dc.description.abstract | Resumo: Com a finalidade de prolongar e melhorar a qualidade de vida dos pacientes submetidos à revascularização do miocárdio, tem-se intensificado o uso de enxertos arteriais, sabendo-se que estes têm maior perviabilidade a longo prazo. O presente trabalho analisa a revascularização do miocárdio com a utilização de ambas as artérias torácicas internas (ATIs), em 60 pacientes, operados de março de 1987 a maio de 1996. Cinqüenta (83,3%) eram do sexo masculino e 10 (16,7%) do feminino. A idade variou de 36 a 76 anos, com média de 55,6 ± 9,3 anos. Trinta e cinco (58,3%) apresentavam angina instável, 22 (36,7%) angina estável e 3 (5%) encontravam-se assintomáticos. A hipertensão arterial estava presente em 27 (45%), diabete melito em 11 (18,3%), obesidade em 26 (43,3%), tabagismo em 48 (80%), apresentavam história familiar de doença arterial 30 (50%) e eram sedentários 31 (51,7%). Lesão de um vaso coronariano foi encontrada em 7 (11,6%) casos, de dois vasos em 16 (26,7%), de três vasos em 34 (56,7%) e da artéria coronária esquerda em 3 (5%) dos casos. Os tempos medianos de pinçamento aórtico, de circulação extracorpórea e médio de operação foram de 43,5; 79; 248 ± 65,9 minutos, respectivamente. Além das duas ATIs foram empregadas nas operações a veia safena magna, a artéria radial e a artéria gastroepiplóica direita. Foram realizados 2,9 enxertos/paciente. Ocorreram 3 óbitos (5%). O primeiro por Síndrome de baixo débito cardíaco (SBDC), outro por infecção esternal e o terceiro devido à defeito no ventilador durante o pós-operatório. Dez (16,7%) pacientes apresentaram complicações respiratórias, sendo 5 (50%) pneumonia, 2 (20%) derrame pleural, 2 (20%) pneumonia com derrame pleural e 1 (10%) broncoespasmo grave. O sexo masculino (p<0,001), idade avançada (p<0,01), angina instável (p<0,005), tabagismo (p<0,001) e utilização de solução cardioplégica cristalóide (p<0,005) foram variáveis estatisticamente significativas para complicação respiratória. Onze pacientes (18,3%) apresentaram SBDC. O sexo feminino (p<0,025), angina instável (p<0,005), o sedentarismo (p<0,005) e o tabagismo (p<0,001), foram as variáveis estatisticamente significativas para o seu aparecimento. A incidência de reoperação por sangramento foi de 1,7%, igual valor para infarto do miocárdio peroperatório e para infecção de esterno. Concluiu-se que o uso bilateral da ATI, na revascularização do miocárdio, pode ser realizado com baixa mortalidade, as complicações respiratórias são mais freqüentes nos tabagistas, idosos e com angina instável, os tempos de CEC, operação e de uso de ventilador, não influenciaram no aparecimento desta complicação, a SBDC foi mais freqüente nos pacientes do sexo feminino, nos pacientes com angina instável, nos sedentários e tabagistas e a infecção do esterno, a incidência de IAM perop e reoperação por sangramento, foi pouco freqüente. | pt_BR |
dc.description.abstract | Abstract: With the objective of prolonging life and improving the quality of life of patients undergoing myocardial revascularization, arterial grafts have been increasingly used, for it is known these grafts have better long-term patency. The present study analyzes the use of both internal thoracic arteries (ITA) in 60 patients, operated upon from March 1987 to May 1996. 50 patients (83.3%) were male and 10 (16.7%) were female. Ages ranged from 36 to 76 years, with a mean age of 55.6 ± 9,3 years. 35 patients (58.3%) presented with unstable angina, 22 patients (36.7%) with stable angina and three patients (5%) were asymptomatic. 27 patients (45%) had arterial hypertension; 11 patients (18.3%) had diabetes mellitus; 26 patients (43.3%) were obese; 48 patients (80%) were cigarette smokers; 30 patients (50%) had a familiar history of occlusive arterial disease; and 31 (51.7 %) patients were sedentary. Occlusive lesions of one coronary artery were found in 7 patients (11.6%); of two coronary arteries in 16 patients (26.7%); of three coronary arteries in 34 patients (56.7%) and of the left main coronary artery in three patients (3%). The mean time of aortic clamping, time of extracorporeal circulation and total operative time were 43.5 minutes ; 79 minutes; and 248 ± 65.9 minutes, respectively. Besides the two internal thoracic arteries, the saphenous vein, the radial artery and right gastroepiploic artery were used in the operations. An average of 2.9 grafts was performed per patient. There were three operative deaths (5%) : one due to low cardiac output syndrome; one due to sternal infection and one due to a failure of the mechanical ventilator in the postoperative period. Respiratory complications were observed in 10 patients (16.7%) : five (50%) had pneumonia; 2 (20%) had pneumonia with pleural effusion ; and one (10%) had severe bronchospasm . Male sex (p<0.005); old age (p<0.01); unstable angina (p<0.005); cigarette smoking (p<0.001) and use of cristaloid cardioplegic solution (p<0.005) were statistically significant variables associated with respiratory complications. 11 patients (18.3%) developed low cardiac output syndrome (LCOS). Female sex (p<0.025); unstable angina (p<0.005) sedentarism (p<0.005) and cigarette smoking (p<0.001) were the statistically significant variables for the development of LCOS. The rates of reoperation for bleeding; perioperative myocardial infarction and sternal infection were all 1,7%. It is concluded that: the use of both ITA in myocardial revascularization can be performed with a low mortality rate ; respiratory complications were more frequent in the cigarette smokers, the elderly and those with unstable angina ; the time of extracorporeal circulation, total operative time and mechanical ventilation time were not associated with higher rates of respiratory complications ; LCOS was more statistically more frequent in women, in patients with unstable angina, in patients who smoked and in patients with a history of sedentarism; and sternal infection was ân infrequent complication. | pt_BR |
