dc.description.abstractResumo: A segmentação da maxila associada a osteotomia de Le Fort I é uma técnica cirúrgica utilizada para correção de discrepâncias transversais de maxila. Ela possibilita corrigir a assimetria intra-arco, podendo manipular os segmentos ósseos separadamente, em um único ato cirúrgico. Mesmo com os avanços ao longo dos anos, ainda existem complicações decorrentes da intervenção cirúrgica, como a necrose óssea, necrose pulpar e defeitos periodontais. Dentre os principais problemas relacionados as segmentações maxilares estão a manutenção do aporte sanguíneo e a tensão excessiva da fibromucosa palatina após o procedimento cirúrgico. O método de elementos finitos (MEF) é uma ferramenta apropriada para o uso na Cirurgia e Traumatologia Buco-Maxilo-Faciais, pois é capaz de determinar a medida do estresse, deformação e deslocamento no complexo ósseo e fibromucosa, após diversas maneiras de aplicação de carga. Sendo assim o objetivo desse estudo foi analisar o efeito das forças aplicadas no ato operatório sobre o deslocamento ósseo e a tensão na fibromucosa em diferentes desenhos de segmentação de maxila, através da análise de elementos finitos. Um crânio tridimensional foi gerado a partir de imagens tomográficas e realizado simulações cirúrgicas. Foram trabalhados com 04 modelos, sendo dois deles dividindo a maxila em três segmentos (M1 e M2) e dois deles dividindo a maxila em quatro segmentos (M3 e M4). A diferença entre M1 e M2 e entre o M3 e M4 está na localização das osteotomias interdentárias. Nos modelos M1 e M3, as osteotomias interdentárias foram realizadas entre os incisivos laterais e os caninos superiores. Já no M2 e M4 as osteotomias foram realizadas entre os caninos e primeiros pré-molares superiores. Em todos os modelos, foi aplicada uma força de 100 newtons (N) na face palatina dos dentes 16 e 26 simulando o afastamento digital e o suporte foi inserido na face lateral do osso zigomático direito e esquerdo. A partir disso foram mensurados o deslocamento e a tensão dos segmentos. Foi possível observar que, comparando a técnica entre 3 e 4 segmentos com a mesma osteotomia interdental, isto é, M1 com M3 e M2 com M4, houve um maior deslocamento ósseo e menor tensão na fibromucosa na técnica com 3 segmentos. Quando comparamos a mesma quantidade de segmentos com regiões distintas de osteotomia interdental, ou seja, M1 com M2 e M3 com M4 foi possível verificar que houve uma menor tensão na fibromucosa e um maior deslocamento ósseo nos cortes entre incisivo lateral e canino, comparados a osteotomia entre canino e primeiro pré-molar. Portanto a técnica M1 mostrou-se com menor resistência possibilitando maior deslocamento e consequentemente gerando menos tensão na fibromucosa. Palavras-chave: Osteotomia segmentar. dc.description.abstractAbstract: The segmentation of the maxilla associated with Le Fort I osteotomy is a surgical technique used to correct transverse maxillary discrepancies in adults. It allows correcting intra-arch asymmetry, and can manipulate the bone segments separately, in a single surgical act. Even with advances over the years, there are still complications resulting from surgical intervention, such as bone necrosis, pulpal necrosis and periodontal defects. Among the main problems related to maxillary segmentations are the maintenance of the blood supply and the excessive tension of the fibrinous palatine after the surgical procedure. The finite element method (MEF) is an appropriate tool for use in Buco- Maxillofacial Surgery and Traumatology, since it is able to determine the extent of stress, deformation and displacement in the bone and fibromucosa complex, after several ways of applying charge. Thus, the objective of this study was to analyze the effect of forces applied in the operative procedure on bone displacement and stress in the fibromucosa in different designs of maxilla segmentation, through the analysis of finite elements. A three-dimensional skull was generated from tomographic images of a skull and performed surgical simulations. Four models were studied, two of them dividing the maxilla into three segments (M1 and M2) and two of them dividing the maxilla into four segments (M3 and M4). The difference between M1 and M2 and between M3 and M4 lies in the location of interdental osteotomies. In the M1 and M3 models, the interdental osteotomies were performed between the lateral incisors and the upper canines. In M2 and M4, the osteotomies were performed between the canines and the first maxillary premolars. In all models, a force of 100 newtons (N) was applied to the palatal face of the teeth 16 and 26 simulating the digital spacing and the support was inserted in the lateral aspect of the zygomatic bone, referring to a static face. From this, the displacement and tension of the segments were measured. It was possible to observe that, comparing the technique between 3 and 4 segments with the same interdental osteotomy, that is, M1 with M3 and M2 with M4, there was a greater bone displacement and lower tension in the fibromucosa in the technique with 3 segments. When comparing the same number of segments with different regions of interdental osteotomy, that is, M1 with M2 and M3 with M4, it was possible to verify that there was a lower tension in the fibromucosa and a greater bone displacement in the cuts between lateral incisor and canine, compared to osteotomy between canine and first premolar. Therefore, the 3-segment segmental technique with interdental osteotomy being performed between lateral and canine incisors (M1) was shown to have less resistance, allowing greater displacement and consequently generating less tension in the fibromucosa.
