Da autodestrutividade ao circuito pulsional : correlações com o trabalho em saúde mental

Resumo: Partindo da prática clínica em um Centro de Atenção Psicossocial, a presente dissertação teve como objetivo investigar a dinâmica envolvida na repetição de comportamentos autodestrutivos, observada no atendimento de pacientes em sofrimento psíquico intenso. As queixas associadas a esse fenômeno indicavam a presença simultânea de um alívio e um sofrimento intenso face o potencial destrutivo aí envolvido. A pesquisa se propôs a indagar, a partir da obra freudiana, o que estaria dando sustentação a essa repetição, considerando a hipótese de que esse mecanismo faz parte da estrutura psíquica, relacionado ao circuito pulsional. Seguindo o método psicanalítico de investigação, com o intuito de buscar respostas na teoria para os questionamentos advindos da clínica, o primeiro capítulo da dissertação objetivou explanar o modo de funcionamento e o contexto histórico, social e político em que está inserido o CAPS, destacando as reformas psiquiátricas e a Rede de Atenção Psicossocial. No segundo capítulo, centralizado nos artigos pré-psicanalíticos de Freud, o estudo se debruçou sobre a investigação da construção da teoria pulsional. A partir da clínica da histeria, destacamos a formulação das questões energéticas do funcionamento psíquico como pilar para a formalização do conceito de pulsão. Também evidenciamos a concepção do princípio de constância, do prazer/desprazer e os trilhamentos energéticos. No terceiro capítulo, foram percorridos o primeiro e segundo dualismo pulsional, evidenciando os conflitos entre as pulsões sexuais e de autoconservação; entre as pulsões de vida e de morte; bem como ressaltamos as noções da primeira e segunda tópica. Abordamos, ainda, os destinos pulsionais, destacando o papel do sadomasoquismo na dinâmica dos comportamentos autodestrutivos. No quarto capítulo objetivamos retornar à clínica e, para isso, separamos dois eixos norteadores: no primeiro detalhamos as interrogações da prática psicanalítica no CAPS. No segundo, destacamos três situações clínicas, a adicção, os transtornos alimentares e as tentativas de suicídio/automutilação, ressaltando o conflito entre a satisfação e o sofrimento na busca pelo escoamento pulsional. Esse percurso teórico possibilitou a conclusão de que os comportamentos autodestrutivos estão ligados à dinâmica pulsional, envolvendo preponderantemente uma das vicissitudes pulsionais, o sadomasoquismo. Esse destino pulsional está ancorado no embate entre as pulsões de vida e de morte. Devido a isso, prevalece uma impossibilidade em renunciar às atitudes autodestrutivas, pela satisfação intensa que elas proporcionam, mesmo que posteriormente sobrevenha um sofrimento igualmente intenso. Abstract: Starting from the clinical practice in a Psychosocial Care Center, this dissertation aimed to investigate the dynamics involved in the repetition of self-destructive behaviors, observed in the care of patients in intense psychological distress. The complaints associated with this phenomenon indicated the simultaneous presence of relief and intense suffering in the face of the destructive potential involved. The research proposed to investigate, from the Freudian work, what would be supporting this repetition, considering the hypothesis that this mechanism is part of the psychic structure, related to the pulsional circuit. Following the psychoanalytic method of investigation, in order to seek answers in theory to the questions arising from the clinic, the first chapter of the dissertation aimed to explain the mode of operation and the historical, social and political context in which the CAPS is inserted, highlighting the psychiatric reforms and the Psychosocial Care Network. In the second chapter, centered on Freud's pre-psychoanalytic articles, the study focused on the investigation of the construction of the drive theory. From the hysteria clinic, we highlight the formulation of energetic issues of psychic functioning as a pillar for the formalization of the concept of drive. We also highlight the conception of the principle of constancy, pleasure/displeasure and the energetic trails. In the third chapter, the first and second drive dualism were covered, highlighting the conflicts between sexual and self-preservation drives; between life and death drives; as well as highlighting the notions of the first and second topic. We also address the drive destinations, highlighting the role of sadomasochism in the dynamics of self-destructive behaviors. In the fourth chapter we aim to return to the clinic and, for this, we separate two guiding axes: in the first we detail the questions of psychoanalytic practice in CAPS. In the second, we highlight three clinical situations, addiction, eating disorders and suicide/automutilation attempts, emphasizing the conflict between satisfaction and suffering in the search for pulsional flow. This theoretical path made it possible to conclude that self-destructive behaviors are linked to drive dynamics, involving predominantly one of the drive vicissitudes, sadomasochism. This drive destiny is anchored in the clash between the life and death drives. Because of this, it is impossible to renounce self-destructive attitudes, due to the intense satisfaction they provide, even if later an equally intense suffering ensues.
