Prevalencia dos sintomas atribuídos ao climatério, em relaçao à idade e/ou à menopausa

Resumo: Analisa a correlação entre, de um lado, a idade e a menopausa e, de outro, a prevalência de queixas atribuídas ao climatério, em mulheres da região Oeste do Paraná, atendidas em hospital conveniado com o SUS e em clínica particular na cidade de Cascavel, de junho 1998 a maio de 1999. O estudo comparativo é de corte transversal, com 367 mulheres divididas em três grupos: o primeiro, grupo com 167 mulheres entre 40 e 49 anos com eumenorréia (em pré-menopausa) o segundo, com 65 mulheres em pós-menopausa, na mesma faixa etária; e um terceiro grupo de 135 mulheres em pós-menopausa e de 50 a 59 anos. Todas consentiram voluntariamente em participar da pesquisa. As informações sobre sintomas e sua caracterização (ou não) como problemas para as pacientes, foram adaptados a partir de questionário proposto por Sarrel (1993); versa sobre 16 queixas, que foram classificadas em cardiovasculares, geniturinárias, neurológicas, psicológicas e sexuais. As mulheres em pós-menopausa e de 40 a 49 anos foram comparadas com mulheres da mesma idade em pré-menopausa; e com mulheres entre 50 a 59 anos, e em pós-menopausa. Para análise do resultado foi utilizado o teste do quiquadrado, com valor de p inferior a 10% (p<0,10). Os resultados do presente estudo revelam que a grande queixa associada ao início da menopausa são as ondas de calor/fogacho junto com a sensação de secura vaginal. Enquanto os fogachos diminuem e desaparecem mais tarde, a queixa geniturinária (secura vaginal) permanece. Outras queixas cardiovasculares (fôlego curto/falta de ar; dor ou pressão no peito; palpitação; fraqueza ou fadiga) atribuídas a menopausa não aparecem associadas ao momento do início desta fase de vida ou da idade da mulher, porém a associação surge quando avaliada em relação ao tempo do início da menopausa. As queixas psíquicas-neurológicas (perda de memória; amortecimento/dormência; dor de cabeça; medo de estar sozinha; depressão, ansiedade, distúrbios do sono) não tiveram relação nem com a idade nem com a menopausa, porém a proporção de mulheres que consideraram essas queixas como problemas, foi maior quanto maior a idade e o tempo de menopausa. As queixas sexuais (perda do desejo sexual; dor na relação sexual) tiveram maior prevalência associadas a menopausa e idade principalmente quando consideradas problemas, assim como ao maior tempo após-menopausa. Abstract: This study analyzes the relation between, on one side, the age and the menopause, and on the other, the prevalence of the complaints attributed to the climatery in women from the West of Paraná State seen at Agreed Hospitals of SUS (Sistema Único de Saúde, the public health system), and at private clinics in Cascavel City, from June 1998 to 1999. This comparative study is a transversal cut with 367 women divided in three groups: the first one with 167 women between 40 and 49 years old with regular menstruation (in pre-menopause); the second one with 65 women at the same age group in post-menopause; and the third and last group with 135 women between 50 and 59 years old in post-menopause. All of them volunteered to participate in the research. The information about the symptoms and their characterization (or not) as problems to the patients was adopted from the questionnaire suggested by Sarrel (1993) which reports 16 complaints classified as Cardiac, Genitourinary, Neurological, Psychological and Sexual. The women between 40 and 49 years old in post-menopause were compared with women from the same age group in pre-menopause and with women between 50 to 59 years old and in post-menopause. To analyze the data from the group with regular menstruation and at post-menopause at different age groups was used the Quisquare test with p inferior value to 10% (p<0,10), and a logistic regression model for the Cardiac, Genitourinary, Neurological, Psychological and Sexual groups. According to the analyzed data, the big complaints associated with the beginning of the menopause are the heat waves/hot flushes and the feeling of dried vagina. While the hot flushes later decrease and disappear, the genitourinary complaints (vaginal dry) remain. Other cardiac complaints attributed to the menopause - such as breathlessness, chest pain, palpitation, weakness - do not appear to be associated with the starting point of this stage of a woman's life or with her age. However, the association comes when it is evaluated in relation to the beginning of the menopause. The psychic-neurological complaints (such as lack of memory, numbness, headache, fear of being alone, depression, anxiety, sleep disturbances) did not have relation neither with the age nor with the menopause. However, the percentage of women which considered the above complaints problems was bigger the older the woman was and the longer the period of menopause. The sexual complaints (such as lack of sexual lust, pain during intercourse) were more associated with the menopause and age, especially when they were considered problems, and there was a longer post-menopause period.
