Uso crônico de metilfenidato e ritmicidade circadiana de crianças com transtorno de déficit de atenção/hiperatividade

Resumo: O transtorno de déficit de atenção/hiperatividade (TDAH) é o transtorno neuropsiquiátrico mais frequente na infância. Seus principais sintomas são desatenção, hiperatividade e impulsividade. Problemas de sono tais como sonolência diurna, maiores latência e fragmentação de sono, sensação de cansaço ou dificuldade para despertar e pesadelos são mais comuns em crianças com TDAH. No Brasil, o metilfenidato é o tratamento medicamentoso preferencial para o TDAH tanto para crianças, quanto para adultos. Alguns estudos mostraram que esse tratamento é eficaz para o transtorno, porém outros indicaram que seu uso pode estar relacionado ao aumento na ocorrência de insônia em alguns pacientes. O objetivo deste estudo foi comparar os ciclos vigília/sono (CVS) e atividade/repouso de crianças com transtorno de déficit de atenção/hiperatividade a crianças sem o transtorno e avaliar a influência do uso crônico do metilfenidato sobre esses ciclos. Os dados foram coletados por meio do uso de actímetro de punho, durante sete dias consecutivos, diários e questionários de hábitos de sono. Cento e seis crianças, estudantes de escolas públicas de Curitiba, com idades de 6 a 13 anos concluíram o estudo. Os voluntários foram distribuídos em três grupos: CTL (controle: crianças sem qualquer transtorno psiquiátrico; n=36), TDAH (crianças com TDAH não medicadas; n=37) e TDAH+MTF (crianças com TDAH medicadas com metilfenidato; n=33). As variáveis dependentes do estudo foram: horários de dormir, acordar e meia-fase de sono, tempo na cama, duração e eficiência de sono, número e duração de despertares noturnos. Além disso, foram obtidos parâmetros do ritmo atividade/repouso, por meio de variáveis denominadas não-paramétricas, entre elas L5, início de L5, M10, início de M10 e amplitude relativa. Os dados foram analisados por ANOVA de uma via e, quando necessário, o teste de Fisher foi aplicado. Para todas as análises o nível de significância de 5% foi considerado. Crianças com TDAH demonstraram sono mais fragmentado quando comparados aos do grupo controle. O uso crônico do metilfenidato não prejudicou a eficiência de sono, tampouco influenciou os parâmetros do CVS e do ritmo atividade/repouso, visto que os dados dos grupos TDAH e TDAH+MTF foram semelhantes. Abstract: The attention deficit/ hyperactivity disorder (ADHD) is the most common disorder in the childhood. The main symptoms of this disorder are inattention, hyperactivity and impulsivity. Sleep problems such as daytime sleepiness, higher sleep latency and fragmentation, tiredness or difficulty on awakening and nightmares are more common in children with ADHD. In Brazil, methylphenidate is the preferential drug treatment for ADHD for both children as well as adults. Some studies have showed that this treatment is effective for the disorder, but others have indicated that its use can be related to increased risk of insomnia in some patients. The objectives of this study were to compare the sleep-wake cycle and the rest-activity rhythm between children with or without ADHD and to evaluate the influence of chronic use of methylphenidate on these rhythms. Data were collected by means of wrist actigraphs, for seven consecutive days, logs and sleep questionnaires. One hundred and six children from public schools of Curitiba, aged six to thirteen years concluded the study. The volunteers were distributed in three groups: CTL (control: children without any psychiatric disorder; n=36), ADHD (children with ADHD without medication; n=37) and ADHD+MPH (children with ADHD medicated with methylphenidate; n=33). Exclusion criteria were: intellectual disability (IQ<70), female sex and use of medications that alter sleep patterns. The dependent variables of the study were: sleep onset, sleep offset, time in bed, sleep duration, sleep efficiency, number of awakenings and wake after sleep onset. Moreover, some rest-activity parameters were obtained with variables called non-parametric, including L5, M10, L5 onset, M10 onset and relative amplitude. Data were analyzed by one-way ANOVA and, when necessary, Fisher test was applied. For all analyzes the significance level of 5% was considered. Children with ADHD demonstrated higher sleep fragmentation related to control group. The chronic use of methylphenidate did not impair sleep efficiency; neither influenced sleep-wake cycle and rest-activity rhythm parameters, whereas the data of ADHD and ADHD+MPH group’s results were similar.
