Níveis plasmáticos de marcadores de crescimento (IGF-1,IGFBP-3) e leptina em pacientes com fibrose cística : relaçao com avaliaçao antropométrica, composiçao corporal e níveis plasmáticos de interleucina-6

Resumo: Os pacientes com FC apresentam uma dificuldade em manter um estado nutricional adequado. A redução dos níveis de IGF-I, IGFBP-3 e leptina poderia indicar o processo catabólico persistente associado à inflamação na FC. OBJETIVOS: determinar as concentrações plasmáticas de IGF-I, IGFBP-3, leptina e correlacionar com as variáveis antropométricas, a composição corporal e a concentração plasmática de IL-6, em pacientes com FC e em indivíduos saudáveis. PACIENTES E MÉTODOS: 30 pacientes com FC, idade entre 5 a 37 anos e um grupo controle de 42 indivíduos saudáveis com idade entre 4,9 a 40 anos. IGF-I, IGFBP-3 e leptina foram determinados por radioimunoensaio (RIA). A IL-6 foi determinada por método de ELISA. Realizou-se avaliação antropométrica (peso, estatura e IMC) e DXA para avaliação da composição corporal. Obteve-se o inquérito alimentar de 3 dias para cálculo da média de ingestão calórica. RESULTADOS: a média da massa de gordura corporal e quantidade de massa magra foi menor nos pacientes com FC em relação ao grupo controle (p<0,05). As concentrações de IGF-I em ng/mL no grupo FC (105,2 ± 42,16) foi significativamente menor que o encontrado no grupo controle (165,7 ± 70,71; p<0,001), correlacionou-se com a quantidade de massa gorda (r=0,39; p=0,032 ) e a massa magra (r=0,36; p=0,04). Nas crianças até 14 anos, o escore-Z de IGF-I correlacionou-se com o peso (r=0,50; p <0,05), IMC em escore-Z (r=0,45; p<0,05) e com a concentração de leptina (r=0,57; p<0,05). A concentração de IGFBP-3 em ng/mL nos fibrocísticos ( 3289,2 ± 849,9) foi significativamente menor do que no grupo controle (3745,7 ± 1149,5; p < 0,05), porém não apresentou correlação com as variáveis analisadas. O IGFBP-3 expresso em escore-Z apresentou correlação com o peso (r=0,47; p<0,05), o IMC (r=0,38; p< 0,05) e com a concentração plasmática de leptina (r= 0,59; p < 0,05). A concentração de leptina em ng/mL nos pacientes com FC (2,68 ± 2,12) foi significativamente menor do que a encontrado no grupo controle (4,66 ± 3,68 ng/mL; p< 0,001). No grupo FC houve uma correlação positiva entre a concentração de leptina e a massa de gordura corporal (r=0,80; p < 0,001) e a ingestão calórica (r=0,56; p<0,05). A concentração de IL-6 em pg/mL nos pacientes com FC (14,1 ± 9,21) foi significativamente maior que no grupo controle (6,42 ± 1,37; p< 0,001). Não houve correlação significativa de níveis de IL-6 com as variáveis analisadas. CONCLUSÕES: os pacientes com FC apresentam diminuição da massa gorda e muscular comparado com o grupo controle. A composição corporal dos fibrocísticos se correlacionou principalmente com os níveis plasmáticos de leptina e, em menor grau, com os níveis de IGF-I. Os níveis elevados de IL-6 próprios do processo inflamatório na FC, não se relacionou com as variáveis analisadas neste grupo de pacientes estudados. Abstract: Patients with CF hardly maintain an appropriate nutritional status. Reduction of the levels of IGF-I, leptin could indicate the persistent catabolic process associated to the inflammation in CF. OBJECTIVES: to determine plasma concentrations of IGF-I, IGFBP-3, leptin and correlate with anthropometric data, body composition and the plasma concentration of IL-6, in patients with CF and in healthy individuals. PATIENTS AND METHODS: 30 patients with CF, aged 5 to 37 years and a control group of 42 healthy individuals age 4.9 to 40 years. IGF-I, IGFBP-3 and leptin were determined by radioimmunoassay (RIA). IL-6 was determined by ELISA. All individuals were evaluated for anthropometric data (weight, stature and BMI) and DXA for evaluation of body composition. The average caloric ingestion was obtained by 3- day food intake register. RESULTS: the average fat mass and lean mass were lower in patient with CF as compared to healthy controls (p <0.05). IGF-I concentrations in ng/mL were significantly lower in CF group (105.2 ± 42.16) than in controls (165.7 ± 70.71; p<0.001). IGF-I was correlated to the amount of fat mass (r=0.39; p=0.032) and lean mass (r=0.36; p=0.04). In children up to 14 years, the score-Z of IGF-I was correlated with the weight (r=0.5; p <0.05), BMI in score-Z (r=0.45; p <0.05) and with leptin concentration (r=0.57; p <0.05). The concentration of IGFBP-3 (ng/mL) in CF (3289.2 ± 849.9) was significantly lower than in the control group (3745.7 ± 1149.5; p <0.05). However, it did not correlate with the analyzed variables. The IGFBP-3 expressed in score-Z correlated with weight (r=0.47; p <0.05), BMI (r=0.38; p <0.05) and with leptin concentration (r = 0.59; p <0.05). The leptin concentration (ng/mL) in patients with CF (2.68 ± 2.12) was lower than in the control group (4.66 ± 3.68; p <0,001). In the group CF there was a positive correlation between leptin concentration and the fat mass (r=0.80; p <0.001) and caloric ingestion (r=0.56; p <0.05). The concentration of IL-6 (pg/mL) in patients with CF (14.1 ± 9.21) was significantly higher than in controls (6.42 ± 1.37; p <0,001). There were not significant correlation of levels of IL-6 with the analyzed variables. CONCLUSIONS: the patients with CF had decrease the fat and lean mass compared with the control group. The body composition of the CF patients was correlated mainly with the leptin concentration and, in lower level, with the IGF-I concentration. Increased II-6 plasma levels in CF, a characteristic of the inflammatory process, were not related to other variables in this group of patients.
