Avaliação da profundidade anestésica e de parâmetros fisiológicos durante infusão sequencial alvo controlada de propofol em cães

Resumo: Avaliar variação no índice bispectral (BIS), plano anestésico e parâmetros fisiológicos em cães submetidos a infusão sequencial alvo controlada de propofol. Estudo experimental prospectivo. Vinte cães machos, de 1-3 anos de idade pesando 9,8-44 kg. Os cães foram pré-medicados por via intramuscular com metadona (0,2 mg/kg ) e acepromazina (0,03 mg/kg) e após 30 minutos, indução anestésica com propofol e a manutenção foi iniciada usando uma técnica de infusão controlada por alvo sequencial (STCI). A concentração plasmática alvo de propofol foi fixada em 5 µg/ml para indução anestésica e para manutenção de 120 minutos. O software TivaTrainer 9.1 foi usado para calcular taxas de infusão de propofol no modo de infusão controlada pelo alvo (TCI) utilizando variáveis farmacocinéticas espécies específicas. O valor do BIS, pressão arterial sistólica e frequência cardíaca foram registrados a cada 5 minutos, a partir do início da indução até o final da anestesia. Hematócrito, proteína plasmática total e glicemia foram mensurados antes, durante e após a anestesia. A análise de variância foi realizada em dados numéricos usando o teste de Friedman seguido do ajuste de Bonferroni (p <0,05). Ocorreu diferença estatística entre o valor basal do BIS (T0), com uma mediana de 84,5 (81-97), com o BIS em todos os momentos após T15. A profundidade anestésica cirúrgica foi atingida em 18 dos 20 cães após 10 minutos de infusão e em todos os cães após 20 minutos, com um valor BIS mediano de 72 (53-89) durante a anestesia. A intubação orotraqueal ocorreu entre 1,9 e 19,3 minutos. Os parâmetros fisiológicos mensurados durante STCI apresentaram valor mediano, máximo e mínimo respectivamente de glicemia (96mg/dl, 70mg/dl, 120mg/dl), hematócrito (35%, 25%, 48%); proteína plasmática total (5,8g/dl; 4g/dl, 7,6g/dl), frequência cardíaca (85bpm, 48bpm, 169bpm) e pressão arterial sistólica (110mmhg, 64mmhg, 198mmhg). A profundidade máxima da anestesia foi alcançada após 20 minutos de anestesia e então permaneceu estável durante todo o período anestésico. Não ocorreram variações estatisticamente significativas (p<0,05) durante a anestesia dos parâmetros fisiológicos. Entretanto, quando comparados com os valores basais anteriores a medicação pré-anestésica de hematócrito, proteína plasmática total e frequência cardíaca apresentaram diferença estatística (p <0,05). Abstract: To evaluate the change in bispectral index (BIS) and clinical anesthetic depth in dogs undergoing propofol sequential target-controlled infusion and physiological parameters in dogs submitted to sequential target-controlled infusion of propofol. Prospective experimental study.Twenty male dogs, from 1-3 years old and weighing 9.8 - 44 kg. Dogs were premedicated intramuscularly with methadone (0.2 mg/kg) and acepromazine (0.03 mg/ kg) and after 30 minutes, propofol anesthetic induction and maintenance was initiated using a sequential target-controlled infusion technique (STCI). Target plasma concentration of propofol was set at 5 µg/ml for both anesthetic induction and for 120 minutes maintenance. TivaTrainer 9.1 software was used to calculate anesthetic infusion rates in target-controlled infusion (TCI) mode using species-specific pharmacokinetic variables. The BIS value was recorded every 5 minutes, from the beginning of induction until the end of anesthesia. Hematocrit, total plasma protein and blood glucose were measured before, during and after anesthesia. Analysis of variance was performed on numerical data using the Friedman test followed by the Bonferroni adjustment (p < 0.05). There was a statistical difference between the baseline BIS value (T0), with a median of 84.5 (81-97), and BIS at all times after T15. Surgical anesthetic depth was reached in 18 of 20 dogs after 10 minutes of infusion and in all dogs after 20 minutes, with a median BIS value of 72 (53-89) at time of surgical anesthesia. Orotracheal intubation occurred between 1.9 and 19.3 minutes. The physiological parameters measured during STCI showed median, maximum and minimum values, respectively, of blood glucose (96mg/dl, 70mg/dl, 120mg/dl), hematocrit (37%, 25%, 48%); total plasma protein (5,9g/dl; 4,1g/dl; 7,6g/dl), heart rate (85bpm, 48bpm, 169bpm) and systolic blood pressure (110mmhg, 64mmhg, 198mmhg). The maximal depth of anesthesia was reached after 20 minutes of anesthesia and then remained stable throughout the anesthetic period. There were no statistically significant variations (p<0.05) during anesthesia of the physiological parameters. However, when compared to baseline values prior to pre-anesthetic medication, hematocrit, total plasma protein and heart rate were statistically different (p < 0.05).
