Influência da desnutrição intra-uterina na cicatrização da parede abdominal de ratos lactentes avaliada mediante estudo tensiométrico e da morfometria do colágeno

Resumo: Com o objetivo de avaliar os efeitos da desnutrição intra-uterina sobre cicatrização de parede abdominal dos filhotes foram avaliados ratos recém-nascidos após o desmame. As ratas prenhas foram divididas em dois grupos, um submetido à desnutrição no período gestacional, mediante restrição dietética (grupo D) e outro, à nutrição adequada como controle (grupo N). Após o desmame aos 21 dias de vida foi realizada laparotomia transversa em todos os ratos. Subseqüentemente, cada grupo foi dividido em dois subgrupos, de acordo com o dia de morte pós-operatória dos animais, no 7º dia, subgrupos N7 e D7, ou 21o dia, subgrupos N21 e D21. As ratas prenhas foram acompanhadas quanto ao peso durante a gestação e lactação e tiveram sua albumina sérica mensurada ao final da gestação. Os filhotes tiveram seu peso, comprimento do corpo e da cauda medidos a cada sete dias no período de lactação, pós-desmame e nas datas de morte. A força máxima, força de ruptura e a tensão das cicatrizes foram mensuradas em máquina de tração computadorizada. A espessura da camada muscular, a espessura e largura da cicatriz cirúrgica e a contagem das células na área de cicatrização foram estudadas na coloração hematoxilina-eosina. A morfometria do colágeno foi avaliada mediante coloração picrosírius sob luz polarizada e forneceu dados para o cálculo de um índice de maturação do colágeno. O estudo comparou estas variáveis entre si, través dos testes "t de Student" e Modelos Hierárquicos, em cada momento previamente determinado. Os resultados mostraram que as ratas prenhas desnutridas ganharam menos peso durante o período estacional que as ratas nutridas (p <0,0001); a mensuração da albumina sérica materna ao final da gestação apresentou valores maiores nas ratas nutridas (p < 0,0003); o peso (p = 0,0003) e o comprimento do corpo (p = 0,0003) dos ratos desnutridos intra-útero foram significativamente menores ao nascimento, com recuperação destes na data do desmame (p = 0,0605 e p = 0,5599, respectivamente). As cicatrizes abdominais nos ratos nutridos suportaram força de ruptura superior ao grupo de ratos desnutridos intraútero no 7º dia de pós-operatório (p = 0,0495) e similar no 21º dia (p = 0,7199); a força máxima mensurada no grupo dos nutridos foi superior apenas aos sete dias (p = 0,0061); a tensão apresentou significância com predomínio dos animais nutridos sobre os desnutridos aos 7 (p = 0,0503) e 21 dias (p=0,0477) . Nenhum dos parâmetros anatomopatológicos estudados mostrou diferença entre si nos momentos avaliados. A morfometria do colágeno demonstrou semelhança nos momentos estudados (p = 0,3187) para o colágeno tipo I aos 7 dias e aos 21 dias de pós-operatório (p = 0,4212); a deposição do colágeno tipo I aumentou entre os dias 7 e 21 de modo significativo.Concluiu-se que a desnutrição materna afeta a resistência à tração das cicatrizes cirúrgicas dos filhotes no pós-operatório precoce (7º dia). Abstract: In order to study the effects of intrauterine malnutrition on the healing of the abdominal wall, weaning newborn rats were assessed. The pregnant rats were divided into two groups, one of the groups underwent malnutrition during pregnancy (group D) and the other received adequate nourishment to be later used as control (group N). The baby rats were weaned at 21 days and the transverse laparotomy was performed. Each group was divided into two subgroups, in accordance with the date when the surgical scar was analyzed, on the 7th , subgroups N7 and D7, or 21st day of the postoperative period, subgroups N21 and D21. The weight of the pregnant rats was observed during the pregnancy and lactation and their albumin serum was measured at the end of pregnancy. The weight, body length and tail length of the baby rats were measured every seventh day during lactation, weaning and at the death day. Automated traction device assessed the maximum strength, rupture strength and tensile strength of the scars. The thickness of the muscular layer, thickness and width of the surgical scar and counting of cells in the healing area were studied using hematoxylin-eosin staining. Collagen morphometry was assessed using picrosirius staining under polarized light. The study compared these variables, one against the other, at every time point previously set by statistical methods. The results shown that weight gain among the pregnant rats, under alnutrition, was reduced during pregnancy (p <0,0001); albumin serum measured at the end of the pregnancy was higher in the properly nourished rats (p < 0,0003); weight (p = 0,0003) and body length (p = 0,0003) of the rats under malnutrition were significantly smaller at birth, with recovery at the weaning date (p = 0,0605 e p = 0,5599, respectively); body length gain for the animals under malnutrition was statistically higher during lactation (p = 0,0410). The abdominal scars, in the properly nourished group, endured a rupture strength higher on the 7th day of the postoperative period (p = 0,0495) and was similar to the one seen in the group under malnutrition group at the 21st day of the post surgical period (p = 0,7199); the maximum strength measured in the well nourished group was significantly superior at the 7th day (p = 0,0061); the tensile strength presented marginal significance with the adequately nourished animals predominating at all times, on the 7th (p = 0,0503) and 21st day (p = 0,0477) . No studied anatomopathological parameter has shown differences between each other at the evaluated time points. Collagen morphometry was similar at the studied times (p = 0,3187) for collagen type I on the 7th day and on the 21st day of the post surgical period (p = 0,4212); collagen type I deposition significantly increased between the 7th and the 21st day. As conclusion, intra-uterine malnutrition affects the abdominal wound strength of weaning rats in early postoperative (7th day).
