Efeitos da hidrocortisona e do diclofenaco na fase inflamatória da cicatrização colônica : estudo experimental em ratos

Resumo: O presente estudo objetivou verificar os possíveis efeitos da hidrocortisona e do diclofenaco na fase inflamatória da cicatrização, pela avaliação da pressão de ruptura à insuflação e das alterações de permeabilidade vascular, nas anastomoses colônicas em ratos Utilizou-se 56 ratos Wistar, machos, divididos em 3 grupos grupo C ( controle ) com 17 animais; grupo E1 (glicocorticóide) com 18 ratos: grupo E2 ( diclofenaco ) com 21 animais Subdividiu-se os grupos em subgrupos de acordo com o período de avaliação: 3.° e 7.° dias P.O. Realizou-se anastomose colônica a 5 cm da borda anal em todos os grupos Puncionou-se a veia peniana para inoculação de Azul de Evans, na dose de 20mg/Kg, conforme datas pré-estabelecidas e, após 8 horas, sacrificaram-se os animais. Avaliou-se a pressão de ruptura à insuflação nos segmentos que continham a anastomose, os quais foram submetidos ao micrométodo espectrofotocolorimétrico, para a determinação das concentrações tissulares do corante. A pressão de ruptura à insuflação foi maior ao 7.° dia de P O. em relação ao 3.°, em todos os grupos. A concentração de Azul de Evans foi maior ao 3.° dia de P.O. em relação ao 7.°, no grupo E2. Comparando-se os grupos, observou-se maior pressão de ruptura à insuflação no grupo controle que nos grupos experimento, ao 3.° dia de P O. Ao 7.° dia entretanto, a pressão de ruptura no grupo E1 foi menor que o grupo controle e que E2. A concentração de Azul de Evans nos grupos controle e E1 foi menor que no E2, ao 3.° dia de P O. Ao 7.° dia em E1 a concentração foi menor que em E2 e que no grupo controle Concluiu-se que, na fase inflamatória, a ação do glicocorticóide foi mais pronunciada do que a do diclofenaco, retardando o início da cicatrização de anastomose colônica em ratos. Abstract: The aim of the present study was to evaluate the role of hidrocortison and diclofenac on the inflamatory phase of the healing process of a colonic anastomosis, through the bursting strength and changes of vascular permeability. Fifty six male Wistar rats were allocated in 3 groups group C ( control ) with 17 animals; group E1 ( Hidrocortison ) with 18 rats; group E2 ( Diclofenac ) with 21 animals. Each of the mentioned groups were divided in 2 subgroups, according to the post-operative time, 3rd or 7th post-operative day. The colonic anastomosis was performed at 5 cm from the anal verge. Through the penis vein, Evans Blue dye was injected in the dosis of 20 mg/Kg on the pre-established post-operative days. The bursting strength of the intestinal segment containing the anastomosis was evaluated, as well as the concentration of the dye in the tissues. In all the groups studied the bursting strength was greater on the 7th P.O. day in relation to the 3rd one The concentration of Evans Blue dye was greater on the 3rd P.O. day in relation to the 7th day in the E2 group. Comparing the groups among themselves, it was observed that the bursting stenght in the control group was greater than the E2 and E1 groups on the 3rd P.O. day. On the 7th P O. day however, the bursting strenght in the E1 group was lower than in the control and E2 groups The concentration of Evans Blue dye in the E1 group was lower than in the E2 group on the 3rd P.O. day. On the 7th day E1 it was lower than in E2 and in the control group. It is conclued that in the inflamatory phase the action of glicorticoid was stronger than that of diclofenac, and consequently delaying of healing was observed in the colonic anastomosis of rats.
