Fatores associados à percepção da qualidade de vida em pacientes que serão submetidos à cirurgia ortognática

Resumo: Objetivos: Avaliar fatores associados à percepção da Qualidade de Vida (QV) em pacientes que serão submetidos à cirurgia ortognática. Desenho do estudo: Caracteriza-se como estudo transversal observacional com 91 participantes, entre 18 e 64 anos de idade, realizado em duas Universidades de Curitiba, no período de 18 meses. A percepção da QV dos pacientes foi avaliada através do Orthognathic Quality of Life Questionaire (OQLQ). Também foram catalogados dados individuais dos participantes como idade e sexo. O perfil facial foi classificado em Perfil I, Perfil II e Perfil III e a assimetria facial considerada presente quando o paciente apresentava desvio igual ou superior a 4mm da linha média da mandíbula em relação a linha média da maxila. Foram coletadas células da mucosa bucal para análise de DNA . Os marcadores rs3800373 e rs1360780 para o FKBP Prolyl Isomerase 5 (FKBP5) foram genotipados. Os dados foram submetidos à análise estatística, com nível de significância de 5%. A variável idade foi dicotomizada de acordo com a mediana e os testes de Kruskal-Wallis e U de Mann-Whitney foram utilizados para comparação entre os grupos, também foi utilizado o coeficiente de correlação de Espearman. Para calcular o equilíbrio de Hardy-Weinberg foi utilizado o teste Qui-Quadrado. Resultados: Observou-se que as mulheres apresentaram impacto negativo na percepção da QV geral quando comparadas aos homens (p = 0,019). Além disso, as mulheres apresentaram impacto negativo na percepção da QV nos domínios "Função Oral" (p = 0,032) e "Consciência da deformidade" (p = 0,009). No modelo dominante (CC/CT), os indivíduos com ao menos um alelo C para o marcador rs1360780 apresentaram um impacto negativo na QV no domínio "Estética facial" (p = 0,037). Conclusão: Mulheres apresentam um impacto negativo na QV em relação aos homens. Indivíduos com genótipo CC/CT também apresentaram um impacto negativo na percepção da QV quando comparados a indivíduos com genótipo TT. Abstract: Objectives: To evaluate factors associated with the Quality of Life (QoL) perception in patients who will undergo orthognathic surgery. Study design: It's characterized as a cross-sectional observational study with 91 participants, between 18 and 64 years of age, carried out at two universities in Curitiba, over a period of 18 months. The QoL perception of patients was assessed using the Orthognathic Quality of Life Questionaire (OQLQ). Individual participant data such as age and sex were also cataloged. The facial profile was classified into Profile I, Profile II and Profile III, and facial asymmetry was considered present when the patient had a deviation equal to or greater than 4mm from the mandible midline in relation to the maxilla midline. DNA was collected from cells of the oral mucosa. Markers rs3800373 and rs1360780 for FKBP Prolyl Isomerase 5 (FKBP5) were genotyped. Data were subjected to statistical analysis, with a significance level of 5%. The variable age was dichotomized according to the median and the Kruskal-Wallis and Mann-Whitney U tests were used to compare the groups, the Espearman correlation coefficient was also used. To calculate the Hardy-Weinberg equilibrium, the Chi-Square test was used. Results: It was observed that women had a negative impact on the general QoL perception when compared to men (p = 0.019). In addition, women had a negative impact on the QoL perception in the domains "Oral Function" (p = 0.032) and "Awareness of the deformity" (p = 0.009). In the dominant model (CC/CT), individuals with at least one C allele for the rs1360780 marker had a negative impact on QoL in the "Facial aesthetics" domain (p = 0.037). Conclusion: Women have a negative impact on QoL compared to men. Individuals with the CC/CT genotype also had a negative impact on the QoL perception when compared to individuals with the TT genotype.
