Utilização da música no cuidado a crianças e adolescentes com câncer : estudo de casos múltiplos

Abstract

Resumo: Objetivo: Avaliar a influência da música no quadro de dor em crianças e adolescentes com câncer internados em uma unidade de oncopediatria. Metodologia: Trata-se de um estudo de casos múltiplos. Aprovada pelo Comitê de Ética em Pesquisa em Seres Humanos, do Setor de Saúde, da Universidade Federal do Paraná. Definiu-se a proposição do estudo com a questão norteadora: Qual a influencia da música no quadro de dor de crianças e adolescentes com câncer internados em uma unidade de oncopediatria? Participaram do estudo oito casos de crianças e adolescentes com dor assistidos em uma unidade de internação da oncopediatria. A coleta de dados ocorreu no período de fevereiro a agosto de 2016. Os dados foram coletados do prontuário físico e eletrônico por meio de formulário estruturado sobre o perfil sociodemográfico e clínico; a preferência musical por meio de questionário; e os dados da avaliação das variáveis dependentes e da variável independente por meio da observação direta. As variáveis dependentes foram: frequência cardíaca, frequência respiratória, pressão arterial, temperatura corporal e dor; e a variável independente: a intervenção musical. Para avaliação da dor foram utilizadas duas escalas, a Escala de Faces Revisada e a Escala de Faces, Pernas, Atividade, Choro e Consolabilidade. A intervenção musical consistiu na audição de músicas com fones de ouvido. A avaliação das variáveis dependentes ocorreu em três momentos: antes da audição musical, 5 e 60 minutos após o término da audição musical. Para a análise dos dados por meio de estatística descritiva, empregaram-se a distribuição de frequência, média, mediana, desvio padrão e porcentagem, avaliando-se a significância das variáveis e a qualidade do ajuste. Utilizou-se o programa R Core Team com nível de significância de 0,05. Resultados: Das oito unidades de análise com diagnóstico de Linfoma de BurKitt, Leucemia Linfoide Aguda, Tumor de Tronco Cerebral, Tumor Neuroectodérmico Primitivo, Teratoma imaturo de ovário e Linfoma Linfoblástico de Células T, cinco eram crianças, sendo duas do sexo masculino e três do sexo feminino, três eram adolescentes, um do sexo masculino e duas do sexo feminino com idades entre 6 e 15 anos. As variáveis dependentes sofreram alterações no decorrer do tempo da intervenção musical de p-valor >0,05, portanto não significativas, mas para SPF-R e FLACC, os resultados de p-valor foram <0,05, ou seja, apresentaram alterações significativas. A queixa de dor foi em decorrência da localização do tumor e das terapêuticas e procedimentos. As unidades de análise apresentaram quadro de dor aguda que variou de 1 a 5 dias, dos tipos "queimação", "ardência", "pontada" e "fisgada". Conclusão: Os parâmetros vitais sofreram variações durante e após o término da intervenção musical, a escolha da música pela preferência proporcionou um momento agradável e houve diminuição da dor conforme as escalas de avaliação utilizadas. A audição musical pode ser utilizada como uma intervenção de enfermagem no cuidado a crianças com câncer, é uma estratégia válida e indica possibilidades e vantagens com seu uso, podendo ser incluída aos cuidados e terapêuticas convencionais. Abstract: Objective: Evaluating the influence of music on pain in children and adolescents with cancer which are hospitalized in an oncopediatrics unit. Methodology: It is about a multiple case study. Approved by the Ethics Committee on Research in Human Beings, of the Health Sector, Federal University of Paraná. The study proposition was defined with the guiding question: What is the influence of the music the pain of children and adolescents with cancer hospitalized in an oncopediatrics unit? The study included eigth cases of children and adolescents with pain hospitalized in an oncopediatrics inpatient unit. Data collection occurred from February to August 2016. The data were collected from the physical and electronic records through a structured form on the sociodemographic and clinical profile; musical preference by means of a questionnaire; and the data of the evaluation of the dependent variables and the independent variable by means of direct observation. The dependent variables were: heart rate, respiratory rate, blood pressure, body temperature and pain; and the independent variable: musical intervention. Two scales were used to evaluate the pain, the Revised Face Scale and the Faces Scale, Legs, Activity, Cry and Consolability. The musical intervention consisted of hearing songs with headphones. The evaluation of the dependent variables occurred in three moments: before the musical hearing, 5 and 60 minutes after the end of the musical hearing. For the analysis of the data by means of descriptive statistics, was used the frequency distribution, mean, median, standard deviation and percentage, evaluating the significance of the variables and the quality of the adjustment. The R Core Team software was used with a significance level of 0.05. Results: Of the eight units of analysis with Burkitt's Lymphoma diagnosis, Acute Lymphoid Leukemia, Cerebral Trunk Tumor, Primitive Neuroectodermal Tumor, Immature Ovarian Teratoma and T-Cell Lymphoblastic Lymphoma, five were children, two males and three females, and three were adolescents, one male and two female aged between 6 and 15 years. The dependent variables underwent changes during the time of the musical intervention of p-value> 0.05, therefore, they were not significant, but for SPF-R and FLACC, p-value results were <0.05, that is, presented significant changes. The complaint of pain was due to the location of the tumor and the therapeutics and procedures. The units of analysis presented acute pain ranging from 1 to 5 days, of the types "burning", "blazing", "spike" and "hooked". It was observed that the vital parameters suffered variations during and after the end of the musical intervention, that the choice of the music by the preference provided a pleasant moment and that there was decrease of the pain according to the evaluation scales used. Musical hearing can be used as a nursing intervention in the care of children with cancer, it is a valid strategy and indicates possibilities and advantages with its use and can be included in care and conventional therapeutics.
