Níveis séricos de IGF-I e IGFBP-3 até os 90 dias de vida em crianças nascidas pequenas para a idade gestacional : relação com parâmetros antropométricos

Resumo: Crianças nascidas pequenas para idade gestacional (PIG) apresentam um risco aumentado para baixa estatura e anormalidades no eixo hormônio de crescimento-fator de crescimento insulina-símile (GHIGF- I). OBJETIVOS: avaliar as concentrações de IGF-I e da proteína ligadora de IGF (IGFBP-3) ao nascimento e com 48 horas de vida em recém-nascidos PIG e adequados para a idade gestacional (AIG) e sua correlação com o peso (PN) e talhe ao nascimento e com 48 horas de vida; além disto, as concentrações de IGF-I e IGFBP-3 até 90 dias de vida também foram avaliadas. PACIENTES E MÉTODOS: 161 recém-nascidos a termo (53 PIG, 22 meninos, PN -2,42+0,52 desvios padrões- DP e talhe -2,25±0,66 DP; 108 AIG, 50 meninos, PN -0,88±0,81 DP e talhe -0,59±0,74 DP) foram incluídos no estudo. Foram coletadas amostras de sangue do cordão umbilical e com 48 horas de vida. Em 22 crianças nascidas PIG e em 21 crianças nascidas AIG foram coletadas amostras de sangue com uma idade média de 62 dias de vida. O IGF-I foi avaliado por um método de radioimunoensaio (RIA) sem extração de proteínas e na presença de um excesso de IGF-II (Mediagnost GmbH, Tübingen, Alemanha) e o IGFBP-3 foi analisado por RIA previamente descrito (BLUM, BREIER, 1994). O nível médio de IGF-I ao nascimento foi menor no grupo PIG (44,3±18,8ng/mL) comparado com o grupo AIG (54±19,8ng/mL; P<0,01). Em contraste, não foram encontradas diferenças nos níveis de IGFBP-3 (1026,6±257,5 w 1019,5±266,9ng/mL). As concentrações de IGF-I com 48 horas diminuíram para 18,2±4,3ng/mL (diminuição de 55%) no grupo PIG e para 17,4±6,5ng/mL (diminuição de 63%) no grupo AIG, sem diferenças nos níveis de IGF-I entre os dois grupos. Uma diminuição semelhante foi encontrada nos níveis de IGFBP-3. Os níveis de IGF-I ao nascimento correlacionaram-se com o PN no grupo PIG (r-0,31; P<0,05) e no grupo AIG (r=0,29; P<0,01), mas não houve correlação com o talhe com 48 horas de vida. Nas crianças nascidas PIG que foram avaliadas até 90 dias de vida, foi encontrado um aumento percentual diário médio no peso de l,22±0,39%/dia e no talhe de 0,30±0,05%/dia, comparado com 0,87±0,44%/dia e 0,26±0"06%/dia no grupo AIG (P<0,01 e P<0,05, respectivamente), mostrando uma recuperação no crescimento nas crianças nascidas PIG. Neste momento, o nível médio de IGF-I aumentou para 48,63+19,83ng/mL e 39,62+23,53ng/mL nos grupos PIG e AIG, respectivamente. CONCLUSÕES: os níveis de IGF-I são menores em recém-nascidos PIG comparados com recém-nascidos AIG. Uma rápida diminuição nos níveis de IGF-I e de IGFBP-3 ocorreu após o nascimento em ambos os grupos provavelmente secundária ao jejum relativo durante os primeiros dias de vida, bem como a sua elevação até 90 dias de vida, com um maior aumento nas crianças nascidas PIG. Ao mesmo tempo, ocorreu uma recuperação do crescimento nestas crianças. Mais estudos são necessários para mostrar que estas mudanças podem estar relacionadas com a estatura final adulta. Abstract: Children bom small-for-gestational age (SGA) are of an increased risk of short stature and abnormalities in the growth hormone-insulin-like growth factor-I axis (GH-IGF-I). AIMS: to evaluate IGF-I and insulin-like growth factor binding protein (IGFBP-3) concentrations at birth and at 48 hours of life in newborns bom SGA and bom appropriate-for-gestational age (AGA) and the correlation with weight (BW) and length (BL) at birth and at 48 hours of life. In addition, IGF-I and IGFBP-3 concentrations until 3 months of age were also evaluated. PATIENTS AND METHODS: 161 fullterm newborns (53 SGA, 22 males, BW -2,42 ± 0,52 and BL -2,25 ± 0,66 SDS; 108 AGA, 50 males, BW -0,88 ± 0,81 and BL -0,59 ± 0,74 SDS) were included in the study. Samples from umbilical cord and at 48 hours of life were collected. In 22 children bom SGA and in 21 bom AGA, samples were also collected at mean of 62 days. IGF-I was measured by an IGFBP-blocked RIA without extraction and in the presence of an approximately 250-fold excess of IGF-II and IGFBP-3 was determined using a previously reported RIA method (Mediagnost GmbH, Tübingen, Germany; BLUM, BREIR, 1994). RESULTS: Mean IGF-I levels at birth was lower in the SGA (44,3 ± 18,8ng/mL) compared to the AGA group (54,0 ± 19,8ng/ml; p<0,01). In contrast, no difference was found in IGFBP-3 values (1026,6 ± 257-,5 vs 1019,5 ± 266,9ng/ml). IGF-I levels at 48 hours decreased to 16,2 ± 4,3ng/mL (55 % decrease) in the SGA group and to 17,4 ± 6,5ng/mL (63 % decrease) in the AGA group, with no further difference in IGF-I levels between the groups. Similar drop was found in IGFBP-3 levels. IGFI levels at birth correlated to BW in the SGA (r = 0,31; P < 0.05) and in the AGA (r = 0,29; P < 0.01) groups, but not with size at 48 hours. In those SGA children who were evaluated until 90 days of life, a mean increase in weight of 1,22±0,39 %/day and in height of 0,30±0,05 %/day was found, compared with 0,87±0.44 %/day and 0,26±0,06 %/day in the AGA group (P<0,01 and P<0,05, respectively), showing the catch-up growth in the SGA children, at this time, IGF-I levels increased to 48,63±19,83ng/mL and to 39,62±23,53ng/mL in the SGA and AGA children, respectively. CONCLUSION: IGF-I levels are lower in SGA newborns compared to AGA babies. A rapid drop in IGF-I and IGFBP-3 levels after birth occurs in both groups probably due to starvation during the first days of life. A further increase in IGF-I and IGFBP-3 levels occurs within 90 days of life in both groups, with higher increase in the SGA children. At the same time a catch-up growth occurs in the SGA children. Further studies are needed to show if these changes are related to final adult height.
