Pragmática clínica e Alzheimer : investigando o impacto da afasia de fala na comunicação dos idosos

Abstract

Resumo: Este trabalho investiga aspectos da fala em idosos com Demência de Alzheimer, com foco específico no funcionamento pragmático da linguagem. O estudo tem como objetivos analisar as mudanças nas habilidades pragmáticas em indivíduos com Alzheimer em diferentes estágios da doença, investigar a relação entre a progressão da Doença de Alzheimer e a compreensão das normas pragmáticas em situações conversacionais, bem como explorar as dificuldades pragmáticas enfrentadas por cuidadores durante as interações comunicativas com pessoas que manifestam a doença. A pesquisa fundamenta-se em uma abordagem linguística, articulando a Teoria da Relevância, proposta por Sperber e Wilson, e os pressupostos da Pragmática Clínica, conforme desenvolvido por Louise Cummings, que constituem o eixo teórico central do trabalho. Inicialmente, realizou-se um levantamento bibliográfico criterioso sobre estudos relacionados à linguagem e à Demência de Alzheimer, com objetivo de apresentar a doença, delimitar o estado da arte da temática e sustentar a análise empírica. Na etapa metodológica, procedeu-se com a seleção de vídeos disponíveis na plataforma Instagram, nos quais idosos diagnosticados com essa doença interagem com seus cuidadores em contextos cotidianos. Foram selecionados quatro idosos, sendo um vídeo de cada. Com base nos vídeos, focalizando tanto o contexto situacional quanto os processos dialogais observados, as interações foram descritas e organizadas por meio de quadros analíticos. A análise dos diálogos foi conduzida a partir das teorias basilares e complementadas pela aplicação do Protocolo Pragmático de Putting e Kirchner, utilizado como instrumento metodológico para descrição dos fenômenos interacionais, sem finalidade diagnóstica clínica. Os resultados evidenciam a ocorrência de diversos rompimentos no funcionamento pragmático da linguagem, manifestados por dificuldades na manutenção de tópicos discursivos, no reconhecimento e cumprimento de diretivas, na realização de inferências adequadas e na integração entre linguagem, ação e contexto. Observa-se ainda descontinuidades na gestão dos turnos de fala e na produção de respostas contingentes às expectativas comunicativas dos interlocutores. Tais rompimentos impactam diretamente na eficácia da interação, exigindo ajustes constantes por parte dos cuidadores para a sustentação do diálogo. Conclui-se que a análise das interações permite compreender o funcionamento da linguagem em idoso com Demência de Alzheimer para além de uma perspectiva clínica, evidenciando a relevância dos processos interacionais e inferências na comunicação cotidiana. O estudo contribui, assim, para o aprofundamento das descrições linguísticas da fala no contexto do Alzheimer e para reflexões sobre práticas comunicativas mais sensíveis às limitações pragmáticas desses sujeitos

Abstract: This study investigates aspects of speech in elderly people with Alzheimer's Dementia, with a specific focus on pragmatic language functioning.The study aims to analyze changes in pragmatic skills in individuals with Alzheimer's at different stages of the disease, to investigate the relationship between the progression of Alzheimer's Disease and the understanding of pragmatic norms in conversational situations, as well as to explore the pragmatic difficulties faced by caregivers during communicative interactions with people who manifest the disease. The research is based on a linguistic approach, articulating the Theory of Relevance, proposed by Sperber and Wilson and the assumptions of Clinical Pragmatics, as developed by Louise Cummings which constitute the central theoretical axis of the work. Initially, a careful bibliographic survey was carried out on studies related to language and Alzheimer's Dementia with the objective of presenting the disease, delimit the state of the art of the theme and sustain the empirical analysis. In the methodological stage, the selection of videos available on the Instagram platform was carried out in which elderly people diagnosed with this disease interact with their caregivers in everyday contexts. Four elderly people were selected, one video of each. Based on the videos, focusing on both the situational context and the dialogical processes observed, the interactions were described and organized through analytical tables. The analysis of the dialogues was conducted from the basic theories and complemented by the application of the Pragmatic Protocol of Putting e Kirchner used as a methodological instrument to describe interactional phenomena, without clinical diagnostic purpose. The results show the occurrence of several ruptures in the pragmatic functioning of language, manifested by difficulties in maintaining discursive topics, in recognizing and following instructions, in making appropriate inferences, and in integrating language, action, and context. Discontinuities are also observed in the management of speaking turns and in the production of responses contingent on the communicative expectations of the interlocutors. Such disruptions directly impact the effectiveness of the interaction, requiring constant adjustments from caregivers to sustain the dialogue. In conclusion, the analysis of interactions allows us to understand the functioning of language in elderly individuals with Alzheimer's dementia beyond a purely clinical perspective, highlighting the relevance of interactional processes and inferences in everyday communication. This study thus contributes to a deeper understanding of linguistic descriptions of speech in the context of Alzheimer's disease and to reflections on communicative practices that are more sensitive to the pragmatic limitations of these individuals
