is employed in the follow-up after primary therapy, the smoking habits of the patients should be considered in order to avoid false interpretations. Smoking habit may lead to false positive values. The sensitivity and specificity depend on the method of determination of PLAP. According to the results of this study PLAP may be considered, with a sensitivity of 41 % and a specifity of 83,3 %, a potential marker in the diagnosis and follow-up of patients with pure seminomas and composed tumors with seminomatous elements. Although a limitation of specificity could be observed, several studies support the determination of PLAP for seminomas, tumors that do not have markers with higher sensitivity.
