dc.description.abstractResumo: 350 exames broncofibroscõpicos foram realizados em 316 pacientes, sendo a atelectasia, a hemoptise e o nódulo ou massa intratorácicos as principais indicações do exame. 198 pacientes (62,65%) eram fumantes, cuja média de tabagismo foi de 41,40 maços/anos. 80 pacientes com tumor endoscopicamente visível (TEV) foram submetidos ã 80 biópsias, 20 lavados e 18 escovamentos brônquicos. 57 pacientes com carcinoma broncogênico endoscopicamente visível (CBEV) foram identificados: seu tabagismo médio foi de 49,14 maços/anos, e 52,63% deles tiveram contraindicação de tratamento cirúrgico determinado somente pelos achados endoscópicos. 56 pacientes com atelectasia foram examinados e em 98,22% deles as causas foram determinadas, sendo as mais freqüentes as secreções retidas, o carcinoma broncogênico e a inflamação. Dos 43 pacientes com hemoptise, a causa foi determinada em 67,44% e o local em 60,46%. Em 46 pacientes com nódulo ou massa intratorácicos, o exame permitiu o diagnóstico histológico em 65% deles. A morbidade associada ao exame foi de 6,3 2% e a mortalidade, de 0,3%. A análise destes e outros dados do estudo permite concluir que: 1) a intensidade do tabagismo é significativamente maior para os pacientes do grupo CBEV (índice ts2,41), e o coeficiente' de correlação entre idade e intensidade de tabagismo é desprezível e sem significância (r=0,14 - 0,13; N=57; P=NS); 2) o preço médio atual para aquisição de cigarros para cada paciente do grupo CBEV é de Cz$ 360.000,00; 3) a biópsia é superior ao lavado e escovamento - -2 bronquicos em eficacia diagnostica para o grupo TEV (x =18,63; P<0,001); 4) a relação masculino/feminino para o grupo CBEV é de 10,4:1 e 93% deles são fumantes; 5)atelectasia, hemoptise e nódulo ou massa intratorácicos são as indicações mais freqüentes do exame, e nestes últimos, o exame consegue determinar o tipo histológico da lesão em 65% dos casos; 6) a broncofibroscopia, por si só, contraindica tratamento cirúrgico em metade dos pacientes do grupo CBEV; 7) do exame decorrem baixa morbidade e mínima mortalidade.pt_BR
dc.description.abstractAbstract: 3 50 fiberoptic bronchoscopies were performed in 316 patients, in whom atelectasis, hemoptysis and an intrathoracic mass or nodule constituted the main indications - for the exam. 198 patients (62.65%) were smokers, and their smoking average was 41.40 packs/years. 80 patients with endoscopically visible tumors (EVT) were submitted to 80 bronchial biopsies, 20 washings and 18 brushings. 57 patients with endoscopically visible bronchogenic carcinomas (EVBC) were identified: their average smoking was 49.14 packs/years, and 52.63% of them had the contraindication to surgical treatment determined solely by the endoscopic findings. 56 patients with atelectasis were examined and in 98.22% of them the causes were determined. Retained secretions, bronchogenic carcinoma and inflammation were the most frequent causes. Among the 43 patients with hemoptysis, the cause was determined in 67.44% and the source of origin in 60.46% of them. In 46 patients with an intrathoracic mass or nodule, the examination allowed a histological diagnosis to be made in 65% of them. The morbidity associated with the examination was 6.32% and the mortality, 0.3%. The analysis of these and other data from the study allows one to conclude that: 1) the smoking average is significantly higher for the patients in the EVBC group (t=2.41) and the correlation coefficient between their ages and smoking averages is negligible and without significance (r=0.14 - 0.13; N=57; P=NS); 2) the updated mean price for the acquisition of cigarettes for every patient in the EVBC group is Cz$ 360.000,00; 3) bronchial biopsy, for the EVT group, is superior both to washing and brushing _2 as far as diagnostic efficacy is concerned (x =18.63; P<C0.001); 4) the male/female ratio for the EVBC group is 10.4:1 and 93% of the group are smokers; 5) atelectasis, hemoptysis and an intrathoracic mass or nodule are the most frequent indications for the exam, and in this latter group, fiberoptic bronchoscopy is able to determine the histological type of the lesion in 65% of the cases; 6) fiberoptic bronchoscopy alone contraindicates surgical treatment to half of the patients in the EVBC group; 7) the exam is associated with low morbidity and minimal mortality rates.pt_BR
