Tradução, validação e confiabilidade do questionário de capacidade funcional Haemophilia Activities List - HAL para pacientes com hemofilia no Brasil

Resumo: O Haemophilia Activities List - HAL é um questionário específico para indivíduos com
hemofilia e avalia a auto-percepção das habilidades funcionais em adultos. Objetivo:
traduzir, validar, testar a confiabilidade e a consistência interna do HAL para a
população brasileira com hemofilia A ou B, grave e moderada; Investigar se existe
diferença quanto a auto-percepção das habilidades funcionais, em adultos com
hemofilia classificados segundo a presença de sangramento articular, e a realização
de cirurgias ortopédicas em membros inferiores. Métodos: Trata-se de um estudo
transversal, que segue critérios de tradução pré-estabelecidos. Uma versão do HAL
em Português-Brasileiro foi aplicada em 52 indivíduos com hemofilia (34,51 ± 12,87
anos). Para a validação do construto foram realizadas correlações dos escores do
HAL com os escores dos questionários HAQ e WOMAC. A confiabilidade foi testada
por meio do teste e re-teste entre avaliadores 1 e 2 em duas entrevistas e a
consistência interna calculada pelo alpha de Cronbach. A diferença no escore do
HAL entre os grupos com e sem sangramento articular e a diferença nos grupos com
cirurgia ou sem cirurgia, foi calculada por meio do teste não-paramétrico de MannWhitney. A significância estatística p = 0,05 foi considerda para todos os testes.
Resultados: Houve correlação significante entre o escore total do HAL e o escore
total do HAQ (r=-0,55, p=0,000) assim como entre HAL total e WOMAC total(r=-0,58,
p<0,000). O Coeficiente de Correlação Interna (CCI) foi excelente intra-examinador
(CCI=0.876) e inter-examinadores (CCI=0.972). O índice de consistência interna do
"HAL total" foi alto (a de Cronbach 0,932). Não houve diferença entre os grupos com
e sem sangramento em relação ao escore total do HAL (p=0,531). O grupo com
cirurgia apresentou melhor desempenho considerando "Funções dos membros
inferiores", "Cuidados pessoais", "Funções dos membros superiores", com diferença
significante no "HAL total" (p=0,044) indicando menor comprometimento funcional
nestes domínios. Conclusão: O HAL-Brasil mostrou-se um instrumento de avaliação
válido e confiável para adultos com hemofilia no Brasil. Abstract: The Haemophilia Activities List - HAL is a specific questionnaire for individuals with
hemophilia and evaluate the self-perception of functional abilities in adults. Objective:
To translate, validate, test reliability and internal consistency of the HAL for the
Brazilian population with hemophilia A or B, severe and moderate; Investigate
whether there is difference in the self-perception of functional abilities in adults with
hemophilia classified according to the presence of bleeding joint, and performing
orthopedic surgery of the lower limbs. Methods: This is a cross-sectional study,
following pre-established criteria for translation. A version of HAL in PortugueseBrazilian was applied in 52 patients with haemophilia A (34.51 ± 12.87 years). For
construct validation was performed correlations of HAL scores with the scores of
HAQ and WOMAC questionnaires. Reliability was tested through the test and re-test
between evaluators 1 and 2 in two interviews and internal consistency calculated by
Cronbach's alpha. The difference in HAL score between the groups with and without
bleeding joint and the difference in the groups with surgery or no surgery was

of the upper limbs", with a significant difference in "total HAL" (p = 0.044), indicating
less functional impairment in these domains. Conclusion: HAL-Brazil proved to be a
valid and reliable assessment tool for adults with hemophilia in Brazil
