Remoção do material obturador com diferentes sistemas durante o retratamento endodôntico : revisão de literatura

Resumo : O tratamento endodôntico tem como objetivo manter ou recuperar a saúde dos tecidos periapicais, porém quando não alcançando, o retratamento endodôntico deve ser considerado como a primeira opção, devido a sua característica menos invasiva. O retratamento do canal radicular visa remover o material obturador prévio para realização de um novo preparo químico-mecânico e obturação, que sejam efetivos e possam proporcionar a cura da doença periapical persistente ou emergente após o tratamento primário. Dentro da especialidade existem diversos instrumentos e técnicas para realização deste procedimento, porém ainda não há um consenso na literatura do protocolo mais eficaz. Assim, o presente estudo teve como objetivo realizar uma revisão de literatura a respeito da eficácia da remoção de material obturador do sistema de canais radiculares por meio de diversas técnicas e dispositivos, incluindo instrumentos manuais e mecanizados, sendo estes com cinemática rotatória, reciprocante e adaptativa, com diferentes ligas metálicas e ainda, métodos complementares que aprimoram a limpeza, a partir do uso de insertos sônicos, ultrassônicos e outros instrumentos com diferentes propostas. Conclui-se que os estudos mais recentes analisados apontam que sistemas reciprocantes são eficientes na remoção do material obturador e que a utilização de instrumentos em técnicas complementares potencializam a limpeza. Mas, independentemente do instrumento, cinemática, liga metálica ou métodos complementares, constatou-se que até o presente momento não é possível remover todo o material obturador do sistema de canais radiculares durante o retratamento endodôntico. Abstract: The endodontic treatment aims to maintain or restore the health of the periapical tissues, however, when not achieving, endodontic retreatment should be considered as the first option, due to its less invasive characteristic. The retreatment of the root canal is seen to remove the previous filling material to perform a new chemical-mechanical preparation and filling, which are effective and provide a cure for persistent or emerging periapical disease after primary treatment. Within the specialty, there are several instruments and techniques for performing this procedure, but there is still no consensus in the literature on the most effective protocol. Thus, the present study aimed to carry out a literature review regarding the removal of filling material from the root canal system through various techniques and devices, including manual and mechanized, which are with rotary, reciprocating and adaptive kinematics, with different metallic alloys and also complementary methods that improve cleaning the use of sonic and ultrasonic inserts and other instruments with different proposals. It is concluded that the most efficient studies indicate that reciprocating systems are efficient in removing the material, obturator and that the use of complementary technical means enhance cleaning. However, regardless of the instrument, kinematics, metallic alloy or complementary methods, it was found that so far it is not possible to remove all the filling material from the root canal system during endodontic retreatment.
