Efeitos de procedimentos focados na relação terapêutica sobre comportamentos geralmente descritos nos quadros de depressão

Resumo: A literatura analítico-comportamental indica que a alta freqüência de comportamentos de fuga/esquiva de estimulação aversiva, emitidos por clientes com depressão, somada à escassez de reforçamento positivo, têm por conseqüência um repertório social restrito. As relações interpessoais estabelecidas com esses clientes podem tornar-se aversivas, gerando contexto para afastamento das pessoas com quem eles interagem. As psicoterapias analítico-comportamentais contemporâneas propõem que intervenções com foco na relação terapêutica podem gerar o restabelecimento de relações de intimidade, confiança, entre outras, favorecendo modificação dos repertórios sociais restritos e, possivelmente, a melhora da depressão. O presente estudo teve por objetivo a análise da relação terapêutica estabelecida entre terapeuta e clientes que apresentam comportamentos geralmente descritos nos quadros de depressão. Buscou-se avaliar os efeitos de intervenções com foco no aqui/agora, privilegiando resposta aos CRBs, em dois casos clínicos de clientes com depressão. Para tal investigação, optou-se por um delineamento experimental de sujeito único A-B, de linha de base múltipla inter-sujeitos. Na Fase A, a terapeuta não deveria responder aos comportamentos clinicamente relevantes das clientes, e deveria manter o foco apenas nos relatos de comportamentos-problema e de melhora que ocorressem fora do contexto da sessão terapêutica. Na Fase B, a terapeuta deveria priorizar intervenções com foco no aqui/agora da sessão, privilegiando resposta aos CRBs. Foram filmadas 29 sessões de psicoterapia com duas clientes que apresentaram, inicialmente, níveis de depressão moderada e grave, segundo o Inventário de Depressão de Beck (BDI). As sessões foram categorizadas semanalmente com a utilização da FAPRS (Funcional Analytic Psychotherapy Rating Scale), e os comportamentos críticos do quadro de depressão foram avaliados segundo a observação da terapeuta. O conjunto das medidas obtidas pelo estudo indicou que, após a introdução de intervenções com foco no aqui/agora, privilegiando resposta aos CRBs, a freqüência relativa de CRB1 diminuiu, assim como a de CRB2 aumentou. Ambas as clientes apresentaram melhora dos comportamentos geralmente descritos nos quadros de depressão após a introdução da intervenção. Foi possível verificar que a relação terapêutica esteve sensível à variáveis intervenientes à pesquisa, e que manter o foco no aqui/agora foi dificultoso com esse tipo de cliente. Abstract: The behavior-analytic literature indicates that high frequency of avoidance and escape behaviors from aversive stimulation, emitted by clients with depression, added to a lack of positive reinforcement, has as a consequence a restricted social repertoire. Interpersonal relationships established with these clients can become aversive, generating a context that distances people with whom they interact. The contemporary behavior-analytic psychotherapies propose that interventions focused on the therapeutic relationship can generate the reestablishment of intimate and trusting relationships, promoting among others change of the restricted social repertoires and possibly the improvement of depression. This study aimed to analyze the therapeutic relationship established between therapist and clients who present behaviors usually described in cases of depression. It was sought to evaluate the effects of interventions focusing on the here/now, privileging responses to the CRBs, in two clinical cases of clients with depression. For such investigation, a within-subject multiple baseline design A-B was chosen. In Phase A, the therapist should not respond to the client’s clinical relevant behaviors, and should focus only on the report of problem-behaviors and of improvement which occurred out of the therapeutic session context. In Phase B, the therapist should prioritize interventions focusing on the session’s here/now situations, privileging response to the CRBs. Twenty nine psychotherapy sessions have been video taped with two clients who presented moderate and severe depression cases, according to the Beck’s Depression Inventory (BDI). The sessions have been categorized weekly using the FAPRS (Functional Analytic Psychotherapy Rating Scale), and the critical behaviors of the depression cases were assessed through the therapist’s observation. The results indicate that after the introduction of interventions with the focus on here/now, privileging the responses to the CRBs, the relative frequency to the CRB1 has reduced, as well as the CRB2 increased. After the intervention, both clients presented improvement of behaviors usually described in cases of depression. It was possible to verify that the therapeutic relationship was sensitive to variables intervening in the research, and that keeping the focus on the here/now was difficult with this type of clients.
