Sinais neurológicos como preditores do desenvolvimento cognitivo de crianças nascidas pré-termo

Resumo: A evolução técnico-profissional nas Unidades de Terapia Intensiva Neonatal e o crescente investimento em programas do governo voltados ao cuidado materno e infantil possibilitaram a redução dos índices de mortalidade de crianças nascidas pré-termo. Entretanto, o aumento nas taxas de sobrevivência não foi acompanhado por uma redução na incidência de morbidades relacionadas a este nascimento prematuro. É freqüente a descrição de prejuízos a curto e longo prazo, referentes à vida acadêmica, social e emocional dos pré-termos. Neste sentido, tornam-se cada vez mais importantes pesquisas que objetivem a identificação e/ou o refinamento de fatores preditivos de alterações no neurodesenvolvimento. O objetivo deste estudo foi avaliar o valor da Avaliação Neurológica Amiel-Tison à Termo na predição do desenvolvimento cognitivo de crianças nascidas pré-termo, entre 3 e 24 meses de idade corrigida. Além disso, procurou identificar quais os fatores de risco para alterações no desenvolvimento cognitivo, verificar se o valor preditivo das variáveis estudadas foi moderado pela idade e comparar os resultados da avaliação cognitiva quando considerada a idade cronológica e corrigida das crianças nascidas pré-termo. Para avaliação do desenvolvimento cognitivo foi utilizada a Escala Cognitiva das Escalas Bayley de Desenvolvimento Infantil, Terceira Edição. Foi desenvolvido um estudo observacional prospectivo, com crianças nascidas pré-termo atendidas no Centro de Neuropediatria do Hospital de Clínicas da UFPR. No Grupo 1, composto por crianças com idade corrigida entre 3 e 12 meses, foi encontrada associação entre o desempenho na Escala Cognitiva e as variáveis grau de escolaridade materna e classe socioeconômica. No Grupo 2, composto por crianças com idade corrigida entre 13 e 24 meses, verificou-se associação entre o resultado da avaliação cognitiva e os fatores alteração do palato e PCA. Foi confirmada a hipótese de que o valor preditivo das variáveis estudadas é moderado pela idade. Além disso, foi encontrada diferença significativa entre os escores quando considerada a idade cronológica e quando considerada a corrigida das crianças avaliadas. Em ambos os grupos, a maioria das crianças obtiveram classificação normal na avaliação neurológica e escores na Escala Cognitiva dentro da média. Apesar de considerar o acompanhamento ambulatorial um mecanismo de proteção, sugere-se a superestimação dos escores na Escala Cognitiva. Os resultados deste estudo contrapõem-se, em parte, aos dados encontrados na literatura, por não ter encontrado associação entre a Avaliação Neurológica Amiel-Tison e o outcome cognitivo. Por outro lado, segue a tendência de estudos atuais que sugerem falhas na Terceira Edição das Escalas Bayley no que se refere à identificação de atrasos no desenvolvimento em crianças de risco. Os dados encontrados nesta pesquisa colaboram para o delineamento de prognósticos mais precisos, ampliando a visão sobre a categoria de crianças com riscos para prejuízos no desenvolvimento cognitivo. Ao mesmo tempo, partindo do conceito de Atenção Precoce, contribui para o planejamento interdisciplinar de estratégias de prevenção nos âmbitos primário, secundário e terciário, incluindo os diferentes microsistemas família, saúde e educação com vistas a oferecer melhor qualidade de vida para as crianças nascidas pré-termo. Abstract: The technical and professional improvement of the Neonatal Intensive Care Units and the growing investment in government programs aimed at mother and child care have allowed for the lowering of the mortality rates of children born preterm. However, the higher survival rates were not accompanied by a lowering in the morbidity related to these preterm births. Impairments at a short and a long term in the academic, social and emotional life of preterm children are frequently reported. In this respect, researches aiming at the identification and/or refinement of the predictive factors of alteration in the neurodevelopment are getting more and more important. The present study aims to assess the predictive value of The Amiel-Tison Neurological Assessment at Term in the prediction of cognitive development in preterm children between 3 and 24 months at corrected age. Moreover, the study sought to identify the risk factors for alterations of the cognitive development, to verify if the predictive value of the variables that were studied was moderated by age and to compare the results of the cognitive evaluation when the chronological age and the corrected age of preterm children were considered. The Bayley Scales of Infant and Toddler Development, Third Edition was utilized in order to evaluate the cognitive development. A prospective observational study was carried out with preterm children that were looked after at the Neuropediatrics Center of the Federal University of Parana.s Hospital. In Group 1, comprised of children with corrected age ranging from 3 to 12 months, a relationship between the performance in the Cognitive Scale and the variables mother.s level of education and socioeconomic class was found. In Group 2, comprised of children with corrected age ranging from 13 to 24 months, a relationship between the outcome of the cognitive evaluation and the factors altered palate and PDA was observed. The hypothesis that the predictive value of the variables at issue is moderated by age was confirmed. Furthermore, a significant difference was found between the scores when the chronological age and the corrected age were considered. In both groups, most children achieved normal classification at the neurological assessment and average scores at the Cognitive Scale. Even though we consider the follow-up at the hospital a protection mechanism, it is advised to overestimate the scores of the Cognitive Scale. The results of this study partially contrast the data found in the literature due to the lack of relationship between the Amiel-Tison Neurological Assessment and the cognitive outcome. On the other hand, it is aligned with the current studies which suggest flaws at the Third Edition of the Bayley Scales in what concerns the detection of development delays in the children at risk. The data found in this research contribute for the outlining of more accurate prognosis, broadening the view over the category of children under risk of impairments in their cognitive development. At the same time, having the concept of Early Attention as a starting point, this study contributes for an interdisciplinary planning of strategies for prevention at the primary, secondary and tertiary health care levels, including the microsystems family, health and education, aiming to offer the best quality of life for children born preterm.
