A vivência da equipe de enfermagem sobre a violência praticada por pacientes com transtorno mental

Abstract

Resumo: A violência tem íntima relação com a história da humanidade, se trata de fenômeno complexo e multifacetado, presente em todas as sociedades e, apresenta-se de diversas maneiras. Histórica e socialmente, foi construída a concepção de que as pessoas com transtorno mental são agressivas e violentas. Nos dias atuais, essa visão ainda persiste no imaginário social, inclusive para alguns profissionais de saúde. Esta pesquisa teve como questão norteadora: Qual a vivência da equipe de enfermagem, de um Pronto Atendimento, mediante a violência provocada pelo portador de transtorno mental em sua prática profissional? Para responder a essa questão foram elaborados os objetivos: Descrever a vivência do profissional, na prática da enfermagem, mediante a violência advinda da pessoa com transtorno mental; Analisar a violência sofrida pelo profissional na prática da enfermagem da violência advinda da pessoa com transtorno mental. Trata-se de pesquisa qualitativa e exploratória, realizada no período de 2010 a 2011, no Pronto Atendimento de um hospital geral de Curitiba, Paraná. Foram sujeitos desta pesquisa 38 profissionais da equipe de enfermagem: 05 enfermeiros, 05 técnicos e 28 auxiliares de enfermagem. Os dados foram obtidos mediante entrevista semiestruturada, registrada em aparelho digital e analisados de acordo com a Análise de Conteúdo Temático- Categorial. As categorias que emergiram foram: 1) A violência cometida pelo paciente com transtorno mental à equipe de enfermagem; 2) Sentimentos e atitudes da equipe de enfermagem em face ou não de violência sofrida. Obteve-se que a equipe de enfermagem sofre violência durante o cuidado ao paciente com transtorno mental, contudo em menor escala se comparado a pacientes em geral. Os resultados mostraram que dos 38 sujeitos, somente 05 tiveram vivência de violência cometida pelo paciente com transtorno mental, em toda a sua trajetória de prática na Enfermagem, que foi superior a três anos, chegando a vinte e oito anos. Na realidade estudada, pode-se considerar que a violência provocada por pessoas com transtorno mental é mais inferida do que real e é baseada no mito do indivíduo com transtorno mental como perigoso e violento. Isso porquanto, boa parte dos sujeitos, mesmo sem ter sofrido violência, referiram que sentem medo de prestar cuidado, produzindo discriminação e preconceito contra esse indivíduo. Acredita-se que assim como um mito é socialmente construído, nutrido e socialmente perpetuado, como o de pessoas com transtorno mental como violentas, da mesma forma este pode ser ressignificado. Para tanto, ressalta-se a necessidade do desenvolvimento e da divulgação de novas pesquisas sobre a temática.

Abstract: Violence has close relation with the history of mankind, it is complex and multifaceted phenomenon, present in all societies, and presents itself in several ways. Historically and socially, was built the idea that people with mental illness are violent and aggressive. Nowadays, this view still persists in the social imaginary, including some health professionals. This research had the guideline question: What is the experience of nursing staff, an emergency department, through violence caused by mental patients in his practice? To answer this question the goals were developed: Describe the experience of the professional practice of nursing through violence coming from the person with mental disorder; to analyze the violence suffered by the professional in the nursing practice in cases of violence coming from the person with mental disorder. It is qualitative and exploratory research conducted in the period 2010 to 2011 in the Emergency Department of a general hospital in Curitiba, Parana. The subjects of this study were 38 professional nursing staff: 05 nurses, 05 technicians and 28 nursing assistants. The data were obtained through a semistructured interview, recorded in digital device and analyzed according to Theme-Categorical Content Analysis. The categories that emerged were: 1) The violence committed by patients with mental disorders to the nursing staff; 2) Feelings and attitudes of nursing staff in front of, or not, the violence suffered. It was found that the nursing staff suffers violence during the treatment of patients with mental disorders, but on a smaller scale compared to patients in general. The results showed that among 38 subjects, only 05 had experience of violence committed by patients with mental illness throughout his career in nursing practice, which was more than three years, reaching twenty-eight years. In the reality studied, it can be consider that the violence caused by people with mental disorder is more implied than real and it is based on the myth of the individual with mental illness as dangerous and violent. This is because, most of the subjects, even without having experienced violence, reported that they are afraid to provide care, producing discrimination and prejudice against this individual. It is believed that as well a myth is socially constructed, socially nurtured and perpetuated, such as people with mental illness as violent, as this can be reframed. To this end, it emphasizes the need of the development and dissemination of new research on the subject.
